Rapid detection of Rh(D)- or K-positive fetal red cells in chorion villus samples by a flow cytometric technique.

Rh(D)- and K-negative women who have become severely isoimmunized by pregnancy are at risk of fetal loss or damage in subsequent pregnancies. A flow cytometric method is described whereby the presence of Rh(D) or K antigen on fetal erythrocytes may be determined using chorion villus samples taken during the first trimester. This method has the advantage of speed and sensitivity with results being available within 2 h. Decisions as to management of the pregnancy or termination may thus be made with minimal delay.

[Problematic blood transfusion (author's transl)].

Two patients were admitted with signs of haemolysis due to severe transfusion reactions. Serological investigations showed the presence of the antibodies anti-IKb and anti-Fya. After a short period of haemodialysis the patient with Kidd antibodies died. A further patient with triple antibodies (anti-c, -K, -Lub) was so pre-immunised that with an antigen frequency of over 99.9% no compatible blood could be found in Europe. By means of information from the European Central Index in Amsterdam compatible blood for the operation could be obtained from Australia. In the fourth case the antibody anti

Pregnancy outcome in mothers who develop Kell antibodies.

This is a review of the obstetric histories of women with Kell antibodies who delivered between 1964 and 1983. The majority of women with Anti-Kell in their sera have Kell negative husbands and have been sensitised by blood transfusion. In only six mothers was antibody formation the result of isoimmunisation and all cases where the baby was known to be Kell positive ended in fetal death. Amniocentesis is not helpful in managing these cases, and at times might be misleading, so that other techniques, such as ultrasound, might be more helpful. A review of the literature confirms that Kell isoimm

Red cell alloimmunization in sickle cell disease.

Alloimmunization to red cell antigens contributes to morbidity in transfused patients. It has been recommended that blood for sickle cell patients need not be matched for antigens other than ABO and Rh(D), as there is no greater incidence of antibody production than in other multitransfused patient populations. Post transfusion alloimmunization was studied in a group of 34 sickle cell disease patients attending a U.K. haemoglobinopathy clinic. Red cell antibodies were formed in 17.6% of the transfused patients and Rhesus and Kell antibodies accounted for 66% of this total. In order to reduce a

Chromosomal mapping of the blood group genes.

The assignment of blood group loci proceeds more slowly than that of biochemical markers, since it depends largely on pedigree analysis and these studies take longer to achieve significance than some of the rapid modern methods. However, seven structural blood group loci ABO, MNSs, Rh, Fy, Sc, Xg, and Ch--Rg are confidently assigned, and four more provisionally or tentatively sited. Other loci are recognized as part of linkage groups that will help in the eventual assignment to their chromosomes. There is little linkage information, and no assignments, for other independent loci involved in th

Anti-erythrocyte antibodies, leukocytotoxins and human renal allograft survival.

ABO blood group compatibility is generally required for successful human kidney transplants and new data suggest donor-recipient incompatibility for the Lewis or multiple minor blood groups is detrimental. This is of special importance in the setting of liberal transfusions prior to transplant. To assess the impact of immunizing exposure to disparate blood groups of ABO-matched transfusions and kidney transplants, we studied the interrelationships of anti-erythrocyte antibodies, leukocytotoxins and graft survival in 42 kidney transplant recipients. Three hundred forty-four sera were screened f

The mouse Kell blood group gene (Kel): cDNA sequence, genomic organization, expression, and enzymatic function.

The human Kell blood group system is important in transfusion medicine, since Kell is a polymorphic protein and some of its antigens can cause severe reactions if mismatched blood is transfused, while maternal alloimmunization may lead to fetal and neonatal anemia. In humans, Kell is an Mr 93,000 type II membrane glycoprotein with endothelin-3-converting enzyme activity that is linked by a single disulfide bond to another protein, XK, that spans the membrane ten times. An absence of XK leads to clinical symptoms termed the McLeod syndrome. We determined the cDNA sequence of the mouse Kell homo

Naturally occurring anti-Kpa in an infant with recurrent bacterial infection: A case report and review of the literature.

Naturally occurring anti-Kpa antibody is extremely rare and was first reported in 1957, named after the first producer 'Penney'. However, the subsequent anti-Kpa reports presented were all anti-Kpa due to isoimmunization. Individuals with severe bacterial infections particularly Gram-negative bacteria are known to be capable of producing cross-reactive antibodies against Kell blood group system. However, such uncommon antibodies like anti-Kpa can be easily missed in routine pre-transfusion testing unless the panel cells containing low incidence antigen are used for antibody screening. Here, we

Post-transfusion alloimmunization in patients with sickle cell disease.

