[Drug-associated osteonecrosis of the jaw with oral bisphosphonates in osteoporosis - periodontal diseases also harbor risks].

During treatment with bisphosphonates (peroral or intravenous), there is a risk of osteonecrosis, "medication-related osteonecrosis of the jaw" (MRONJ), as a direct consequence of oral surgery or due to chronic infections such as periodontitis or peri-implantitis, particularly with high-dose medication in malignant patients. Necrosis of the jawbone has also been described with oral bisphosphonates in osteoporosis.

Diseases having an influence on inhibition of angiogenesis as risk factors of osteonecrosis of the jaw.

OBJECTIVES: The objective of this study was to retrospectively investigate the association of diseases having an influence on inhibition of angiogenesis such as hypertension, diabetes mellitus type II, hypercholesterolemia, and rheumatoid arthritis (RA) with the development of osteonecrosis of the jaws. MATERIALS AND METHODS: The 135 patients were allocated into 4 groups of bisphosphonate-related osteonecrosis of the jaw (BRONJ) group (1A); non-BRONJ group (1B); osteonecrosis of the jaw (ONJ) group (2A); and control group (2B), according to histologic results and use of bisphosphonate. [PROGNOSTICATION OF "HARD" TRACHEAL INTUBATION IN PATIENTS, SUFFERING PURULENT-INFLAMMATORY DISEASES OF THE JAW-FACIAL REGION].

Clinical follow-up was conducted in 75 patients, to whom the operation and intensive therapy were performed in 2006-2015 yrs for phlegmon of the neck. Difficulties while providing passability of respiratory ways have occurred in 73.3% patients. It was established, that the predictors of "hard" tracheal intubation in patients, operated on for cervical phlegmon, are follows: the cervical tissues oedema, restriction of the mouth opening lesser than 3 cm, odontogenous phlegmon of oral cavity and the neck, rigid epiglottis, the larynx oedema. Presence of these factors demands application of endosco

Incidence of bisphosphonate-related osteonecrosis of the jaw in consideration of primary diseases and concomitant therapies.

BACKGROUND: Since its first description by Marx in 2003, the etiology of bisphosphonate-related osteonecrosis of the jaw (BRONJ) is the subject of numerous scientific discussions for oral and maxillofacial surgeons. Many retrospective studies on its etiology and pathogenesis have been carried out to explain pathological mechanisms; most of them just take a close look at the issue of dosage and application. Recently, attempts have been made, to identify co-factors which might promote the development of BRONJ. PATIENTS AND METHODS: The present study is based on data of 169 patients with osseous

Quantification by energy dispersive x-ray spectroscopy of alendronate in the diseased jaw bone of patients with bisphosphonate-related jaw osteonecrosis.

OBJECTIVE: Recently, specific in vitro bisphosphonate concentrations have been established for reaching a toxic threshold that could result in the induction of bisphosphonate-related osteonecrosis of the jaw (BRONJ), but these data have not been validated in vivo. The purpose of this study was to quantify the concentration of bisphosphonates (BPs) in the diseased jaw bone of patients experiencing BRONJ. STUDY DESIGN: We hypothesized that if the average natural nitrogen content of mammalian bone is known, the excess of nitrogen in the jaw bone of BRONJ patients is likely to reflect the concent

Challenged by the jaw - an interview study on patients' experiences of temporomandibular disorders in rheumatic inflammatory diseases.

PURPOSE: Rheumatic inflammatory diseases affecting the temporomandibular joint and the masticatory system (TMD) have been described as painful and limiting. However, the condition is often overlooked in primary care. The objective of this qualitative study was to explore and describe TMD-related experiences and perceptions of persons with rheumatic inflammatory disease, and to put this into a rehabilitation perspective. MATERIALS AND METHODS: Seven participants with rheumatic inflammatory disease and concomitant TMD were interviewed using a semi-structured interview guide. Giorgi's phenomenol

Paget's disease of the jaws: Histopathological features of a series of 31 cases.

