The diagnosis and treatment of instability of the subtalar joint.

We have developed a method of measuring anterior displacement of the calcaneus on the talus in instability of the subtalar joint and have used the technique to demonstrate anterior instability in 50 patients (72 feet) showing a positive drawer sign. The angle of the posterior facet of the talus was also measured to assess the bony configuration. Our patients with subtalar joint instability could be divided into three categories. The first group had a history of trauma leading to ankle instability (26 cases), the second showed generalised joint laxity (10 cases) and the third were young females

[Instability of the wrist joint--diagnosis and therapy].

Wrist-joint instability can be caused by injuries to ligaments or bones, it can be intermittent or permanent. Clinical symptoms and pain sensations are non-characteristic. The X-ray findings are the clue to the diagnosis. In this article recommendations are given for exact X-ray positions of the hand and their evaluation. The classification of instability is corresponding to the X-ray findings. Further investigations like radiographs under stress, dynamic investigations or the arthrography of the wrist-joint can be helpful to find out the exact pathologic changes. They are variable and are ran

Metatarsophalangeal joint instability of the lesser toes.

Metatarsophalangeal joint instability of the lesser toes is a common finding and a common cause of metatarsalgia. The clinical presentation can include swelling without digital deformity; however, often, this can progress to the development of coronal and transverse plane malalignment. In some cases, frank metatarsophalangeal joint dislocation can develop. The treatment regimen has historically focused on indirect surgical realignment using soft tissue release, soft tissue reefing, tendon transfers, and periarticular osteotomies. An improved understanding of the plantar plate has recently led

[Computerized tomography in instability of the femoropatellar joint].

Compared with normal X-ray examination, a CT scan can give more information about patellar alignment, especially close to full extension of the knee-joint. We compared the normal knee-joints of 10 volunteers with the knee-joints of 15 patients (19 recurrent luxations, 5 habitual subluxations). We were able to distinguish between instabilities with and without changes seen on the X-ray pictures, making it possible to develop a systematic concept of treatment. In the case of recurrent dislocation with radiologically visible instability, surgical repair is the treatment of choice, while for habit

[CT diagnosis in instability of the shoulder joint].

CT is able to demonstrate the predisposing factors for recurrent and posttraumatic subluxation of the shoulder. In addition to demonstrating bony changes in the glenoid (Bankart lesion) and in the humeral head (Hill-Sachs lesion), CT can measure the degree of retroversion of the glenoid and torsion of the humerus. Measurements on 17 shoulders with habitual and 24 shoulders with recurrent posttraumatic subluxation showed low values for glenoid retroversion as compared with a control group. Particularly patients with habitual anterior subluxation frequently showed anteversion of the glenoid. Mea

Instability of the patellofemoral joint in Down syndrome.

The knees of 210 institutionalized patients with Down syndrome were evaluated to determine the prevalence of patellofemoral instability and to assess the value of orthotic and surgical treatment. The cases of 151 non-institutionalized patients with Down syndrome were also reviewed. The patella was dislocatable or dislocated in thirty-five knees (8.3 per cent) in the institutionalized group and in twelve knees (4.0 per cent) in the non-institutionalized group. This interfered with walking in some patients, but only three of the patients were unable to walk because of patellofemoral instability.

Recurrent instability of the gleno-humeral joint.

The importance of instability in the determination of painful syndromes of the shoulder is examined, with particular reference to young people who engage in sport. The physiopathology and clinical features are described, and the importance of diagnostic tests is emphasized. These often disclose the underlying pathological anatomy. Surgical treatment, which is indicated whenever the lesion is well documented, follows the methods used in treating recurrent dislocation. The validity of the method of Latarjet and Bristow is emphasized.

Lesser Metatarsophalangeal Joint Instability: Nonsurgical Treatment Alternatives.

Lesser metatarsophalangeal (MTP) joint instability can cause pain in the MTP joints with ambulation. The goal of nonsurgical treatment is to offload the metatarsal heads and stabilize the MTP joints. Achilles stretching can be effective by decreasing pressure in the forefoot with ambulation. Metatarsal offloading does not stabilize the lesser MTP joints but can alleviate pain in the plantar surface of the metatarsal heads. Crossover taping and strapping can stabilize the MTP joint and resist MTP hyperdorsiflexion. Steroid injections can alleviate inflammation but is also associated with furthe

Distal Radioulnar Joint Instability: Diagnosis and Treatment.

