Relationship between oral condition and risk factors for jaw osteonecrosis in patients with hip fractures.

Purpose : Antiresorptive agents, such as bisphosphonates, are useful for the prevention of the recurrence of hip fractures. However, their administration has a risk of antiresorptive agent-related osteonecrosis of the jaw (ARONJ), and risk factors include poor oral hygiene. It is difficult for an orthopedic surgeon to examine a patient's oral condition thoroughly. This study evaluated the relationship between risk factors for ARONJ and intraoral findings in hip fracture patients. Materials and Methods : We evaluated 79 patients (average age of 82.2 years) with hip fracture surgery who underwen

Osteonecrosis of the Jaw and Concomitant Atypical Femoral Fractures with Bisphosphonates: A Comprehensive Literature Review.

Bisphosphonates are the first line of treatment for osteoporosis. Complications of bisphosphonates, such as osteonecrosis of the jaw (ONJ) and atypical femoral fractures (AFF), have been reported in the medical literature. Concomitant occurrence of both of these complications is very rare. In this review article, we will discuss the etiology, pathogenesis, and clinical studies, as well as case studies and their management per the latest clinical guidelines.

Concurrent bisphosphonate-related bilateral atypical subtrochanteric fractures and osteonecrosis of the jaw on bone scintigraphy.

An 82-year-old woman presented with bilateral thigh pain. She had pain in her right thigh operated for a low-trauma fracture 2 years earlier and newly developed pain in her left thigh without trauma. A whole-body bone scan revealed increased tracer uptake in her bilateral subtrochanteric femoral shafts and in the right mandible without evidence of metastatic bone disease. She had been taking bisphosphonates for 7 years to treat osteoporosis and was soon diagnosed with atypical subtrochanteric fractures and bisphosphonate-related osteonecrosis of the jaw. The bone scan simultaneously identified

Diagnosis of jaw and dentoalveolar fractures in a traumatized patient with cone beam computed tomography.

The purpose of this case report is to discuss and illustrate the clinical usage of Cone Beam Computed Tomography (CBCT) for the diagnosis of maxillofacial fractures in a traumatized patient. In this presentation, a 30-year-old male patient who was referred to Oral Diagnosis and Radiology Department with a limitation of mouth opening was reported. The history of the patient revealed a traumatic injury on his face because of a fall. The patient was initially examined by a medical practitioner in the emergency department of a public hospital. According to 2D cephalometric analysis, no fracture ex

[Risk of jaw osteonecrosis and atypical femoral fracture: how to inform patients with osteoporosis?].

Amino-bisphosphonates (BP), such as zoledronate, alendronate, ibandronate or risedronate, and the antibody therapies with denosumab (DMAb) or romosozumab (ROMO) are highly effective therapies for reducing the risk of vertebral fractures and non-vertebral fractures in patients with osteoporosis. Generally very well tolerated, these antiresorptive therapies have an association with bone-specific adverse events such as osteonecrosis of the jaw (ONJ) and atypical femoral fractures (AFF). This association leads to uncertainty among patients and treating physicians as to how the benefit-risk should

Applications of Maxillomandibular Fixation, Occlusal Guidance, and Jaw Physiotherapy in the Management of Fractures of the Mandible.

The goal of mandibular fracture management is to restore form and function. Maxillomandibular fixation, elastic occlusal guidance, and postoperative physiotherapy are essential elements to optimizing outcomes. Restoration of premorbid occlusion is paramount. Thus, an expert understanding of occlusion, coupled with the application of maxillomandibular techniques to achieve bony reduction with idealized dental occlusion, is required in the proper management of mandible fractures. Postoperatively, complete recovery initially requires elastic occlusal guidance followed by jaw range of motion physi

Pathological fracture of the coronoid process secondary to medication-related osteonecrosis of the jaw (MRONJ).

INTRODUCTION: Medication-related osteonecrosis of the jaw (MRONJ) is a growing problem within the field of oral and maxillofacial surgery. It is defined as the presence of exposed necrotic alveolar bone that does not resolve over a period of 8 weeks in a patient taking bisphosphonates, who has not had radiotherapy to the jaw [1]. Since the first report in 2003 that highlighted the potential harm caused by MRONJ, many more patients have been diagnosed with the condition [2]. The growth in recent years is likely due to the more potent drugs delivered intravenously however there is some evidence

Atypical femoral fracture combined with osteonecrosis of jaw during osteoporosis treatment with bisphosphonate.

Bisphosphonate, a potent anti-resorptive agent, is generally accepted as a safe, effective, well tolerated treatment for postmenopausal osteoporosis. Atypical femoral fracture (AFF) and bisphosphonate related osteonecrosis of jaw (BRONJ) are the increasing morbidities in patients treated with long term bisphosphonate. Pathogenic mechanisms of AFF and BRONJ are not fully identified and not identical. We report a case of BRONJ followed by AFF and its nonunion in a 67-year-old woman patient receiving an oral bisphosphonate during 7 years for the treatment of osteoporosis.

