Alpha-adrenoceptor blockade by labetalol during long-term dosing.

The hypotensive effect of short-term labetalol, the alpha- and beta-adrenoceptor blocker, is greater in subjects in the orthostatic position, possibly because of the alpha-adrenoceptor blockade. During prolonged use the orthostatic blood pressure fall disappears. To verify whether or not this is due to reduction in alpha-blocking activity, phenylephrine-induced increase in blood pressure was studied in six subjects with mild essential hypertension before and after 3 and 6 days and 1 and 6 mo of continuous treatment with 200 mg labetalol three times a day by mouth. At the same intervals, isopro

A case of probable labetalol induced hyperkalaemia in pre-eclampsia.

CASE DESCRIPTION: Hyperkalemia can cause altered cardiac electrical conduction resulting in death. We describe a case of a 23-year old pregnant patient who presented with severe epigastric pain and vomiting. She was severely pre- eclamptic and received initial treatment with intravenous labetalol and decision was taken to deliver. She quickly became hyperkalaemic (serum potassium level 6.4 mmol/L) and labetalol was discontinued and intravenous hydralazine commenced. Post-surgery, her potassium levels were normal but due to rapidly rising blood pressure labetalol was recommenced, resulting in e

Labetalol vs. methyldopa in the treatment of pregnancy-induced hypertension.

OBJECTIVE: To assess the efficacy and safety of labetalol compared with methyldopa in the management of mild and moderate cases of pregnancy-induced hypertension (PIH). METHODS: One hundred four primigravidas with PIH were randomly allocated to receive either labetalol (group A) or methyldopa (group B). The dose of the drugs was doubled every 48 h to maintain a mean arterial blood pressure < or = 103.6 mmHg. Clinico-biochemical effects and frequency of side effects were studied. The statistical level of significance was taken at P < 0.05. RESULTS: Ten patients in group B (18.5%) developed si

Coronary vasospasm secondary to labetalol in a patient with aortic dissection.

A 66-year-old hypertensive woman presented with epigastric and scapular pain on the basis of type 3 aortic dissection. Appropriate therapy with a combined alpha-adrenergic and beta-adrenergic antagonist agent prevented further ongoing dissection and amelioration of symptoms. On day 5, an episode of coronary vasospasm occurred presumably due to beta-blockade with unopposed alpha-adrenergic activity.

Intravenous labetalol in the treatment of severe hypertension and hypertensive emergencies.

The antihypertensive effects of intravenous labetalol were evaluated in 59 patients with hypertensive crises or severe hypertension in need of rapid lowering of blood pressure in a multicenter study. Patients appearing with a supine diastolic blood pressure 125 mm Hg or greater, or a supine systolic blood pressure of more than 200 mm Hg received an initial mini-bolus injection (20 mg) of labetalol. This was followed by repeated incremental doses of 20 to 80 mg given at 10 minute intervals to achieve a supine diastolic blood pressure of less than 95 mm Hg or decrease 30 mm Hg or greater, or a s

Labetalol in tetanus. The treatment of sympathetic nervous system overactivity.

The cardiovascular instability in some cases of severe tetanus is due to increased circulating catecholamines. In 15 patients with this complication of tetanus, labetalol, a drug with alpha- and beta-peripheral adrenergic blocking properties, was used in management. The drug was administered orally, by i.v. bolus or by continuous infusion. Wide variation in dosage was needed both from case to case and in the same patient during the course of his disease. In most cases the pulse and blood pressure were reduced by labetalol, although their variability was not much improved. The effect on the car

Analgesic effect of peridural labetalol in the treatment of cancer pain.

Labetalol is an alpha- and beta-blocking agent commonly used in anti-hypertensive therapy. Because of its beta-blocking and local anesthetic effect, labetalol via peridural catheter was supposed to reduce pain in patients suffering from gynecologic cancers. Thirty patients with terminal carcinomas (breast, uterus, ovary), whose pain was caused either by bone metastases or by primary invasion cancer, were treated. Peridural catheters were inserted and fixed at various levels, thoracic or lumbar, and 50 mg labetalol was injected every 4 h at the beginning and every 12-24 h on the following 2-3 d

Circulatory effects of noradrenaline and adrenaline before and after labetalol.

