The anaesthesia management of a 10-year old girl with a fracture to her distal arm is presented, using inhalational anaesthesia with a laryngeal mask and a supplementary axillary plexus block. Due to light anaesthesia the patient vomited during the course of surgery resulting in a severe pulmonary aspiration necessitating four days of mechanical ventilation in the ICU. The safety of the laryngeal mask airway for emergency surgery is discussed.

The laryngeal mask airway (LMA) has been used extensively to provide a safe airway in spontaneously breathing patients who are not at risk from aspiration of gastric contents. The role of the LMA in the event of a failed intubation in an obstetrical patient, and its place in a failed intubation drill remains unclear. Two hundred and fifty consultant obstetric anaesthetists in the United Kingdom were asked to complete an anonymous questionnaire regarding their views about using the laryngeal mask airway (LMA) in obstetrical anaesthesia. The LMA was available in 91.4% of obstetric units. Seventy

The laryngeal mask airway (LMA) was first used at the Department of Orthopedics, School of Medicine, University of Zagreb on May 8, 1991. Two hundred and three patients were undergoing elective orthopedic surgery during the first year of the LMA use. A size-3 mask was used for women and children weighing over 25 kg (55 lbs) and a size-4 mask for men. Research has been undertaken in 12 patients aged between 30-73 years scheduled for total hip replacement. Blood pressure, heart rate and hemoglobin oxygen saturation were continuously monitored with a noninvasive method. No signs of cardiovascular

In 1985 Brain et al. published their first experience with the laryngeal mask, developed by themselves. With this mask it is possible to seal the larynx and ventilate a patient during anesthesia without endotracheal intubation. Meanwhile, further reports of successful use have been published, especially in Great Britain. We decided to investigate this new anesthetic device. In 15 patients (ASA groups I and II) undergoing elective operations in the supine position the laryngeal mask was inserted after induction of anesthesia with propofol and alfentanil (Fig. 1). Positioning of the laryngeal ma

Positive pressure ventilation (PPV) is crucial to neonatal cardiopulmonary resuscitation because respiratory failure precedes cardiac failure in newborns affected by perinatal asphyxia. Prolonged ineffective PPV could lead to a need for advanced resuscitation such as intubation, chest compression, and epinephrine. Every 30 s delay in initiation of PPV increased the risk of death or morbidity by 16%. The most effective interface for providing PPV in the early phases of resuscitation is still unclear. Laryngeal masks (LMs) are supraglottic airway devices that provide less invasive and relatively

OBJECTIVE: Correct and rapid positioning of reinforced laryngeal masks (LMA) is often a problem in anaesthetics. Although different techniques have been developed, none of them are satisfactory. A newly designed introduction device was used to place reinforced laryngeal masks. These are first results of a clinical observational study.
METHODS: A special fork fixed on a handhold was constructed. The introduction of the LMA was possible in an arched movement down to the hypopharynx using appropriate sizes for different sizes of LMA's.
RESULTS: The intubation in 500 surgical cases (30 % adult,

Background/Objectives: Supraglottic airway devices are commonly used airway management tools, with various second-generation laryngeal masks available for patients undergoing general anesthesia. These devices offer improved sealing, gastric suction capabilities, and the potential for tracheal intubation. This study compared the recently introduced SingularityTM Air laryngeal mask with two well-established devices, Ambu® AuraGainTM and i-GEL®, under clinical conditions. Methods: We prospectively included 98 adult patients scheduled for elective surgery requiring general anesthesia. Patients wer

BACKGROUND AND OBJECTIVE: The Supreme laryngeal mask airway (LMA) is a new single-use polyvinyl chloride supraglottic device that combines the functionality of the ProSeal and Fastrach airways. High oropharyngeal leak pressures are important as they indicate airway protection, feasibility of positive pressure ventilation and likelihood of successful LMA placement. The oropharyngeal leak pressure of the LMA Supreme is not well established versus the LMA ProSeal. This study was designed to compare the safety and efficacy of the LMA Supreme versus the LMA ProSeal in elective ambulatory procedures

BACKGROUND: The ProSeal laryngeal mask airway (PLMA) is a new laryngeal mask device with a modified cuff to improve seal and a drainage tube to provide a channel for regurgitated fluid and gastric tube placement. In the present randomized, crossover study, the authors tested the hypothesis that ease of insertion, airway sealing pressure, and fiberoptic position differ between the PLMA and the standard laryngeal mask airway (LMA). For the PLMA, we also assess ease of gastric tube placement and the efficacy of an introducer tool.
METHODS: Sixty paralyzed, anesthetized adult patients were studie

BACKGROUND AND AIMS: Laryngeal Mask Airway Protector (LMPt) and Laryngeal Mask Airway Proseal (LMPs) have a high oropharyngeal leak pressure (OLP) but have not been compared for laparoscopic surgery. It was hypothesized that LMPs and LMPt have different clinical performances while managing the airway of patients undergoing laparoscopic surgery. The aim of the study was to compare the LMPs and LMPt for managing the airway of patients undergoing laparoscopic surgery.
MATERIAL AND METHODS: This was a prospective, single-blind, randomized, comparative trial. It included adult patients of either g

BACKGROUND AND OBJECTIVES: Laryngeal mask airway (LMA), which has been used frequently in airway management, can cause laryngopharyngeal injury and morbidity. In this trial, we compare the macroscopic changes on laryngopharyngeal structures and the postoperative laryngopharyngeal morbidity by using LMA supreme with LMA proseal in children.
METHODS: We divided patients into two groups. We inserted size three LMA proseal into the first group and size three LMA supreme into the second group. Before LMA insertion and after LMA removal, we performed direct laryngoscopy on the patients. We compared

We report a case of aggravation of laryngeal rheumatoid arthritis (RA) after the use of a laryngeal mask airway (LMA). The patient was a 55-year-old woman with severe RA who underwent wrist arthrodesis under general anesthesia using a size 3 LMA. A few days after the operation, she reported hoarseness. Inflammation of the arytenoid region and vocal cord immobility were observed by fiberoptic nasolaryngoscopy. Vocal function returned to normal by postoperative day 42 in response to prednisolone therapy. In this case, stridor was misdiagnosed preoperatively, and postoperative symptoms and local

We report a case of difficult intubation with congenital laryngeal web. An 11-year-old boy was scheduled for urinary surgery. After anesthetic induction, the anesthesiologist encountered resistance at the vocal cord preventing satisfactory intubation. A size 3 laryngeal mask was immediately inserted, and an adequate airway was obtained. We found a laryngeal web at the vocal cord by bronchoscope inserted thorough the laryngeal mask. The peroperative course was uneventful. Immediately after removal of the laryngeal mask, the patient developed airway obstruction due to sputum and/or laryngeal spa
