Kingella kingae septic arthritis in children: recognising an elusive pathogen.

PURPOSE: Kingella kingae is an increasingly identified cause of musculoskeletal infections in young children. We report our experience with a recently developed polymerase chain reaction (PCR) method and review the clinical course of children diagnosed with K. kingae septic arthritis in a tertiary referral paediatric hospital. METHODS: All positive cases of K. kingae identified by PCR analysis of synovial fluid from August 2010 until July 2013 were included. A chart review was undertaken to determine history, presentation and management. RESULTS: 27 Children (14 male, 13 female) had PCR posi

Near-Fatal, Delayed Diagnosis of Kingella kingae Endocarditis in an Infant.

We report a case of a 15-month-old male patient who presented with a two-week history of fever, lethargy, and mild respiratory symptoms. Initial investigations showed markedly elevated inflammatory markers, and chest imaging was interpreted as pneumonia. The patient was discharged on oral amoxicillin. Blood cultures later identified Kingella kingae (K. kingae), but no change in antibiotic therapy or follow-up was initiated due to apparent clinical improvement. Thirteen days later, the patient was readmitted in septic condition with a large mitral valve vegetation. He developed cardiac arrest d

Osteoarticular infections: Blood as a determinant factor in the isolation of Kingella kingae.

We assessed the capacity of Kingella kingae to grow in blood culture bottles (BCB), taking into account the concentrations of the microorganism and blood in the culture medium. An initial suspension (McFarland 0.5) of 32 strains of K. kingae was serially diluted. One mL of the initial suspension and 1 mL of the subsequent dilutions were inoculated in two BCB, together with 1 mL of human blood in the 2nd BCB. Also, 1mL serial dilutions of human blood were added to BCBs previously inoculated with 1 mL of K. kingae dilution 1/104. In non-blood-supplemented BCB, 23 strains grew with the initial su

The Past, Present, and Future of Kingella kingae Detection in Pediatric Osteoarthritis.

As a result of the increasing use of improved detection methods, Kingella kingae, a Gram-negative component of the pediatric oropharyngeal microbiota, is increasingly appreciated as the prime etiology of septic arthritis, osteomyelitis, and spondylodiscitis in children aged 6 to 48 months. The medical literature was reviewed to summarize the laboratory methods required for detecting the organism. Kingella kingae is notoriously fastidious, and seeding skeletal system samples onto solid culture media usually fails to isolate it. Inoculation of synovial fluid aspirates and bone exudates into bloo

Native joint septic arthritis due to Kingella kingae in an adult.

A 65-year-old woman with chronic osteoarthritis of the knees presented with a one-week history of acutely worsening right knee pain and swelling. Arthrocentesis was performed and synovial fluid was indicative of septic arthritis with a negative Gram stain for bacteria. Magnetic Resonance Imaging (MRI) was obtained, revealing a large anterior periarticular abscess with concomitant septic arthritis. Orthopedic surgeons performed urgent incision and drainage of the abscess and washout of the joint. Synovial fluid culture grew Kingella kingae and the patient was treated with four weeks of ceftriax

Five cases of Kingella kingae skeletal infection in a French hospital.

Five cases of Kingella kingae skeletal infections were diagnosed in children admitted to La Timone Hospital between 1992 and 1997. Patients were between 6 and 31 months old and presented with septic spondylodiskitis, calcaneus osteomyelitis, and hip-joint arthritis. All displayed either an upper respiratory tract infection or eczema during the month prior to their admission. Laboratory findings included an elevated leukocyte count and an elevated erythrocyte sedimentation rate. Standard radiography was unrevealing, but 99mTc bone scans and magnetic resonance imaging showed significant abnormal

Increasing prevalence of Kingella kingae in osteoarticular infections in young children.