1 Two studies were carried out in the same six normal healthy males to compare the circulatory effects of exogenously infused noradrenaline and adrenaline before and after intravenous labetalol. 2 Noradrenaline before and after labetalol produced a similar pattern of circulatory change, namely dose related systolic and diastolic pressor responses accompanied by profound bradycardia and reduced cardiac output. 3 Labetalol competitively antagonized both the systolic and diastolic pressor effects of noradrenaline. 4 Adrenaline at low doses produced a diastolic depressor response accompanied by in

Metabolic Activation and Cytotoxicity of Labetalol Hydrochloride Mediated by Sulfotransferases.

Labetalol hydrochloride (LHCl), an α- and β-adrenoreceptor blocker, is widely used for the treatment of hypertension as well as angina pectoris. Previous reports have demonstrated the adverse events during clinical application of LHCl, such as liver injury and acute renal failure. The present study aimed to investigate metabolic activation of LHCl to initiate the elucidation of the mechanisms of its liver toxicity. One glutathione (GSH) conjugate was detected in rat and human primary hepatocytes as well as bile of rats after exposure to LHCl. The GSH conjugate was chemically synthesized and ch

Use of enoximone in management of anaphylaxis complicated by labetalol use.

A 42-year-old woman with end-stage renal failure was admitted to the intensive care unit following resuscitation from a pulseless electrical activity cardiac arrest after intravenous piperacillin/tazobactam. Persistent bradycardia and hypotension, unresponsive to epinephrine and norepinephrine, were suspected to have been exacerbated by chronic labetalol therapy for resistant arterial hypertension. As an alternative, the non-adrenergic inotrope, enoximone, was started. This, combined with thrombolysis for possible pulmonary embolism, heralded significant haemodynamic improvement, allowing wean

High dose intravenous labetalol for the resection of metastatic pheochromocytoma.

A case of intrahepatic metastatic pheochromocytoma with lymph nodes involvement undergoing resection of tumor was reported; intravenous labetalol in high dose regimen (2 mg/kg administered before skin incision) was used for blood pressure control during the operation. Marked sinus bradycardia was noted after receiving total dose of intravenous labetalol and treated [corrected] with intravenous atropine 0.4 mg. The total operation time was 6.5 h. We found that under isoflurane anesthesia, duration of action of intravenous labetalol was prolonged as adequate intraoperative blood pressure control

Labetalol in the treatment of patients with hypertension and renal function impairment.

1 Labetalol has been used to treat patients with renal hypertension or hypertension in the presence of renal functional impairment. In some patients changes in the glomerular filtration rate (GFR) were followed during the treatment. 2 Sixty patients were treated with labetalol for up to 24 months. Forty-nine of the patients were taking a diuretic and 23 were receiving additional antihypertensive therapy. The antihypertensive effect of labetalol has so far been assessed in 51 patients. Forty-six of these 51 patients have responded with a fall in the mean supine BP from 184/115 to 148/93 mmHg an

Controlled comparison of labetalol and propranolol in the management of severe hypertension.

1 Labetalol, a new hypotensive drug combining alpha- and beta-adrenoreceptor antagonist properties, has been compared with propranolol in the treatment of severe hypertension (blood pressure 190/115-249/139 mmHg) in a double-blind trial lasting 14 weeks. Additional diuretic therapy was given to both groups of patients. 2 Both drugs caused an effective reduction in blood pressure, bbut labetalol caused a greater fall in pressure in the standing position and after exercise. Two groups of nine patients have each completed the trial so far. Group average pressures for the last 3 weeks of treatment

Acute Toxicity from Topical Cocaine for Epistaxis: Treatment with Labetalol.

BACKGROUND: Topical cocaine is sometimes used for the treatment of epistaxis, as it has both potent anesthetic and vasoconstrictive properties. Cocaine has unpredictable cardiovascular effects, such as sudden hypertension, tachycardia, coronary arterial vasoconstriction, and dysrhythmia. CASE REPORT: We report a case of acute iatrogenic cardiovascular toxicity from the use of topical cocaine in a 56-year-old man presenting to the Emergency Department with profound epistaxis. To prepare for cauterization and nasal packing, the patient received 4% topical cocaine-soaked nasal pledgets. He becam
