Scaling up Kangaroo Mother Care in Ethiopia and India: a multi-site implementation research study.

OBJECTIVES: Kangaroo Mother Care (KMC), prolonged skin-to-skin care of the low birth weight baby with the mother plus exclusive breastfeeding reduces neonatal mortality. Global KMC coverage is low. This study was conducted to develop and evaluate context-adapted implementation models to achieve improved coverage. DESIGN: This study used mixed-methods applying implementation science to develop an adaptable strategy to improve implementation. Formative research informed the initial model which was refined in three iterative cycles. The models included three components: (1) maximising access to

[PAPSE study: the role of baby slings in skin-to-skin contact practices in the neonatal unit and during the hospital stay].

The benefits of skin-to-skin contact (STSC) are well known, while those of babywearing have been less studied. However, when combined, they improve the baby's physiological regulation and contribute to the bonding process, which is essential for the infant's secure attachment and future exploratory abilities. While STSC is widely offered in neonatal intensive care units, it appears to be less common in general neonatal care. The use of a baby sling could encourage its implementation and increase its use by parents.

Impact of mother-baby friendly initiative plus approach on improving human milk feeding for neonates in hospital: a quality improvement before-and-after uncontrolled study.

Breastfeeding, use of pasteurised donor human milk when mother's own milk is unavailable and kangaroo mother care have independently proven benefits in improving survival of vulnerable sick babies. A triangulated approach called the Mother Baby Friendly Initiative Plus (MBFI+) model, bringing together the combined benefits of these proven interventions, was used to improve exclusive human milk feeding at health facilities through quality improvement and system strengthening approach. This quality improvement before-and-after uncontrolled study enrolled 5343 term and 278 very low birth weight (

Barriers to Kangaroo Care in the NICU: A Qualitative Study Analyzing Parent Survey Responses.

BACKGROUND: Despite its benefits, parents in the neonatal intensive care unit (NICU) face significant barriers to kangaroo care (KC). Clinician-reported barriers to KC include staff education, environment, and equipment among others; however, parent-perceived barriers are underexplored. PURPOSE: To examine parental understanding of KC, parental perception of experiences with KC, and parental views on the key factors that help or hinder KC. METHODS: This is an observational, mixed-methods study that used an author-developed survey to assess parental feelings, perceived importance, and barrier

Kangaroo Cuddle Transport for Neonatal Palliative Care: A Service Evaluation.

BACKGROUND: Newborn infants represent a significant percentage of childhood deaths (>60%) with over 90% of newborn end-of-life care taking place in an acute hospital setting. This is in direct contrast to adults and children where specialised hospital or hospice care is used during their palliative care experience. To support family choice, a neonatal retrieval service developed a new model of care to transfer babies from a hospital to a hospice, a hospital closer to home or a private residence for palliative care while being cuddled by a parent. AIM: Identify and evaluate (i) service workflo

Scaling up kangaroo mother care in the Philippines using policy, regulatory and systems reform to drive changes in birth practices.

The WHO recommends kangaroo mother care (KMC) for stable preterm and low birthweight babies because it has been demonstrated to reduce mortality by up to half compared with conventional incubator-based care. Uptake of KMC in low/middle-income countries has been limited, despite its suitability for low-resource environments. This paper reviews factors that contributed to the adoption and expansion of KMC in the Philippines. Early introduction began in 1999 but national scale-up was slow until 2014 after which a significant improvement in national adoption was observed. The proportion of target

Effect of post-discharge online kangaroo care training on breastfeeding self-efficacy in mothers with preterm infants: A randomised controlled study.

BACKGROUND: Preterm newborns face many health problems due to their incomplete intrauterine development and the immaturity of their systems. One of these problems concerns nutrition. This study aimed to determine the effect of online kangaroo care (KC) training provided post-discharge on breastfeeding self-efficacy in mothers with preterm infants. METHODS: This research was conducted as an experimental study with a randomised control group. Two groups were formed: kangaroo care and control. The sample of the study consisted of 68 mothers of preterm infants. The mothers in the KC group were gi

Effect of the Kangaroo Mother Method after Preterm Delivery on Maternal Stress and Anxiety in the Context of the COVID-19 Pandemic-A Cohort Study.

