OBJECTIVE: To ascertain if cases of radial keratotomy wound rupture were occurring and whether the globes ruptured through the corneal incisions.
DESIGN: Cases of traumatic ruptured globe after incisional corneal refractive surgery were collected from ophthalmologists and from peer-reviewed and other ophthalmic literature.
RESULTS: Twenty-eight human eyes (eight previously unreported) are known to have ruptured through refractive corneal incisions in activities of daily living (n = 12), assault (n = 7), motor vehicle accidents (n = 5), and sports (n = 4). Two patients died of their injuries.

Although topical anesthesia is commonly used for radial keratotomy, a regional intraorbital block may offer better control of the eye in anxious or uncooperative patients. Some complications of retrobulbar injections can produce severe loss of vision. Besides perforation of the globe and optic atrophy due to nerve perineural or intraneural injection of the anesthetic, posterior orbital hemorrhages have been reported, with consequent proptosis, tightness of the eyelids, and even occlusion of the central retinal artery. We report an unusual complication: blood staining of the cornea.

Severe sight-threatening complications were seen in five eyes of three patients following improperly performed radial keratotomy. All patients exhibited neovascularization of the incision sites, severe stromal scarring or ulceration involving the visual axis, loss of the anterior chamber, and iridocorneal adhesions. 20 months ago, 204 eyes have been operated for radial keratotomy. 175 cases of myopia varying from 2 to 6 diopters and 29 cases from 6.5 to 10 diopters. In addition to their myopia 39 cases had a myopic astigmatism. We did only 8 radial incisions with the diamond knife for all the cases of simple myopia and an optic center of 3 and 3.5 mm. A 75-year-old man with an ocular history of 8-cut radial keratotomy (RK) in both eyes presented for cataract surgery evaluation. We performed a penetrating keratoplasty with a donor corneal graft who had been operated by radial keratotomy one year ago. The clinical and endothelial cell density follow-up was 18 months. The endothelial cell loss was comparable to that of a non-operated donor graft. No complications such as reopening or vascular invasion of the incisions occurred. The authors conclude that radial keratotomy is not a contraindication for transplantation and suggest the use of such grafts for eyes with bullous keratopathy with a poor functional prognosis.

PURPOSE: To report complications of radial thermokeratoplasty (RTK) when used in combination with either radial keratotomy (RK) or astigmatic keratotomy (AK).
METHODS: RTK is a technique for the surgical correction of hyperopia and presbyopia. 600 degrees C burns are applied to the peripheral cornea for 0.3 seconds using a specialized cautery probe. The thermal energy generated shrinks stromal collagen and flattens the peripheral cornea. The central cornea is steepened producing a myopic shift.

OBJECTIVE: The study was designed to improve the safety and predictability of radial keratotomy (RK).
METHOD: Penetrating keratoplasty was performed on two eyes following RK for 2 months and 2 years respectively and their corneal buttons were studied histopathologically.
RESULTS: The study revealed that at 2 months, there was only bridge-like healing of corneal epithelium, and oblique "V"-shaped, discontinuous or shallow incisions were seen, at 2 years, the healing process of partial incisions had not completed, and epithelial emboli were seen in the deep part of some incisions.

Using corneal topography we screened 1,368 consecutive eyes that had radial keratotomy from November 1991 through April 1993. Fourteen eyes in nine patients exhibited topographic patterns consistent with "keratoconus suspect" status, but only one had atypical clinical findings. At four to 19 months postoperatively, refraction and uncorrected visual acuity improved in all eyes. Eight eyes gained best corrected visual acuity. To date, all 14 eyes appear to be stable, with no radial-keratotomy-related complications.

BACKGROUND: The edges of radial keratotomy incisions remain separated by epithelium or new collagen. To determine if the amount of gape of the incision edges agrees with the induced corneal flattening, two mathematical models of the cornea were studied.
METHODS: A finite element model of the eye was constructed; radial incisions through Bowman's membrane and stroma were simulated, as well as incisions confined to the stroma but not involving Bowman's or Descemet's membranes. INTRODUCTION: We report the case of a patient operated on for penetrating keratoplasty with a corneal graft with radial keratotomy.
CASE REPORT: A 27-year-old man underwent penetrating keratoplasty for keratoconus. Graft incisions of radial keratotomy were discovered the day after surgery. Corneal graft examination and endothelial cell density were carried out postoperatively.

To report a case of corneal rupture due to blunt trauma occurring 20 years after radial keratotomy (RK) and transverse keratotomy. A 57-year-old woman sustained direct blunt trauma to her right eye.
