Kounis syndrome is defined as the co-incidental occurrence of an acute coronary syndrome with hypersensitivity reactions following an allergenic event and was first described by Kounis and Zavras in 1991 as an allergic angina syndrome. Multiple causes have been described and most of the data in the literature are derived from the description of clinical cases - mostly in adult patients - and the pathophysiology remains only partly explained. Three different variants of Kounis syndrome have been defined: type I (without coronary disease) is defined as chest pain during an acute allergic reactio

We present a case of Kounis-Zavras syndrome in the setting of aspirin-induced asthma, or the Samter-Beer triad of asthma, nasal polyps, and aspirin allergy. The Kounis-Zavras syndrome, also known as Kounis syndrome, leads to angina pectoris or acute coronary syndrome secondary to coronary vasospasm in response to an allergic stimulus, leading to mast-cell degranulation of vasospastic mediators. The vasospasm can result in myocardial infarction, as it did in our patient.

Polycystic ovary syndrome is characterized by ovulatory dysfunction, androgen excess and polycystic ovaries and is associated with hypertension, diabetes, metabolic syndrome and cardiovascular events. Oral contraceptives constitute first-line treatment, particularly when symptomatic hyperandrogenism is present. However, these drugs are associated with cardiovascular events and hypersensitivity reactions that pose problem in differential diagnosis and therapy. We present a 14 year-old female with polycystic ovary syndrome taking oral contraceptive and suffering from recurrent coronary ischemic a

In this case report, we describe a 70-year-old male patient who sustained Kounis syndrome induced by cisatracurium administration immediately following induction of general anesthesia. Acute coronary syndrome combined with anaphylactic shock, termed Kounis syndrome, should be investigated in percutaneous coronary intervention to solve this complex and life-threatening condition. A team effort by cardiologist and anesthesiologist is essential for successful resuscitation. In general, the incidence of an anaphylactic reaction to cisatracurium is low, but a high serum IgE level in combination wit

Kounis syndrome (KS), also known as allergic angina, is a type-1 hypersensitivity reaction affecting the coronary vessels, leading to vasospasm. It manifests like an acute coronary syndrome on the electrocardiogram and is often underdiagnosed. Foods (such as peanuts and fish) and medications (mostly antibiotics and analgesics) are the common triggers. We report a 30-year-old patient who developed KS following a multivitamin injection.

BACKGROUND: Kounis syndrome is an under-recognized cause of acute coronary syndrome precipitated by allergic or anaphylactic reactions. Recognition is critical, as management requires simultaneous treatment of both conditions.
CASE SUMMARY: A 64-year-old woman with cardiovascular risk factors developed anaphylaxis after amoxicillin ingestion. Despite treatment with epinephrine and corticosteroids, she developed chest pain, with electrocardiogram showing inferior ST-segment elevations. Emergent catheterization revealed an occluded mid right coronary artery with nonobstructive left coronary dis

Kounis syndrome is an acute coronary syndrome triggered by allergic hypersensitivity to various stimuli. Here, we report the case of a 54-year-old man who had coronary stent-induced type III Kounis syndrome. The patient developed a rash on the anterior chest wall and experienced throat discomfort 40 minutes after stent implantation in the left anterior descending artery. Given his history of allergies, a presumptive diagnosis of anaphylaxis was made, and 0.5 mg of adrenaline was injected intramuscularly into the left thigh. The patient's symptoms initially resolved; however, approximately 50 m

BACKGROUND: Acute coronary syndrome triggered by hypersensitivity to various natural, chemical, or pharmaceutical allergens is known as Kounis syndrome. Kounis syndrome is classified in three subtypes based on its pathophysiological mechanism and outcome. Among pharmaceuticals, cephalosporins are frequently implicated in allergic illnesses, with ceftriaxone being one of the most frequently reported causes. Based on literature reports and our own experience, we focused on the main clinical features regarding ceftriaxone triggered KS (CTKS).
METHODS: Medical records from all CTKS cases, as publ

Kounis syndrome is an underreported diagnosis, even though it is a frequent pathology. It is an acute coronary syndrome that can present in the setting of an allergic or an anaphylactic reaction that could be triggered by the use of drugs. We present the case of a patient with a known medical history of drug allergies (but not of anaphylaxis), who presented to the emergency department with complaints of headache. After the administration of a painkiller, she developed hypotension, syncope, elevation of troponin, and transitory changes in the ST segment of the electrocardiogram.

