Ultrasound-guided aponeurotomy and interphalangeal joint capsular release for treatment of Dupuytren's disease.

This study describes a new ultrasound-guided surgical technique for aponeurotomy and interphalangeal joint capsular release in patients with Dupuytren's disease and analyses the clinical outcomes. We carried out a retrospective review of 70 digits in 35 patients who underwent ultrasound-guided aponeurotomy and interphalangeal joint capsular release, with a minimum follow-up of 2 years. The primary outcome was the correction of the deformity and the QuickDASH questionnaire score after surgery and at 1 and 2 years. The secondary outcome was the presence of residual contracture immediately after

Failure after soft-tissue release with tendon transfer for flexible iatrogenic hallux varus: a systematic review.

Hallux valgus is a common forefoot pathology often requiring surgical intervention for symptomatic relief. One complication of hallux valgus correction is flexible hallux varus. Iatrogenic flexible hallux varus often requires surgical repair; however, the most advantageous surgical procedure for repair of iatrogenic flexible hallux varus and their sustainability remains unclear. Therefore, we performed a systematic review to determine the sustainability of soft-tissue release with tendon transfer for the correction of iatrogenic flexible hallux varus. Studies were eligible for inclusion only i

[Deformity-associated treatment of the hallux valgus complex].

Hallux valgus represents a combined deformity with malpositioning of the big toe in the metatarsophalangeal joint and metatarsal splaying due to metatarsus primus varus formation. It is defined on the basis of joint condition of the metatarsophalangeal and tarsometatarsal (TMT) joints, the extent and congruence or incongruence of malposition, mobility of the metatarsophalangeal joint and TMT stability. Basic resection appears to be indicated only in exceptional cases. Depending on the degree of severity, deformities can be corrected by means of distal, diaphyseal or proximal osteotomies and TM

Arthroscopic findings after manipulation under anesthesia in idiopathic capsulitis of the shoulder: A prospective study.

BACKGROUND: Manipulation under anesthesia (MUA) of the shoulder joint is a commonly used method for the treatment of adhesive capsulitis. Though it has been known to be associated with a variety of complications, there is a paucity of studies describing the arthroscopic findings after MUA. AIM: To describe the arthroscopic findings in patients with idiopathic adhesive capsulitis of the shoulder after MUA. METHODS: We recruited 28 patients with idiopathic adhesive capsulitis who underwent arthroscopic capsular release. Manipulation of the shoulder was performed under anesthesia in all of thes

Arthroscopic release of shoulder internal rotation contracture in children with brachial plexus birth palsy.

In children with brachial plexus birth palsy, the unopposed contraction of the shoulder internal rotators and adductors, secondary to weakness of the external rotators and abductors, leads to internal rotation contracture of the shoulder joint. Latissimus dorsi and/or teres major tendon transfers combined with open musculotendinous lengthening can improve shoulder function. Arthroscopic release of the anterior glenohumeral ligaments, capsule and upper intra-articular subscapularis tendon, with or without tendon transfer(s), can also be performed in young children to restore external rotation a

[Performance control after arthroscopic arthrolysis with capsulectomy in fresh-frozen elbow joints].

BACKGROUND AND OBJECTIVE: Posttraumatic or postoperative movement restrictions in elbow joints can often occur (including capsular contracture) and can generate everyday limitations. In persistent elbow stiffness, arthroscopic arthrolysis with removal of the dorsal and ventral capsule portions can be carried out. The purpose of this study was to assess the efficacy of arthroscopic capsulectomy by means of an in vitro anatomical study. METHODS: A standardized elbow arthroscopy with ventral and dorsal capsulectomy was performed and image-documented in five fresh-frozen elbow specimens. Subseque

Reconstruction of the hypoplastic thumb.

Thumb hypoplasia is characterized by diminished thumb size, metacarpal adduction, metacarpophalangeal joint instability, thenar muscle hypoplasia or aplasia, extrinsic tendon dysplasia, and in the most severe cases, carpometacarpal joint instability or thumb aplasia. Severe thumb hypoplasia and aplasia are best treated by thumb ablation and pollicization of the index finger. Less severe thumb hypoplasia can be reconstructed by a combination of soft tissue release, first web space local flap coverage, metacarpophalangeal joint collateral ligament and capsule reconstruction, extrinsic tendon ten

[Arthroscopic anterior capsular release in medial contracture of the shoulder secondary to brachial plexus birth palsy. Preliminary results].

PURPOSE OF THE STUDY: Retraction of the shoulder in internal rotation is observed in 25% of children with brachial plexus birth palsy (C5, C6 +/- C7). Early bone and joint deformities affecting the glenohumeral joint are the consequences. The stiff internal rotation requires surgical release which can involve the capsule and ligaments, muscles, or both. Internal release can be combined with muscle transfer to improve active external rotation. We report the results obtained with arthroscopic anterior capsular release combined with latissimus dorsi transfer. MATERIAL AND METHODS: From 1999 thro

Classification and treatment of postburn metacarpophalangeal joint extension contractures in children.

Two hundred and seventy-eight surgically treated postburn metacarpophalangeal joint extension contractures in children were reviewed. A classification system based on the limitation of passive metacarpophalangeal flexion was devised to direct surgical intervention and assess postoperative results. Type I (47%) digits demonstrated greater than 30 degrees of metacarpophalangeal flexion with the wrist fully extended, and scarring was generally limited to the dorsal skin. Type II (34%) digits demonstrated less than 30 degrees of metacarpophalangeal flexion with the wrist maximally extended, and sc

Influence of different anatomical structures on distal soft tissue procedure in hallux valgus surgery.

