Overcoming barriers to advanced biomolecular technologies that inform treatment of solid tumors: a roadmap to access.

The advent of advanced biomolecular technologies for detecting molecular and genomic signatures of individual tumors has transformed oncology care, introducing proven methodologies that can inform treatment with matched targeted therapies and predict response at the individual patient level. However, access to these technologies has been hampered by multiple barriers, most notably price and obtainability. Other barriers include lack of knowledge of available technologies, concerns about value, and outdated infrastructures that impede critical operations within the clinic or laboratory. Accessi

Falsely elevated sodium levels during thiopental treatment in the ICU: technical interference on a laboratory device with important clinical relevance.

INTRODUCTION: Thiopental is a cornerstone in the treatment of refractory status epilepticus and intractable intracranial hypertension. In our center we observed that thiopental might cause falsely elevated serum sodium levels. METHODS: Triggered by a recent case experience of extremely elevated serum sodium levels during thiopental treatment, we retrospectively identified 53 patients treated with thiopental in our intensive care unit between 2007 and 2011 and evaluated electrolyte changes. We differentiated the analysis before and after introduction of a new device for sodium assays (Dimensio

[Laboratory markers of metabolic syndrome in clinical practice].

The incidence of metabolic syndrome is evidenced by the presence of three out of five criteria: larger waistline, elevated blood pressure, raised triglyceride levels, reduced HDL-cholesterol and raised fasting glycaemia (or diabetes mellitus). Laboratory picture of metabolic syndrome also includes presence of small LDL3 particles and increased concentration of apolipoprotein B. Determination of glycaemia and triglycerides and HDL-cholesterol levels is fundamental for the diagnosis of metabolic syndrome. The risk of the so-called ,,pre-analytical" error has to be minimized in order for the resu

Establishing national decision rules for forensic blood alcohol analysis in Italy: A dynamic framework incorporating measurement uncertainty and interlaboratory performance.

This article introduces a dynamic framework for reporting blood alcohol concentration (BAC) by integrating measurement uncertainty and interlaboratory variation. This approach is considered necessary to ensure more consistent decision-making rules when analytical results are used in a legal context. Although these rules have been developed within the Italian context, they are adaptable to other countries as well. Included in the framework are critical challenges stemming from variability in laboratory performance and method uncertainty. Data were collected through a national interlaboratory pr

Coagulation and complement studies in Rocky Mountain spotted fever.

A patient with laboratory-acquired Rocky Mountain spotted fever (RMSF) and concomitant influenza virus infection was studied from the third day of clinical illness. The course of his illness was marked by petechial rash, thrombocytopenia, and elevation of fibrin split products. No evidence of complement activation was observed. Plasma proteins were elevated in a pattern characteristic of the "acute phase reaction." The patient recovered completely, and vascular collapse or clinically important disseminated intravascular coagulation had developed. In febrile patients who had influenza or a clin

A rapidly personalized in-hospital bloodstream infection prediction model: a multicenter retrospective study.

BACKGROUND: Hospital-acquired bloodstream infections (BSIs) are associated with high morbidity and mortality. Early identification of high mortality risk patients is critical for improving outcomes. Existing prediction models, often based on single-center static indicators, lack generalizability and fail to capture dynamic physiological changes. This study aimed to develop a rapid and personalized prognostic model for mortality in patients with suspected BSIs using dynamic laboratory indicators from multicenter data. METHODS: A multicenter retrospective study was conducted using data from the

Endocrine assessment of high-risk pregnancies.

For critical obstetrical judgment, no single laboratory test or biophysical technique has proved completely effective in preventing fetal deaths. Endocrine assessment of high-risk pregnancies has proved helpful in managing pregnancies with diabetes, hypertension, third trimester bleeding, suspected IUGR, and postdate pregnancies. No single endocrine test has proved to be effective in all cases, and much research remains to be done. Of the current endocrine factors being evaluated when all factors are considered, serum unconjugated estriol would appear to be the best predictor of fetal distress

Assessment of the Specimen Size Effect on the Fracture Energy of Macro-Synthetic-Fiber-Reinforced Concrete.

The most frequently used construction material in buildings is concrete exhibiting a brittle behaviour. Adding fibers to concrete can improve its ductility and mechanical properties. To this end, a laboratory study was conducted to present an experimental model for the specimens' size effect of on macro-synthetic fiber-reinforced concrete using variations in fracture energy. Composite concrete beams with different thicknesses and widths were made and tested under mode I to obtain (1) fracture toughness, (2) fracture energy, and (3) critical stress intensity factor values. Results indicated tha

Septic Arthritis of the Wrist.

Septic arthritis of the wrist is an uncommon condition, but one that can result in substantial morbidity. Timely identification and treatment is critical to patient care. No serum laboratory values have been shown to consistently confirm wrist joint infection. Thus, diagnosis is made based mainly on a thorough patient history, physical examination, and joint aspiration. When infection is suspected, aspiration of the wrist should be performed to confirm the diagnosis. Broad-spectrum antibiotics and joint aspiration or surgery are required to manage the infection and prevent sequelae.

Point-of-care ultrasound to evaluate volume status in congestive heart failure.

An accurate volume status assessment is critical to the diagnosis and management of congestive heart failure. Clinically, volume status is typically assessed using a combination of physical examination findings, laboratory values, imaging studies, and invasive procedures. However, these techniques may be unreliable, cost-prohibitive, or impractical. Point-of-care ultrasound (POCUS) has the potential to improve the accuracy of volume assessments with little additional cost to the patient. This review summarizes POCUS techniques that can be used to determine volume status in patients with conges

Criticality accident dosimetry by chromosomal analysis.

The technique of measuring the frequency of dicentric chromosomal aberrations in blood lymphocytes was used to estimate doses in a simulated criticality accident. The simulation consisted of three exposures; approximately 5 Gy with a bare source and 1 and 2 Gy with a lead-shielded source. Three laboratories made separate estimates of the doses. These were made by the iterative method of apportioning the observed dicentric frequencies between the gamma and neutron components, taking account of a given gamma/neutron dose ratio, and referring the separated dicentric frequencies to dose-response c

The influence of the site of sampling and assay medium upon the measurement and interpretation of blood lactate responses to exercise.

UNLABELLED: This paper reports the findings of two investigations into methodological problems associated with the interpretation of blood lactate (BLa) in the sports sciences. In Experiment 1, brachial artery (A), antecubital venous (V) and fingertip capillary (C) blood samples were drawn simultaneously from nine subjects (mean age 21.1 +/- 1.3 years) during an incremental treadmill protocol and immediately assayed for BLa concentration. Experiment 2 investigated the extent of lactate concentration differences in whole blood (WB), lysed blood (LB) and plasma (P) measured using a YSI 23 AM ana

Stability of serial platelet and urine protein measurements in patients receiving nusinersen for spinal muscular atrophy.

INTRODUCTION/AIMS: Patients undergoing nusinersen treatment for spinal muscular atrophy are subject to measurements of platelet count and urine protein before each injection due to concern for platelet depletion and renal dysfunction according to the prescribing information. These tests may be uncomfortable or inconvenient and may cause delays in treatment. However, it is still unclear whether these values have been significantly affected by nusinersen treatment. Our aim in this study was to determine whether these measurements ever reached critical values that necessitated withholding treatme
