Endoscope-Assisted, Transmastoid, High Cervical Approach for Resection of a Jugular Foramen Schwannoma: 2-Dimensional Operative Video.

Jugular foramen schwannomas (JFSs) are relatively rare, benign lesions that account for 10% to 30% of all tumors in the region of the jugular foramen. Given their slow-growing nature, JFSs can become quite large before causing symptoms of lower cranial nerve (LCN) dysfunction, making microsurgical resection a challenge. Successful resection of any JFS is dependent on the identification and preservation of the adjacent, uninvolved LCNs to alleviate nerve compression and preserve function. We report a transmastoid, high cervical approach to a dumbbell-shaped, extracranial JFS that was causing sy

The Comparison of the Right and Left Sigmoid Sinus Cross-Sectional Areas in Fetal Period and the Factors Affecting the Venous Dominance.

OBJECTIVES: Skull base is an important and a challenging area for surgeons. Success in skull base surgery depends on various factors such as pre-operative evaluation, appropriate surgical technique, anesthesia duration, intraoperative neuromonitorization and wound care. MATERIALS AND METHODS: This study was performed in the Anatomy dissection laboratory of M.U. Medical Faculty (Ethical committee approval number 2010-103). Twelve fetuses between 17-33 gestational weeks fixed with formaldehyde were enrolled to the study. RESULTS: This study was planned to investigate the cross sectional areas

[Jugular Foramen, Foramen Magnum Meningiomas].

The jugular foramen, also known as the foramen magnum, is a highly intricate region of the skull base through which numerous critical blood vessels and nerves traverse. Meningiomas, the most common tumors in neurosurgical pathology, can arise at any location where the meninges are present, posing significant challenges. Meningiomas involving the jugular foramen and sublingual neural tube are particularly notable for their potential to extend from intracranial to extracranial sites, necessitating familiarity with extracranial anatomy, which is not typically encountered in clinical practice. Can occipital lobe bending, Gibraltar sign of superior sagittal sinus groove and jugular foramen dimensions predict transverse sinus dominance?

BACKGROUND: Asymmetry between the transverse sinuses (TS) is quite common. We sought to test the possible hypothesis that certain anatomical features - namely, occipital lobe bending, Gibraltar sign of superior sagittal sinus groove (SSS) and jugular foramen (JF) dimensions - can predict dominance of the transverse sinuses on routine axial T1- and T2-weighted images. MATERIALS AND METHODS: One hundred consecutively acquired combined MRI-MRV studies of brain were reviewed. On non-contrast axial T1WI, each reviewer assessed the occipital lobe bending, and Gibraltar sign of SSS groove; on axial

Jugular foramen and venous collaterals may help to discriminate congenital from post-thrombotic jugular stenosis.

PURPOSE: Unilateral jugular stenosis is easily mistaken as jugular hypoplasia for their similar jugular appearances. This study aimed to propose a scheme to differentiate acquired internal jugular vein stenosis (IJVS) from congenital jugular variation through two case examples. METHODS: We presented a dynamic evolution process of the IJVS formation, through a case of a 17-year-old female with paroxysmal nocturnal hemoglobinuria (PNH)-associated right internal jugular venous thrombosis (IJVT), which resulted in post-thrombotic IJVS in the rare context of rapid recanalization. Meanwhile, we com

A skull base extradural hypoglossal neurilemmoma resected via an extended posterolateral approach.

The case of a 43-year-old man with a right skull base hypoglossal neurilemmoma excised via the extended posterolateral approach is presented. There is only one previous case of hypoglossal neurilemmoma in the literature. The surgical technique described is a new approach to the posterior skull base involving a suboccipital craniectomy, mastoidectomy and the removal of the lateral process of the atlas. It provides an inferior approach to the jugular foramen and hypoglossal canal that allows the lower cranial nerves to be identified as they exit from their skull base foramina. In the discussion

Transoral Approach to the Jugular Foramen Region with Preservation of the Eustachian Tube.

OBJECTIVES/HYPOTHESIS: Transnasal exposure of the jugular foramen region (JFR) often requires transection of the Eustachian tube (ET). This study aims to propose a transoral corridor for access to the JFR with preservation of the ET. STUDY DESIGN: Cadaveric dissection and case illustration. METHODS: An endoscopic transoral approach for exposure of the JFR was performed on 5 cadaveric specimens (10 sides). Six patients who underwent a transoral resection of schwannoma within the JFR were retrospectively analyzed. RESULTS: Direct exposure of the JFR with a 0° scope via a transoral approach wa

Mucoepidermoid carcinoma of the parotid gland with invasion of the jugular foramen region: A case report.

