The Artificial Kidney Initiation in Kidney Injury 2 (AKIKI2): study protocol for a randomized controlled trial.

BACKGROUND: The Artificial Kidney Initiation in Kidney Injury (AKIKI) trial showed that a delayed renal replacement therapy (RRT) strategy for severe acute kidney injury (AKI) in critically ill patients was safe and associated with major reduction in RRT initiation compared with an early strategy. The five criteria which mandated RRT initiation in the delayed arm were: severe hyperkalemia, severe acidosis, acute pulmonary edema due to fluid overload resulting in severe hypoxemia, serum urea concentration > 40 mmol/l and oliguria/anuria > 72 h. However, duration of anuria/oliguria and level of

Prediction of Kidney Graft Rejection Using Artificial Neural Network.

OBJECTIVES: Kidney transplantation is the best renal replacement therapy for patients with end-stage renal disease. Several studies have attempted to identify predisposing factors of graft rejection; however, the results have been inconsistent. We aimed to identify prognostic factors associated with kidney transplant rejection using the artificial neural network (ANN) approach and to compare the results with those obtained by logistic regression (LR). METHODS: The study used information regarding 378 patients who had undergone kidney transplantation from a retrospective study conducted in Ham

[Historical archives of Italian nephrology: Introduction of the artificial kidney in Italy].

In Italy the first attempts at haemodialysis, first in vitro and animals and then in humans, were carried out in the early 1950s by the surgeons Mario Battezzati e Carlo Taddei of Genoa with an artificial kidney of their own design. During the same years several other surgeons and urologists in Padua, Milan, Turin, Florence, Modena, Rome, Naples and Palermo designed their own artificial kidneys, which were used to treat anuric patients suffering from acute renal failure. The lively interest in artificial kidneys led, in 1954, to the organisation of a symposium on the subject. This was the firs

Application of artificial intelligence to chronic kidney disease mineral bone disorder.

The global derangement of mineral metabolism that accompanies chronic kidney disease (CKD-MBD) is a major driver of the accelerated mortality for individuals with kidney disease. Advances in the delivery of dialysis, in the composition of phosphate binders, and in the therapies directed towards secondary hyperparathyroidism have failed to improve the cardiovascular event profile in this population. Many obstacles have prevented progress in this field including the incomplete understanding of pathophysiology, the lack of clinical targets for early stages of chronic kidney disease, and the remar

Predicting long-term outcomes of kidney transplantation in the era of artificial intelligence.

The ability to accurately predict long-term kidney transplant survival can assist nephrologists in making therapeutic decisions. However, predicting kidney transplantation (KT) outcomes is challenging due to the complexity of the factors involved. Artificial intelligence (AI) has become an increasingly important tool in the prediction of medical outcomes. Our goal was to utilize both conventional and AI-based methods to predict long-term kidney transplant survival. Our study included 407 KTs divided into two groups (group A: with a graft lifespan greater than 5 years and group B: with poor gra

[Wasting in chronic kidney disease: Refeeding techniques and artificial nutrition practices].

Protein energy wasting (PEW) is an independent factor associated with morbi-mortality in chronic kidney disease. Wasting is particularly common in chronic diseases of organs such as kidney disease with a major impact at the stage of dialysis. It covers 20 to 70% of patients diagnosed with chronic kidney disease according to the degree of evolution of the disease and the diagnostic method used patients. Mechanisms of PEW are based mainly on anorexia and metabolic abnormalities caused by kidney disease. Nutritional treatment differs depending on the stage of the kidney disease acute or chronic t

Bridging structure and function: artificial intelligence-based modelling of kidney proteins.

The ability to predict the three-dimensional structure of a protein from its amino acid sequence has potential to provide insights into its function, and in the context of disease, its pathogenic mechanisms and potential drug targets. Artificial intelligence (AI)-driven algorithms, particularly AlphaFold and RoseTTAFold, have revolutionized the field of protein modelling, enabling rapid, high-confidence predictions of protein structures. In nephrology, these advances have clarified the molecular architecture of key renal systems such as podocyte slit diaphragm complexes, the conformational sta

Combined heart-kidney transplantation after total artificial heart insertion.

We present the first single-center report of 2 consecutive cases of combined heart and kidney transplantation after insertion of a total artificial heart (TAH). Both patients had advanced heart failure and developed dialysis-dependent renal failure after implantation of the TAH. The 2 patients underwent successful heart and kidney transplantation, with restoration of normal heart and kidney function. On the basis of this limited experience, we consider TAH a safe and feasible option for bridging carefully selected patients with heart and kidney failure to combined heart and kidney transplantat

Thrombotic microangiopathies and acute kidney injury induced by artificial termination of pregnancy.

Thrombotic microangiopathy (TMA) is a rare, but potentially lethal condition requiring rapid recognition, diagnosis and initiation of therapy. Here, we present two cases of women with hemolytic anemia, thrombocytopenia and acute kidney injury shortly after surgical termination of pregnancy. Histological examination of their kidneys revealed endothelial cell swelling and luminal stenosis or fibrin-containing thrombi in the glomeruli and arterioles, which support the diagnosis of TMA. The patients were treated with hemodialysis, plasma infusion and corticosteroids with or without immunosuppressi