Distal radioulnar joint (DRUJ) instability and triangular fibrocartilage complex (TFCC) tears are more usual than estimated and are frequently overlooked. Diagnosis is often clinical, which can be confirmed using computed tomography (CT) scan and magnetic resonance imaging (MRI). In doubtful cases, bilateral computed tomography in neutral forearm rotation, supination, and pronation should also be performed. Wrist arthroscopy can be diagnostic and therapeutic for ulnar-sided wrist pain. Two systematic reviews showed equivalent outcomes between open and arthroscopic repair of the TFCC. There is

Magnetic resonance imaging of glenohumeral joint instability.

Shoulder instability is common, especially anterior subluxation and dislocation. The sequelae are well seen on magnetic resonance imaging and include tears of the labrum, glenohumeral ligaments, capsule, tendons, and muscles. This article seeks to discuss and illustrate common pitfalls and lesions associated with instability. Anatomic and technical considerations, including the use of magnetic resonance arthrography, are also addressed.

Persistent pelvic pain and pelvic joint instability.

As accepted as the condition of pelvic pain and pelvic joint instability (PPPJI) is in pregnancy, as controversial and doubted is the same diagnosis in non-pregnant women. Women suffering severe incapacitating PPPJI for years following parturition have recently founded an association. This offers a unique opportunity to investigate their characteristics. 153 of the 215 members answered a questionnaire. The majority (110) suffered PPPJI in first pregnancy with onset preceding 20 weeks gestation in 84 women. Onset was significantly earlier in users of oral contraceptives than in non-users (16.3

[Ischemia in acute traumatic knee joint instability].

Eleven of 202 patients operated for acute knee instability from 1976 to 1988 showed an associated popliteal vascular injury. No patient with intact posterior cruciate ligament had a vascular lesion, and all patients with popliteal vascular injury had a ruptured posterior cruciate ligament. Our patients are analyzed with respect to clinical symptoms of the ischemia, management and results at assessment. Prompt recognition and early revascularisation with debridement of all grossly damaged arterial wall, a tension-free reconstruction and the liberal use of fasciotomy is the key to improved limb

Lesser Metatarsophalangeal Joint Instability: Arthroscopic Treatment Alternatives.

Instability of the lesser toes metatarsophalangeal (MTP) joints is a common forefoot problem and can present as metatarsalgia, synovitis, and lesser toe deformities. The plantar plate contributes to MTP joint stability through its interaction with the plantar aponeurosis, and plantar plate insufficiency is the primary cause of MTP joint instability. Conservative treatment options, including the use of nonsteroidal anti-inflammatory medications, intra-articular steroid injections, orthosis, and accommodative shoe modifications, generally provide only temporary relief and do not effectively halt

First Carpometacarpal Joint Instability: Dorsal Ligament Reconstruction.

The first carpometacarpal (CMC-I) joint has an elaborate ligamentous support. Recent evidence has demonstrated that the dorsal ligament group is imperative for joint stability and that CMC-I joint instability may occur as a consequence of trauma and ligamentous laxity, and other conditions, with possible CMC-I subluxation and the development of osteoarthritis. Although various surgical techniques have been introduced for the treatment of ligamentous CMC-I instability, the Eaton-Littler reconstruction has been regarded as the gold standard. It is widely accepted that impaired hand function may

Sternoclavicular Joint Instability: Symptoms, Diagnosis And Management.

Sternoclavicular joint (SCJ) instability is a rare condition and results from either a traumatic high energy impact, such as a motor vehicle crash or contact sports injury, or non-traumatically as a result of structural pathology. The infrequency of this injury has contributed to its diagnosis being missed as well as the paucity of literature on treatment and outcomes. Patients with SCJ instability often report diminished range of motion as well as shoulder girdle pain. The presentation of instability in the sternoclavicular joint can vary in severity and anterior or posterior directionality.

Functional bracing for distal radioulnar joint instability.

Symptomatic chronic distal radioulnar joint (DRUJ) instability can be a challenging disorder to treat surgically. This study used a cadaver model to assess the effect of functional forearm bracing on DRUJ instability. The specimen upper extremities were mounted on a platform and the DRUJ was destabilized sequentially. The effect of both prefabricated commercial braces and custom-made braces on joint stability was documented by computed tomography. Both braces markedly reduced DRUJ translation in both full pronation and full supination. The custom-made brace overreduced the DRUJ in full pronati

Effect of simulated joint instability and bracing on ankle and subtalar joint flexibility.

It is clinically challenging to distinguish between ankle and subtalar joints instability in vivo. Understanding the changes in load-displacement at the ankle and subtalar joints after ligament injuries may detect specific changes in joint characteristics that cannot be detected by investigating changes in range of motion alone. The effect of restricting joints end range of motion with ankle braces was already established, but little is known about the effect of an ankle brace on the flexibility of the injured ankle and subtalar joints. Therefore, the purposes of this study were to (1) underst