Impaired Callus Formation in Pathological Mandibular Fractures in Medication-Related Osteonecrosis of the Jaw and Osteoradionecrosis.

PURPOSE: Nonsurgical treatment of mandibular fractures secondary to medication-related osteonecrosis of the jaw (MRONJ) or osteoradionecrosis (ORN) mostly results in nonunion, whereas nonsurgical fracture treatment of atrophic fractures can achieve favorable results in selected cases. The aim of this study was to compare callus formation in pathological mandibular fractures due to MRONJ, ORN, or extreme mandibular atrophy. METHODS: A retrospective cohort study reviewing the medical records of all MRONJ-, ORN-, or atrophy-related fractures treated at the departments of maxillofacial surgery in

Jaw fractures in Stockholm 1988-90. Changing trends in injury dynamics over a 10 year interval.

This paper presents a review of injury dynamics of jaw fractures sustained by 1374 patients in the county of Stockholm during 1978-1980 = M1 (795 patients) and 1988-1990 = M2 (579 patients). The material comprised 1620 mandibular (M1 = 904, M2 = 716) and 439 maxillary (M1 = 249, M2 = 190) fractures. Forty-three per cent of the mandibular fractures and 40% of the maxillary fractures were single fractures in M1. The corresponding figures for M2 were 35% and 43%. During both periods the anatomical region most frequently fractured was the mandibular condyle (M1 = 35%, M2 = 29%). Violence was the c

Atypical femoral fractures shortly after osteonecrosis of the jaw in a postmenopausal woman taking alendronate for osteoporosis.

CONTEXT: Bisphosphonates effectively increase bone mineral density and reduce fracture risk in patients with osteoporosis, but there are concerns about osteonecrosis of the jaw (ONJ) and atypical femoral fractures (AFFs) in the long-term users. So far both complications have not been reported as occurring simultaneously in an osteoporotic individual on oral alendronate. OBJECTIVE: The aim of this study was to report a postmenopausal woman presenting with concomitant ONJ and AFF on oral alendronate treatment. SUBJECT, MEASURES, AND RESULT: The patient was a 63-year-old woman with a history of

Interdental wiring in jaw fractures: effects on teeth and surrounding tissues after a one-year follow-up.

Several authors have suggested that interdental wiring may have an adverse effect on the teeth and surrounding tissues. In the present study we examined 250 teeth that had been subjected to wiring and 142 adjacent teeth that had not been wired (controls). During the wiring period, and after a minimum of 1 year following treatment of the jaw fracture, the teeth and periodontal tissues were examined by periodontal probing, percussion sound test, mobility test, sensitivity test, and radiography. One tooth showed temporary increased mobility. Periodontal health was clinically and radiologically fu

Bilateral iatrogenic fracture of the jaw during the removal of third molars: Rare case report.

Mandibular third molar (M3) removal is a routine surgical procedure that can generate a variety of complications, one of the rarest of which is mandibular fracture. The aim of this paper is to report a rare case of bilateral fracture of the jaw during the extraction of mandibular third molars. A 42-year-old woman complained of mandibular pain and malocclusion after the removal of the M3s. The physical examination revealed bone crepitation mandibular bilaterally. Computed tomography confirmed bilateral fracture of the jaw. The patient was submitted to the reduction and fixation of the fractures

Diagnosis and Management of Atypical Femoral Fractures and Medication-Related Osteonecrosis of the Jaw in Patients with Osteoporosis.

Anti-osteoporosis treatments reduce fracture risk but maybe associated with rare adverse events with long-term use such as atypical femoral fractures (AFFs) and medication-related osteonecrosis of the jaw (MRONJ). AFFs are rare but more likely with prolonged bisphosphonate use, whereas MRONJ incidence is higher in cancer patients on high-dose antiresorptive therapy. Following diagnosis, effective treatment options are available to manage both of these rare complications. An individualized treatment approach is advised with close monitoring.

Osteomyelitis of the jaw (with pathological fracture) following extraction of an impacted wisdom tooth. A case report.

Osteomyelitis is an infection and inflammation associated with the bone structures: bone marrow, cortical bone, periosteum, blood vessels and nerves. Although it does not have a frequent presentation, it can sometimes lead to complications such as pathological fractures or even septicemia. We present a clinical case study, to assess the relationship between osteomyelitis of the jaw and pathological fractures, after the extraction of an impacted wisdom tooth. This case highlights a rare complication following the surgical removal of mandibular third molar; a slow evolution of the pathology foll
