Clinical Implications of Auricular Keloid Subsite for Multisite Keloid Involvement: An Exploratory Retrospective Cohort Study.

BACKGROUND: Auricular keloids are common, but whether anatomic subsite identifies patients with broader keloid susceptibility remains uncertain. Because ear piercing can involve repeated local trauma and chronic inflammation, the anatomic subsite and inciting cause may be closely related. The authors explored whether the auricular subsite was associated with multisite keloid involvement. METHODS: The authors reviewed 182 consecutive patients who underwent excision and postoperative radiotherapy for auricular keloids at a single tertiary center. Multisite involvement was defined as keloids at

Cryotherapy in the treatment of keloids.

BACKGROUND: Despite their benign nature, keloids may constitute a severe aesthetic and, in some cases, functional problem with important repercussions on patients' quality of life. There is no consensus on keloid treatment and no wholly satisfactory therapy has yet emerged. OBJECTIVE: To assess the efficacy of cryotherapy in the treatment of keloids. METHODS: 135 patients with 166 keloids received cryosurgical treatment between 1990 and 2004. Freeze times and number of sessions varied. Scar volume was measured before and after treatment. Median follow-up was 48.6 months (range 12.4-72.6 mont

[Modern treatment of keloids].

Keloids are a benign proliferative growth of dermal fibrous tissue, usually resulting from an excessive tissue response to skin trauma or disease in predisposed individuals. The basic cause is unknown. Hypertrophic scars are similar lesions but they will, in time, show some degree of resolution. These lesions can be aesthetically disfiguring and cause functional disturbances. This paper reviews current knowledge of the treatment modalities for keloids and hypertrophic scars. Most physicians emphasize the importance of prevention. No single therapeutic modality is the best. Current various trea

An approach to management of keloids.

Keloids present a major therapeutic dilemma for the surgeon because of frequent recurrences. We use a new protocol for management of large primary or recalcitrant keloids. The technique employs carbon dioxide laser resection of the keloid and then allows the open wound bed to heal by secondary intention. The open wound is treated as though it were a third-degree skin burn. The wounds invariably orient their long axis parallel to the relaxed skin tension lines. No keloid has ever been noted to recur before epithelial migration is complete. Careful follow-up detects early recurrences that are th

Pseudofolliculitis barbae with keloids.

Pseudofolliculitis barbae is a common cutaneous infection occurring in the bearded area of the face of black men. The infection is caused by ingrown hairs that produce an inflammatory foreign body reaction characterized by papules and pustules at the point of hair penetration that may result in permanent scarring, usually in grooved patterns, and occasionally in keloids. The purpose of this article is to describe the successful treatment of a patient with pseudofolliculitis with keloids that was based on the pathophysiology of the disease and the biology of wound repair.

Hypertrophic scars and keloids.

By modifying the wound healing process, it is possible to deal effectively with most abnormal forms of scarring, through perhaps 15 per cent of these lesions cannot be managed to the satisfaction of surgeon and patient. A laboratory test to determine which patients will overrespond to the stimulus of wounding and a better understanding of why these patients have an inordinately high anabolic rate of collagen metabolism will help in the salvage of those patients who are yet unmanageable. The great majority of patients can be helped and with them the lesions are best managed prophylactically if

Keloids and Hypertrophic Scars.

Keloids are an exuberant response to skin wound healing in which abundant scar tissue grows beyond the boundaries of the inciting insult. Age, race, location, family history and personal history of keloids are relevant factors concerning the risk of developing keloids. Because keloids are prone to recurrence after surgical excision, post-operative management plays an important role in the treatment of keloids. There are many modalities that can be used to treat keloids or prevent their recurrence; a multimodal approach is often necessary in difficult cases.

Nodular (keloidal) scleroderma.

Keloidal or nodular scleroderma (NS) is a variant of localized scleroderma (LS) frequently seen in patients with limited or diffuse systemic sclerosis (SSc). It presents as raised, firm plaques or nodules with extensive dermal fibrosis and hyalinized collagen bundles. We present a patient with SSc who presented with this rare entity.

[Prevention and treatment of keloids].

Keloids are pathological scars extending beyond the initial wound's natural borders. The condition occurs as a result of impaired wound healing with excessive collagen deposition. Keloid scars frequently recur, rarely regress, and have a tendency to grow over time. They are aesthetically disfiguring and can be devastating for patients, both physically and emotionally. This review finds that prevention and early intervention are essential for good results. Treatment is often challenging. There are no standardized management guidelines currently available but a combinational therapeutic approach

Pharmacological treatment for keloids.

INTRODUCTION: Keloids are fibroproliferative disorders that are characterized by histological accumulation of collagens and fibroblasts, refractory clinical symptoms such as itching, topical invasiveness, and frequent postsurgical recurrence. At present, to treat or prevent keloids, new drugs are currently being designed and the pharmaceutical indications of known drugs are being expanded. AREAS COVERED: The current pharmacological interventions for keloids are mainly described on the basis of the various hypotheses on keloid etiology and the keloid ingredients that are targeted. These interv

Immunologic associations of keloids.

The mechanisms underlying the pathogenesis of keloids have not been fully characterized despite extensive past and present research. Results of past and present studies have shown that the immune system is actively involved in the development of these lesions. Future investigations into the biochemistry and immunologic factors of keloids are anticipated and expected to produce additional insight. The inability to identify cellular (fibroblast) abnormalities has led most investigators to focus on the humoral regulators of wound healing, that is, biochemical substances, immunologic mediators and

Keloids of the external ear.

Keloid formation is a rare complication of normal scar tissue development, also to the surgeon who performs surgery on the external ear. We are presenting 4 cases of keloid formation which occurred after ear piercing in 2 cases and after otoplasty in another 2 cases. The treatment of keloid formation caused difficulties and is not unequivocal. Therefore we wish to present our experiences as we have seen no cases of keloid formation after otoplasty described in the literature.

Keloids: a review of the literature.

A review of the literature on keloids, a complex and poorly understood subject, is presented. Keloid is a disease principally of the human dermis, and occasionally the cornea, appearing as thick scar tissue invading normal skin or cornea and produced by the deposition of excessive amounts of collagen over prolonged periods. The African negro is particularly susceptible. Available methods of treatment include surgical excision, external superficial radiotherapy, interstitial irradiation, intralesional steroid injections, cryotherapy, ultrasound, systemic chemotherapy, zinc tape strapping, press

Nodular (keloidal) scleroderma.

Two patients with progressive systemic sclerosis (PSS) developed keloidal-like nodules within areas of thickened skin. This extremely unusual event is most likely a keloidal response to the early inflammatory component of scleroderma in patients who are either genetically at risk for keloid formation or in areas of the skin that have a high predilection for keloid formation such as the chest.

Keloids: the natural history.

There is abundant evidence in the literature that keloids and hypertrophic scars are different stages of the same process. The evidence, clinical and histogenetic, also shows that the hypertrophic scar is the earlier of the two stages. It therefore follows that all keloids do evolve through the hypertrophic scar phase. These conclusions have largely eased the confusion of nomenclature and made a clinicopathologic correlation of the available knowledge on keloid possible.