RATIONALE: Mucoepidermoid carcinoma of the parotid gland, being the most prevalent malignant neoplasm among salivary gland tumors, represents the leading incidence rate among primary parotid malignancies. This pathological entity predominantly affects young and middle-aged females, typically presenting as a painless parotid mass. Due to its variable disease progression and nonspecific clinical manifestations, preoperative misdiagnosis as a benign tumor is frequently encountered. However, when parotid mucoepidermoid carcinoma progresses to involve the jugular foramen region, a complex anatomica

Gradenigo's and Vernet's syndrome in an adult man with Candida mastoiditis.

The authors report a case of fungal otitis media complicated by extension of the infection into adjacent structures causing apical petrositis and subsequently involvement of the jugular foramen in a 71-year-old diabetic man. First described in 1907, Gradenigo's syndrome is a serious but rare clinical triad of acute otitis media, unilateral pain in the distribution of cranial nerve V (trigeminal) and ipsilateral cranial nerve VI (abducens) palsy that commonly presents without all three features and is therefore often missed. In this report, our patient was initially misdiagnosed as having a dia

Manipulation of the Lower Cranial Nerves and Superior Jugular Bulb During Subtotal Temporal Bone Resection for Advanced Temporal Bone Malignancies: Anatomical Considerations and Surgical Strategies.

INTRODUCTION: En bloc subtotal temporal bone resection (STBR) is challenging. En bloc resection aiming for negative margins is considered the most reliable treatment strategy for advanced squamous cell carcinoma of the external auditory canal. However, the literature indicates that STBR carries a high perioperative risk. This report retrospectively examines 5 cases of en bloc STBR, considering the manipulating of the lower cranial nerves and the superior jugular bulb (SJB) from a microsurgical anatomical perspective. METHODS: Retrospective observational study. This study includes 5 cases of S

High Riding Jugular Bulb Protruding Into Tympanic Cavity: Longitudinal Radiologic Study in a Deaf Child.

BACKGROUND: High riding jugular bulb (HRJB) develops after 2 years and is rare at younger ages. High riding jugular bulb sometimes protrudes into the tympanic cavity, which can cause hemorrhagic complications during otologic surgery. CASE PRESENTATION: We describe a congenitally deaf child with bilateral inner ear malformations and a right-sided HRJB on CT at 9 months. This child had undergone left cochlear implantation (CI) at 19 months, and right CI was planned at 6 years. However, we decided not to perform the right CI because preoperative CT images revealed that the right jugular bulb (JB

Compartmental Subdivisions of the Jugular Foramen: A Review of the Current Models.

BACKGROUND: At present, no consensus has been reached on the compartmental subdivision of the jugular foramen (JF), which can complicate surgical planning in this area and hinder understanding of foraminal tumor growth patterns. The extradural neural axis compartment (EDNAC) might aid in producing a standardized model in the future. In the present review, we have summarized the models of JF compartmentalization and analyzed how sound they are anatomically. METHODS: The present narrative review identified the key studies and supporting reports that had discussed, referenced, or first presented

Intraoperative recording of laryngeal adductor reflex and cortical motor evoked potentials during jugular foramen schwannoma surgery: a case report and literature review.

PURPOSE: Schwannoma, a tumor originating from the peripheral nervous system, may arise from the vagus nerve, although it is not very often. Injury of the vagus nerve by surgical attempts may have consequences that will seriously affect the patient's quality of life. In recent years, continuous monitoring of the laryngeal adductor reflex (LAR) has become a promising methodology for evaluating vagus nerve function intraoperatively. We refer to our experience changing our surgical strategy due to concurrent deterioration in LAR and CoMEPs intraoperatively. We also provide a literature review and

Lower Cranial Nerve Schwannomas.

Lower cranial nerve (LCN) schwannomas are rare, benign peripheral nerve sheath tumours arising from cranial nerves IX-XII. Though uncommon, they pose significant clinical challenges due to their complex anatomical location and potential for neurological deficits. Jugular foramen schwannomas (JFS) and hypoglossal schwannomas (HS) are the most prevalent subtypes. Clinical presentation varies by nerve involvement, ranging from hoarseness and dysphagia to tongue atrophy and paroxysmal cough. Diagnosis relies heavily on MRI and CT imaging, with differentiation from paragangliomas being critical. Cl
