[Respiration rhythm-controlled electric stimulation of paralyzed laryngeal muscles--animal experiment findings].

Patients with bilateral paresis of the recurrent laryngeal nerves have primarily breathing difficulties. But on the other hand the common operations for widening the glottis produce voice problems. To overcome these difficulties a muscle stimulation device was developed, which is triggered by the inspiratory motion of the thorax. With this device impulses are transmitted by radiofrequency from a transmitter located outside the body to a receiver implanted into subcutaneous tissue of the neck and thence by electrode wires to the posterior cricoarytenoid muscle (PCA). In the dogs with one cut re

Transcriptome Analysis of Left Versus Right Intrinsic Laryngeal Muscles Associated with Innervation.

OBJECTIVES/HYPOTHESIS: Recurrent laryngeal nerve injury diagnosed as idiopathic or due to short-term surgery-related intubation exhibits a higher incidence of left-sided paralysis. While this is often attributed to nerve length, it is hypothesized there are asymmetric differences in the expression of genes related to neuromuscular function that may impact reinnervation and contribute to this laterality phenomenon. To test this hypothesis, this study analyzes the transcriptome profiles of the intrinsic laryngeal muscles (ILMs), comparing gene expression in the left versus right, with particular

Blockade of glial-derived neurotrophic factor in laryngeal muscles promotes appropriate reinnervation.

OBJECTIVES/HYPOTHESIS: Synkinetic reinnervation of the laryngeal muscles is one of the causes of the poor functional recovery after a recurrent laryngeal nerve (RLN) injury. Glial-derived neurotrophic factor (GDNF) is elevated in rat laryngeal muscles during RLN reinnervation. The specific aim of this investigation was to evaluate the effect of anti-GDNF on RLN reinnervation. METHODS: Anti-GDNF antibody was injected into the posterior cricoarytenoid (PCA) 3 days following RLN transection and anastomosis. Larynges were harvested at 7, 14, 28, 56, and 112 days post injury (DPI). Prior to sacrif

Changes in neurotrophic factors of adult rat laryngeal muscles during nerve regeneration.

Injury to the recurrent laryngeal nerve (RLN) leads to the loss of ipsilateral laryngeal fold movement, with dysphonia, and occasionally dysphagia. Functional movement of the vocal folds is never restored due to misrouting of regenerating axons to agonist and antagonist laryngeal muscles. Changes of neurotrophic factor expression within denervated muscles occur after nerve injury and may influence nerve regeneration, axon guidance and muscle reinnervation. This study investigates the expression of certain neurotrophic factors in the laryngeal muscles during the course of axonal regeneration us

Muscle-nerve pedicle laryngeal reinnervation.

A muscle-nerve pedicle implantation to the lateral cricoarytenoid muscle as described by Tucker is an alternative to Teflon injection for treating dysphonia due to vocal cord palsy. Improvement in voice was noted in 19 of the 20 (95%) selected patients who were treated by muscle-nerve pedicle reinnervation. These patients have been followed for 6 months to 10 years. Changes in the voice were documented through assessment of high-quality audio tape recordings by three speech pathologists, results of a patient questionnaire, and evaluation by the surgeon. The improvement in voice quality was att

Dynamics of Intrinsic Laryngeal Muscle Contraction.

OBJECTIVES: Laryngeal function requires neuromuscular activation of the intrinsic laryngeal muscles (ILMs). Rapid activation of the ILMs occurs in cough, laughter, and voice-unvoiced-voiced segments in speech and singing. Abnormal activation is observed in hyperfunctional disorders such as vocal tremor and dystonia. In this study, we evaluate the dynamics of ILM contraction. STUDY/DESIGN: Basic science study in an in vivo canine model. METHODS: The following ILMs were stimulated: thyroarytenoid (TA), lateral cricoarytenoid/interarytenoid (LCA/IA), cricothyroid (CT), all laryngeal adductors (

Effect of dystrophin deficiency on selected intrinsic laryngeal muscles of the mdx mouse.

BACKGROUND: Intrinsic laryngeal muscles (ILM) show biological differences from the broader class of skeletal muscles. Yet most research regarding ILM specialization has been completed on a few muscles, most notably the thyroarytenoid and posterior cricoarytenoid. Little information exists regarding the biology of other ILM. Early evidence suggests that the interarytenoid (IA) and cricothyroid (CT) may be more similar to classic skeletal muscle than their associated laryngeal muscles. Knowledge of the IA and CT's similarity or dissimilarity to typical skeletal muscle may hold implications for t

Attachment of some internal laryngeal muscles at the base of the arytenoid cartilage.

The attachments of the lateral cricoarytenoid muscle, thyroarytenoid muscle and interarytenoid muscle at the base of the arytenoid cartilage were studied in six adult larynges. Fresh specimens were treated by the acetylthiocholine method, sections were made with a freezing microtome and stained with Van Gieson stain. The muscle fibres could thus be traced to their fibrous tendinous attachments. The extent of attachment of each muscle at the base of the arytenoid was traced and its relationship to the maximum concavity of the arytenoid articular facet was computed. The lateral cricoarytenoid mu

Differential expression of glial-derived neurotrophic factor in rat laryngeal muscles during reinnervation.

OBJECTIVES/HYPOTHESIS: Nonspecific, synkinetic reinnervation is one of the causes of poor functional recovery after a peripheral nerve lesion. Knowledge of the differential expression of neurotrophic factors that subserve axon guidance, as well as neuromuscular junction formation and maintenance in the denervated muscles, may allow appropriate interventions that will improve the functional nonsynkinetic reinnervation. STUDY DESIGN: Laboratory experiment. METHODS: The expression of glial-derived neurotrophic factor (GDNF) was studied in the abductor and adductor muscles of the larynx in the r

Functional electrical stimulation of intrinsic laryngeal muscles under varying loads in exercising horses.

Bilateral vocal fold paralysis (BVCP) is a life threatening condition and appears to be a good candidate for therapy using functional electrical stimulation (FES). Developing a working FES system has been technically difficult due to the inaccessible location and small size of the sole arytenoid abductor, the posterior cricoarytenoid (PCA) muscle. A naturally-occurring disease in horses shares many functional and etiological features with BVCP. In this study, the feasibility of FES for equine vocal fold paralysis was explored by testing arytenoid abduction evoked by electrical stimulation of t

Dystrophic pathology in the intrinsic and extrinsic laryngeal muscles in the mdx mouse.

OBJECTIVE: This study aimed to characterize the laryngeal muscles in the mdx mouse model of Duchenne muscular dystrophy (DMD). DESIGN: Pathology was analyzed in the intrinsic and extrinsic laryngeal muscles from mdx mice and compared with other mdx muscle groups and with muscles from age-matched normal control mice. SETTING: In DMD, a dystrophin protein deficiency causes skeletal muscle breakdown. However, some muscle groups, such as the extraocular muscles, are spared from damage. Characterizing other unaffected muscles may form the basis for developing new therapies to alleviate muscle bre

Three-dimensional changes in the upper airway during neuromuscular stimulation of laryngeal muscles.

During swallowing, airway protection depends upon adequate glottal closure and laryngeal elevation to prevent the entry of substances into the airway. Three-dimensional changes in the upper airway during laryngeal muscle stimulation in a canine model were quantified in animals implanted with Peterson type stimulating electrodes in the inferior and superior portions of the thyroarytenoid muscle, together with a reference electrode. Computer tomography scanning was performed on an IMATRON scanner with a 3 mm slice thickness advanced at overlapping 1 mm increments. Stimulation of the thyroaryteno
