It is estimated that one third of the world population is latently infected by Mycobacterium tuberculosis and thus at risk of reactivation. Latent tuberculosis (TB) impact in Switzerland is often overlooked. Diagnosis and prophylaxis are insufficiently undertaken, especially for people at higher risk of reactivation due to immunosuppression. Interferon-gamma release assays replace tuberculosis skin tests for diagnosis of latent infection in adults. It is still recommended to treat prophylactically a case of latent TB infection with 9 months of isoniazid; however therapy with rifampicin for 4 m

Diagnosis and treatment of latent infection with Mycobacterium tuberculosis (LTBI), significantly reduces the risk of developing active tuberculosis and the transmission of the disease in the community. LTBI screening must pass by the exclusion of active disease (symptoms enquiry and chest radiography) and assessment of immune response to Mycobacterium tuberculosis testing with the tests currently available - tuberculin skin test and interferon -gamma release assay (IGRA). The choice of treatment must take into account the efficacy and side effects associated with the same. This document provi

The leading tuberculosis (TB) vaccines currently in clinical trials are all designed as prophylactic vaccines. Although these vaccines are highly active, they will most probably not result in sterilizing immunity and, therefore, will not solve the global problem of latent TB. An attractive strategy is to target the remaining dormant bacteria with vaccines based upon antigens induced as the bacteria change from active multiplication to non-replicating dormancy (latency antigens) or during reactivation as dormant bacteria resume active metabolism (resuscitation antigens). These late-stage antige

OBJECTIVES: Latent infections are well-known for bacteria such as Mycobacterium tuberculosis and for viruses such as Herpesviridae and parasites like Leishmania spp. or Toxoplasma gondii. However, invasive fungi may also be latent and come to reactivate. The aim of this review is to clarify the reactivation concept in major fungal invasive diseases.
METHOD: We have searched for PUBMED publications from 1980 to 2017 with the keywords "fungi", "reactivation", "latency", "dormancy", "granuloma", "Aspergillus", "Mucorales", "Dimorphic fungi", "Histoplasma", "Cryptococcus", "Pneumocystis", "Yeast"

INTRODUCTION: Latent tuberculosis infection (LTBI) is defined as a state of persistent immune response to stimulation by Mycobacterium tuberculosis antigens without evidence of clinically manifested active tuberculosis (TB). Diagnosis and treatment for LTBI are important for TB, especially in high-risk populations. Tuberculin skin test (TST) and interferon-gamma release assays (IGRAs) are used to diagnose LTBI.
AIM: The study aims to present the first results with IGRA test compared with TST in the screening of LTBI and the treatment results in the cases with LTBI in Macedonia.

Diagnosing and treating latent tuberculosis (TB) infection (LTBI) is recognized by the World Health Organization as an important strategy to accelerate the decline in global TB and achieve TB elimination. Even among low-TB burden countries that have achieved high rates of detection and successful treatment for active TB, a number of barriers have prevented implementing or expanding LTBI treatment programmes. Of those infected with TB, relatively few will develop active disease and the current diagnostic tests have a low predictive value. LTBI treatment using isoniazid (INH) has low completion

Tobacco smoking has a significant impact on the risk of tuberculosis and the mortality of the disease. There is a strong dose-response relationship both in terms of quantity and duration of smoking. Latent tuberculous infection and tuberculosis disease are different stages of the same infectious process but the relation between smoking and the risk of being infected with Mycobacterium tuberculosis is less well understood. The aim of this review is to describe the relationship between tobacco consumption and tuberculous infection and the mechanism by which smoking increases the risk. It outline

One third of the world's population is infected with Mycobacterium tuberculosis , and 5% to 10% of those infected will develop active disease. Identification and treatment of latent tuberculosis infection (LTBI) is an essential part of the public health strategy to control the spread of TB. The prenatal period provides an opportunity for obstetric providers to assist in this effort by offering testing and treatment to women at risk.

The essential component of tuberculosis elimination strategy is to identify and treat persons with latent tuberculosis infection (LTBI) who are at high risk for developing active tuberculosis. The tuberculin skin test is the only proven method for identifying LTBI. Although the specificity and the sensitivity are decreased by cross reaction with BCG vaccination and by non tuberculous mycobacteria, there is no better diagnostic tool. The test's positive predictive value is poor in populations with low risk for tuberculosis. Identification of persons with LTBI is focused on groups at high risk w

The risk of developing active tuberculosis is highest within the first 2 years of infection. Therefore, an intervention that targets persons with recent infection, such as identifying contacts of active cases, could be particularly effective as an epidemic control measure. A mathematical model of a tuberculosis epidemic is formulated and used to evaluate the strategy of targeting therapy to persons with recently acquired latent tuberculosis infection. The model is used to quantify the effectiveness of therapy for early latent tuberculosis infection in reducing the prevalence of active tubercul

BACKGROUND: In Australia, demand for specialist infectious diseases services exceeds capacity to provide timely management of latent tuberculosis infection (LTBI) in areas of high refugee and asylum seeker settlement. A model for treating LTBI patients in primary care has been developed and piloted in a refugee-focused primary health service (Monash Health Refugee Health and Wellbeing [MHRHW]) and a universal primary care clinic. This study reports on the development and evaluation of the model, focusing on the model feasibility, and barriers and enablers to its success.
METHODS: A convergent

The study aim was to investigate the natural history of latent tuberculosis infection (LTBI) and the efficacy of LTBI treatment among participants in the national LTBI screening program. Individuals who underwent an interferon-gamma release assay (IGRA) between 2017 and 2020 were included, and tuberculosis (TB) development was investigated until April 2021. Cox proportional hazards models were used to investigate the risk factors affecting TB incidence among IGRA-negative and IGRA-positive individuals who did not undergo treatment. Additionally, the numbers needed to treat (NNT), stratified by

Tuberculosis continues to be a major public health problem. A priority objective is the implementation of early diagnosis, contact investigation and latent tuberculosis infection (LTBI) testing. World Health Organization (WHO) concludes that there is no gold standard for the diagnosis of LTBI; both the tuberculin test and IGRA (interferon gamma release assays) indirectly identify tuberculosis infection; both tests are considered acceptable but imperfect. WHO recommends that regimens that include rifamycins are equally effective but less toxic and more adherent than long regimens with isoniazid

OBJECTIVE: Approximately 12.4 million people in the U.S. have latent tuberculosis infection (LTBI), 73% of whom are non-U.S. born. Identification and treatment of LTBI are essential for tuberculosis eradication. We evaluated an emergency department (ED) - based LTBI screening and linkage to care program.
METHODS: We queried electronic records of a clinical prevention program located in a Midwestern, urban, academic ED that serves as the region's safety-net hospital. Program staff approached non-U.S. born ED patients from TB endemic areas. Patients received QuantiFERON-TB Gold Plus (QFT) blood

Latent tuberculosis infection (LTBI) is a subclinical mycobacterial infection defined on the basis of cellular immune response to mycobacterial antigens. The tuberculin skin test (TST) and the interferon gamma release assay (IGRA) are currently used to establish the diagnosis of LTB. However, neither TST nor IGRA is useful to discriminate between active and latent tuberculosis. Moreover, these tests cannot be used to predict whether an individual with LTBI will develop active tuberculosis (TB) or whether therapy for LTBI could be effective to decrease the risk of developing active TB. Therefor