BACKGROUND: Kounis syndrome is an acute coronary syndrome (ACS) caused by allergic reactions, including coronary artery spasm (type I) caused by allergies without coronary predisposing factors, pre-existing coronary atherosclerosis, and coronary artery disease. Anaphylaxis leads to plaque rupture or erosion leading to acute myocardial infarction (type II) and acute coronary stent thrombosis (type III). Here we share a case of Kounis syndrome type I caused by an allergy caused by a Cryptopteran bite.
CASE PRESENTATION: A 47-year-old woman was admitted to the hospital due to an insect bite for

We report the case of capecitabine-induced ventricular fibrillation arrest, possibly secondary to type I Kounis syndrome. A 47-year-old man with a history of T3N1 moderately differentiated adenocarcinoma of the colon, status-post sigmoid resection, was started on adjuvant capecitabine approximately five months prior to presentation of cardiac arrest secondary to ventricular fibrillation. An electrocardiogram (EKG) revealed ST segment elevation on the lateral leads and the patient was taken emergently to the cardiac catheterization laboratory. The catheterization revealed no angiographically si

Metal-induced allergic reactions are not rare in every day practice but nickel, cobalt and chromium are the most common offenders. Other metal anions and metal alloys represent also emerging causes for hypersensitivity reaction in humans. The metal struts of endovascular and intracardiac devices are usually alloys containing nickel and constitute causes for allergic reactions with possible intracardiac and intracoronary mast cell activation resulting in the Kounis hypersensitivity coronary syndrome. Newer intracoronary stents avoid nickel thus making them less allergenic. It is advisable that,

Kounis syndrome is an under-recognized condition characterized by the simultaneous occurrence of acute coronary syndrome and hypersensitivity reactions. While opioids are known triggers, tramadol-induced Kounis syndrome is extremely rare. A 49-year-old woman with a history of allergy to dimenhydrinate and chronic low back pain developed anaphylactic shock 4 min after receiving 50 mg of subcutaneous tramadol. She developed with dyspnea, glottic edema, muscle rigidity, chest pain, and hypotension. After stabilization with epinephrine, corticosteroids, antihistamines, fluids, and vasopressors, el

A 50-year-old male patient was admitted to the Cardiology Department of our hospital for intermittent chest pain for 9 days. Coronary angiography showed approximately 70% stenosis in the middle part of the left anterior descending branch. When the procedure was about to end, the patient experienced dyspnea, facial flushing and fall of blood pressure suddenly. At this time, the angiography showed severe spasm and stenosis of the left coronary artery. With antianaphylaxis, pressor therapy, fluid resuscitation and intracoronary administration of nitroglycerin, the left coronary spasm was relieved

Kounis syndrome (KS) is a rare syndrome characterized by the co-occurrence of acute coronary syndromes in the setting of mast cell and platelet activation in response to hypersensitivity reactions. It can be manifested as coronary vasospasms, acute myocardial infarction, or stent thrombosis triggered by drugs, vaccines, foods, coronary stents, and insect bites. It is a life-threatening condition that needs to be adequately recognized for early diagnosis and appropriate treatment. In this case report, we present a 71-year-old patient with a history of arterial hypertension and non-ST elevation

Kounis syndrome commonly is described as a complex multisystem phenomenon mainly affecting coronary arteries, resulting in coronary vasospasm in the context of an allergic manifestation. This article reviews the literature regarding perioperative presentations of the syndrome. A systematic search in MEDLINE and Embase databases was performed for case reports through June 16, 2021, on Kounis syndrome triggered by medications administered in the perioperative setting. The authors' search resulted in 35 perioperative reports of Kounis syndrome, with the majority of the cases occurring in men betw