BACKGROUND: The kangaroo mother method (KMM) may benefit infants and mothers in many ways. However, few studies focused on its efficacy on maternal anxiety and stress, especially in the context of the COVID-19 pandemic. OBJECTIVE: To examine the effect of the kangaroo mother method (KMM) on postnatal stress and anxiety in mothers of preterm infants in neonatal intensive care, in the context of the COVID-19 pandemic. METHODS: A cohort study of two groups of 56 mother-infant dyads recruited from a neonatal intensive care unit was conducted. Two groups were compared in terms of the mean duratio

Reconfiguring insufficient breast milk as a sociosomatic problem: mothers of premature babies using the kangaroo method in Brazil.

This study focuses on Brazilian mothers who gave birth to premature babies who were discharged from hospital using the Kangaroo Mother Care Method. While mothers left the hospital breastfeeding exclusively, once back at home, they abandoned exclusive breastfeeding because of insufficient breast milk (IBM). In this project we explored how IBM was interpreted by mothers within their social context. Participatory research using the Creative Sensitive Method was done in the homes of mothers with family members and neighbours. We described the conflicting social discourse that influenced the mother

Efficacy of cloth-to-cloth contact versus conventional care in thermoregulation of term newborns: an open-label randomized controlled trial.

Neonatal hypothermia is a major contributor to morbidity and mortality in low- and middle-income countries (LMICs). While effective in reducing hypothermia, implementation of skin-to-skin contact (SSC) is often hindered by its privacy concerns. This study aimed to evaluate the efficacy and feasibility of cloth-to-cloth contact (CCC) as an alternative to SSC in term newborns at postnatal ward, where the newborn is dressed and placed in the kangaroo position on the mother's chest without exposing her bare skin. In this randomized trial, stable term newborns in a postnatal ward were assigned to e

Womens' experience on barriers and facilitators for implementation with Kangaroo Mother Care at Mbarara Regional Referral Hospital, Uganda: a qualitative study.

BACKGROUND: Kangaroo Mother Care (KMC) is a proven cost-effective intervention for preterm and low‑birth‑weight infants, to provide warmth and secondary health effects. In Uganda, despite a high burden of prematurity, resource constraints limit the delivery of cost-effective interventions. OBJECTIVE(S): This study aimed to identify barriers and enablers for implementing KMC, explore mothers' experience of KMC and apply the i-PARIHS framework to understand why implementation was, or was not, successful at Mbarara Regional Referral Hospital. METHODS: This study was conducted in a rural referra

Kangaroo father care: A pilot feasibility study of physiologic, biologic, and psychosocial measures to capture the effects of father-infant and mother-infant skin-to-skin contact in the Neonatal Intensive Care Unit.

Robust literature supports the positive effects of kangaroo mother care (KMC) on infant physiologic stability and parent-infant bonding in the Neonatal Intensive Care Unit (NICU). Comparatively little is known about kangaroo father care (KFC) in the NICU, and KFC implementation has been limited. Our pilot feasibility study objective was to examine KFC effects on premature infants and fathers as compared to KMC. Parents of preterm NICU infants independently completed a 90-min Kangaroo Care (KC) session on consecutive days. Infant heart rate variability (HRV) and apnea/periodicity measures were

Implementing a Skin-to-Skin Care and Parent Touch Initiative in a Tertiary Cardiac and Surgical Neonatal Intensive Care Unit.

BACKGROUND: Skin-to-skin care (SSC) has been integrated as an essential component of developmental care for preterm infants. Despite documented benefits, SSC is not routinely practiced in the cardiac and surgical neonatal intensive care unit, with a predominantly term population, due to staff apprehension, patient factors and acuity, and environmental constraints. PURPOSE: The purpose of this quality improvement project was to increase SSC, parental holds, and parent touch events for infants in our cardiac and surgical neonatal intensive care unit. When traditional SSC was not possible, alter

SKIN-TO-SKIN CONTACT IN NEONATAL CARE: CLINICAL SIGNIFICANCE, IMPLEMENTATION CHALLENGES, AND LONG-TERM OUTCOMES IN GEORGIA.

Skin-to-skin contact (SSC) is a crucial component of neonatal care, playing a vital role in early physiological stabilization, neurodevelopment, and the formation of maternal-infant bonding. A growing body of evidence indicates that deprivation of SSC, particularly during the immediate postnatal period, is associated with long-term adverse outcomes, including emotional dysregulation, behavioral difficulties, and impaired cognitive development. Importantly, the absence of early SSC has also been shown to negatively affect the initiation and continuation of breastfeeding, thereby limiting its we