The aim of the present study was to analyze the histopathological features of Paget's disease of the jaws observed in a series comprising 31 cases. The study comprised all cases of Paget's disease of the jaws filed in the archives of the Surgical Pathology Laboratory of the Oral Pathology Department, School of Dentistry, University of Buenos Aires, between 1960 and 2018. Their microscopic features were evaluated, and available clinical data and radiographic studies were analyzed. Paget's disease of the jaws accounted for 0.05% of retrieved oral-maxillofacial pathologies. Microscopically, all c

RANKL inhibitors induce osteonecrosis of the jaw in mice with periapical disease.

Antiresorptive medications are essential in treating diseases of pathologic osteoclastic bone resorption, including bone cancer and osteoporosis. Bisphosphonates (BPs) are the most commonly used antiresorptives in clinical practice. Although inhibition of bone resorption is important in regulating unwanted malignant and metabolic osteolysis, BP treatment is associated with potential side effects, including osteonecrosis of the jaws (ONJ). Recently, non-BP antiresorptive medications targeting osteoclastic function and differentiation, such as denosumab, have entered the clinical arena. Denosuma

[Paget disease of the jaws. Review of the subject. Clinical-therapeutic considerations].

Clinical and diagnostic findings of Paget's disease of the jaws are reported; up to date etiological and pathogenical hypotheses are presented. Laboratory data of increased P.A. as a marker of disease evolution is pointed out, particularly the relationship increasing hydroxyprolinuria, because the increase of a malignant transformation. Surgery, always modeling, is limited to cases of severe facial dysmorphosis associated or not with the local disturbance due to compression; in any case it is suggested to perform the surgery in the phases of inactivity of disease, to avoid local complications,

Erdheim-Chester Disease of the Jaws: A Case Report and Review of the Literature.

Erdheim-Chester disease (ECD) is a rare non-Langerhans cell histiocytic disease characterized by xanthogranulomatous infiltration of multiple organ systems by histiocytes that are negative for langerin (CD207) and CD1a. BRAF V600E mutation is present in 54% of the cases. The disease most commonly affects males in their 5th and 7th decades and bone is involved in 96% of cases although there have been only 8 reported cases of ECD involving the jaws. This is a case of ECD in a 57-year-old male who presented for extractions and implant placement. Radiographic examination revealed extensive bone lo

A Rare Sporadic Case of Camurati-Engelmann Disease With Jaw Involvement.

Camurati-Engelmann disease (CED), or progressive diaphyseal dysplasia, is an uncommon bone dysplasia that is inherited in an autosomal-dominant pattern. The disease mainly affects the diaphyses of the long bones but can induce sclerotic changes to the facial skeleton and skull base. The diagnosis of CED is based on clinical and radiologic features. This article presents the clinical and radiologic characteristics of the jaws as visualized on cone-beam computed tomograms of a 46-year-old woman diagnosed with CED.

[Paget´s disease of the mandible: a differential diagnosis of the osteomyelitis of the jaw].

Paget´s disease of bone is a common, progressive disorder with uncertain etiology. It results in the replacement of normal skeletal areas with highly vascularized, low density bone. The most frequent symptom is bone pain, which is a result of periostal irritation due to increased metabolic activity. However, the disease may run without any symptoms. In these cases, an abnormal radiograph or an elevated serum alkaline phosphatase can indicate the disorder. Most cases are discovered accidentally. The average age of patients at the time of diagnosis is about 50 years. Male individuals are prefere

Periodontal disease as a risk factor for bisphosphonate-related osteonecrosis of the jaw.

BACKGROUND: Previous case reports and animal studies suggest that periodontitis is associated with bisphosphonate-related osteonecrosis of the jaw (BRONJ). This case-control study is conducted to evaluate the association between clinical and radiographic measures of periodontal disease and BRONJ. METHODS: Twenty-five patients with BRONJ were matched with 48 controls. Trained examiners measured probing depth, clinical attachment level (CAL), and bleeding on probing on all teeth except third molars and gingival and plaque indices on six index teeth. Alveolar bone height was measured from orthop