The transfusion histories over a 33-month period of 50 patients with sickle cell disease were reviewed to determine the frequency of alloimmunization to red cell antigens following transfusion in these patients. There were 30 females and 20 males, aged 19--49 years. Eighteen (36%) were immunized of which thirteen were females. Five of the patients have formed only one antibody so far, while the other 13 have formed two or more. Thirty-six antibodies were identified: 16 against various Rh antigens, 12 anti-Lewis, 5 anti-Kell and one each of anti-Jka, -Fya and -M. The immunized patients received

Novel KEL allele associated with loss of Kpb identified in a white blood donor.

The importance of identifying variant alleles among blood donors is significant to the safety of transfusion for recipients. Molecular methods have become more prominent in the routine process of antigen typing donor units. Some variant antigens cannot be detected using only serologic methods. Molecular testing allows the determination of nucleotide sequences that are used to predict a phenotype. Antigens of the Kell blood group system are known for being highly immunogenic and causing adverse reactions upon antibody formation. A female white blood donor who typed Kp(b-) using serologic method

[Hydrops fetalis caused by isoimmunization against Kell factor (author's transl)].

The article reports on a case of hydrops fetalis caused by Kell antibodies, antibody formation having been triggered by pregnancy. Since the relative frequency of Kell (and rhesus sub-group) intolerance is increasing due to the successes achieved by anti-D-immunoglobulin prophylaxis, the question arises, on account of the possibly severe course of the disease, whether the Kell factor should be taken into consideration in all blood transfusions in girls during their childhood and in women of childbearing age.

The Utility of a Critical Antibody Titer in Anti-K Alloimmunized Pregnancies: A Systematic Review and Meta-Analysis of Diagnostic Test Accuracy.

Anti-Kell (anti-K) alloimmunization is a known cause of severe hemolytic disease of the fetus and newborn (HDFN), yet the utility of a critical maternal antibody titer in guiding clinical management remains debated. We conducted a systematic review and meta-analysis to evaluate the diagnostic accuracy of a maternal anti-K titer threshold of ≥8 for predicting the need for intrauterine intervention due to severe anti-K-mediated HDFN. In parallel, we characterized all reported cases of severe HDFN occurring in the setting of low maternal anti-K titers (<8). Studies were excluded if they lacked re

The Kmod blood group phenotype in a healthy individual.

This report describes a healthy blood donor whose red cells have weakened expression of Kell blood group antigens. Kell antigen activity could not be detected by flow cytometric analysis and was demonstrable only by sensitive serologic techniques. As with normal-strength Kell antigens, reactivity could be abolished by treatment with 2-aminoethylisothiouronium bromide (AET). The donor's red cells have Kx antigen activity. Other commonly tested blood group antigens (MNSs, Rh, P1, Lewis, Duffy, and Kidd systems) appear normal. Clinical and serologic examination showed that this case is different

Management of a severe transfusional problem in a patient with alloantibody to Kpb (K4).

The case of a 58-year-old male with severe anemia after hemicolectomy is described. The patient proved to be Kp(b-), and the serum contained anti-Kpb. Because no Kp(b-) donors were available, two incompatible units are administered after intravenous gammaglobulin (400 mg/kg/day) and hydrocortisone (500 mg). The tolerance was good, without signs of increased red cell destruction. Pending the arrival of compatible blood from the American Red Cross, the hematocrit reached 18%, and the direct antiglobulin test remained negative.

The effect of extended c, E and K matching in females under 45 years of age on the incidence of transfusion-induced red blood cell alloimmunisation.

Maternal alloantibodies directed against fetal red blood cell (RBC) antigens may cause potentially life-threatening haemolytic disease of the fetus and newborn (HDFN). Dutch transfusion guidelines therefore prescribe preventive cEK matching for all (pre-)fertile females. To quantify the impact of cEK matching, we compared overall and antigen-specific cumulative RBC alloimmunisation incidences in females and males aged <45 years. Among a multicentre cohort comprised of patients who received their first and subsequent RBC unit between 2005 and 2019, first-formed RBC alloantibodies were detected

Erythroblastosis fetalis produced by anti-k.

We report an instance of transfusion-induced anti-k so severe that three intrauterine intravascular fetal transfusions were required. The pretransfusion circulating hemoglobin level in the fetus was 60 g/l and hematocrit was 0.19. This, to our knowledge, is the first example of the rare alloantibody, anti-k, producing erythroblastosis so severe that fetal transfusions were required in order to prevent hydrops fetalis from developing. Anti-k alloimmunization, which in a period of 20 years and 8 months occurred only once in 3,246 alloimmunized pregnancies in Manitoba, can cause severe fetal dise