Sixty children younger than 3 years with culture-positive hematogenous septic arthritis and acute/subacute osteomyelitis treated between 1990 and 1995 were reviewed to identify the infecting organism. Gram-positive bacteria were identified in 47 (78.3%) patients, and gram-negative organisms were identified in 13 (21.7%) patients. Haemophilus influenzae was cultured in none of the cases of septic arthritis and in only one (1.6%) case of acute osteomyelitis. Kingella kingae was cultured in 10 (16.7%) cases, with all of these patients between the ages of 10.5 and 23.5 months. Routine immunization

Kingella kingae: an emerging cause of invasive infections in young children.

Kingella kingae, a fastidious hemolytic gram-negative bacillus once considered to be an exceptional cause of disease, has emerged in recent years as an important invasive pathogen in children. When synovial fluid and other exudates were inoculated into blood culture bottles, enhanced recovery of the organism was observed, and an annual incidence of invasive K. kingae infections of 27.4 per 100,000 children younger than age 24 months was demonstrated in southern Israel. Skeletal infections are the most common clinical presentation of K. kingae, and studies conducted in that region have shown th

Osteoarticular Infections in Children: Accurately Distinguishing between MSSA and Kingella kingae.

Introduction: Osteoarticular infections (OAIs) constitute serious paediatric conditions that may cause severe complications. Identifying the causative organism is one of the mainstays of the care process, since its detection will confirm the diagnosis, enable adjustments to antibiotic therapy and thus optimize outcomes. Two bacteria account for the majority of OAIs before 16 years of age: Staphylococcus aureus is known for affecting the older child, whereas Kingella kingae affects infants and children younger than 4 years old. We aimed to better define clinical characteristic and biological cr

Characterization of TEM-1 β-Lactamase-Producing Kingella kingae Clinical Isolates.

Kingella kingae is a human pathogen that causes pediatric osteoarticular infections and infective endocarditis in children and adults. The bacterium is usually susceptible to β-lactam antibiotics, although β-lactam resistance has been reported in rare isolates. This study was conducted to identify β-lactam-resistant strains and to characterize the resistance mechanism. Screening of a set of 90 K. kingae clinical isolates obtained from different geographic locations revealed high-level resistance to penicillins among 25% of the strains isolated from Minnesota and Iceland. These strains produced

An Evaluation of Novel Inflammatory Biomarkers in Children with Kingella kingae Osteoarticular Infections.

Kingella kingae is now recognised as the leading cause of osteoarticular infections (OAIs) in young children. Because these infections typically produce mild symptoms and a weak inflammatory response, early diagnosis remains challenging. Complete blood count-derived immune-inflammatory biomarkers (IIBs) have recently gained interest as accessible, low-cost indicators of systemic inflammation, with potential diagnostic value in several fields, including oncology, rheumatology, cardiology, and infectious diseases. This study therefore aimed to assess whether IIBs could support the screening of K

Pediatric Osteoarticular Kingella kingae Infections of the Hand and Wrist: A Retrospective Study.

Our understanding of pediatric osteoarticular infections (OAIs) has improved significantly in recent decades. Kingella kingae is now recognized as the most common pathogen responsible for OAIs in pediatric populations younger than 4 years old. Research has provided a better understanding of the specific types, clinical characteristics, biological repercussions, and functional outcomes of these infections. Hands and wrists are rarely infected, with few reports available in the literature. The present study aimed to examine this specific condition in a large patient cohort, explore the implicati

Surface Anchoring of the Kingella kingae Galactan Is Dependent on the Lipopolysaccharide O-Antigen.

Kingella kingae is a leading cause of bone and joint infections and other invasive diseases in young children. A key K. kingae virulence determinant is a secreted exopolysaccharide that mediates resistance to serum complement and neutrophils and is required for full pathogenicity. The K. kingae exopolysaccharide is a galactofuranose homopolymer called galactan and is encoded by the pamABC genes in the pamABCDE locus. In this study, we sought to define the mechanism by which galactan is tethered on the bacterial surface, a prerequisite for mediating evasion of host immune mechanisms. We found t