BACKGROUND: The distal soft tissue procedure is considered to be an effective surgical procedure to increase the correction in hallux valgus surgery. The goal of this study was to evaluate the efficacy of each single step of the lateral release in an experimental setup to assess which surgical steps are essential to correct hallux valgus deformity, which steps may be ineffective, and which steps may be risky. MATERIALS AND METHODS: Three techniques of lateral release that differ in the sequence of surgical steps were evaluated in 15 fresh-frozen cadaver feet. After each step, an identical med

Rare cause of block to reduction after radial head dislocation in children.

This case report documents a rare cause of block to reduction in association with an anterior traumatic dislocation of the radial head in children. The radial head dislocations described were not reducible with a closed reduction secondary to "button holing" of the radial head through the anterior joint capsule. Open reduction was required to release the capsule and reduce the radial head. After reduction, the elbow was immobilized in flexion and the forearm in mild supination for 3 weeks. Postoperatively, all patients demonstrated full elbow function and range of function.

The success rate of first metatarsophalangeal joint lateral soft tissue release through a medial transarticular approach: A cadaveric study.

BACKGROUND: The objective of this study was to evaluate the success rate of first metatarsophalangeal joint (MTPJ) lateral soft tissue release through a medial transarticular approach. METHODS: Ten cadaveric specimens were used (6 females/4 males, mean age, 73.4years). Lateral release was performed through a 4cm medial approach using a number 15 blade. Surgical aim was to release four specific structures: lateral capsule, lateral collateral ligament (LCL), adductor hallucis tendon (AHT) and lateral metatarsosesamoid suspensory ligament (LMSL). Once completed, a dissection of the first interme

Early soft-tissue release in congenital dislocation of the hip.

A technique is presented for gentle soft-tissue release in congenital dislocation of the hip that resists reduction even after traction. The technique involves the release of the adductor longus and iliopsoas tendon, and the opening of the anteromedial joint capsule, if necessary. It is hoped that the more frequent use of this technique will lessen the necessity for pelvic or femoral osteotomy or for a wider open reduction, thereby avoiding the problems associated with these procedures.

Are soft tissue releases sufficient to correct moderate to severe flexion contractures during total knee replacement? Role of hamstring tenotomy and horizontal capsular release.

BACKGROUND: Performing total knee arthroplasty (TKA) in arthritic knees with persistent flexion deformities is a challenging task because secondary tightening of surrounding soft tissues makes it difficult to achieve appropriate ligament balancing. In this study, we shared our algorithm for correction of flexion contractures of moderate to severe grade while performing TKA as well as their outcomes with 1 year of follow up. METHODS: Forty-three patients (61 knees) having knee arthritis with moderate to severe flexion contractures (Lombardi classification) were included in the study. We follow

Comparison of surgical outcomes between rotator cuff repair with and without rotator interval capsular release for rotator cuff tears to prevent and improve postoperative stiffness: a meta-analysis.

BACKGROUND: A common complication of rotator cuff (RC) repair is a postoperative stiffness. Postoperative stiffness may develop and lead to inferior functional outcomes. Rotator cuff repair with additional rotator interval (RI) capsular release can be done to prevent postoperative stiffness in rotator cuff tears. However, routine rotator interval capsular release in rotator cuff repair is controversial. Therefore, we conducted a systematic review and meta-analysis to compare the postoperative outcomes between RC repair with and without RI capsular release for RC tears with and without concomit

[Heyman's operation for the treatment of congenital metatarsus varus].

The authors report their experience with the operation of Heyman, i.e. complete release of the tarsometatarsal and intermetatarsal joint for the treatment of congenital metatarsus varus. This operation of Heyman is capable of meeting the requirements of correction if the indications are precise and if the surgical technique is perfect. The operation is indicated in cases of varus over 15 degrees, passively reducible, with good axes of the hindfoot and without bony alterations of the Lisfranc joint, in children between 3 and 8 years old. There should be a dorsal and medial capsulotomy, with sec

Chronic boutonniere deformity--an anatomic reconstruction.

Restoration of motion and appearance of the chronic boutonniere deformity is often accomplished by splinting. If the deformity persists following appropriate splinting, and if a full passive range of motion of the proximal interphalangeal joint is present, surgery may be recommended. Thirteen patients had surgical reconstruction over a 10-year period, with satisfactory improvement in all but one. This operation involves release of the transverse retinacular ligaments and reconstruction of the damaged central slip by using the local joint capsule and synovium attached to the base of the proxima

Extensor Pollicis Longus Tendon Translocation with a History of Thumb Polydactyly Surgery.

Nontraumatic and nonrheumatoid translocation of the extensor pollicis longus (EPL) tendon at the metacarpophalangeal (MP) joint is rare and may result from insufficiency of the dorsoradial capsule and sagittal band complex. We report a 41-year-old carpet weaver woman with a history of childhood thumb polydactyly reconstruction who presented with painful EPL translocation during thumb motion. She had previously undergone unsuccessful surgery for presumed EPL snapping at the wrist. Intraoperative findings revealed a normal extensor pollicis brevis tendon and 2 EPL tendons, both displaced ulnarly
