The purpose of this review is to discuss the scientific literature on waterborne healthcare-associated infections (HCAIs) published from 1990 to 2012. The review focuses on aquatic bacteria and describes both outbreaks and single cases in relation to patient characteristics, the settings and contaminated sources. An overview of diagnostic methods and environmental investigations is summarized in order to provide guidance for future case investigations. Lastly, on the basis of the prevention and control measures adopted, information and recommendations are given. A total of 125 reports were inc

Legionella pneumophila, a Gram-negative bacillus, is the causative agent of Legionnaire's disease, a form of severe community-acquired pneumonia. Infection can have high morbidity, with a high proportion of patients requiring ICU admission, and up to 10% mortality, which is exacerbated by the lack of efficacy of typical empirical antibiotic therapy against Legionella spp. The fastidious nature of the entire Legionellaceae family historically required inclusion of activated charcoal in the solid medium to remove growth inhibitors, which inherently interferes with accurate antimicrobial suscepti

Legionella micdadei is responsible for community- or nosocomial-acquired pneumonia as well as the influenza-like illness Pontiac fever. The aim of this study was to investigate the ability of L. micdadei to utilize extracellular choline for phosphatidylcholine (PC) synthesis and its consequences for the phospholipid composition of its membrane system and the interaction with the human LL-37 peptide. Comparative analysis of the PC content using isotopic labeling revealed that in presence of exogenous choline 98% of the total PC was synthesized via the Pcs pathway while the remaining 2% were gen

Members of the family Legionellaceae have emerged as important nosocomial pathogens. Bronchopneumonia is the typical illness and mimics other nosocomial pneumonias. On rare occasion, surgical wound infections may be due to legionellae. Legionella pneumophila is the species causing most human illness, but other species may predominate in individual institutions. Pneumonia is acquired by inhalation of environmental aerosols from water sources contaminated by Legionella sp. In the hospital setting contaminated water sources include potable water, heat-exchange systems, and cooling towers. Water t

Many common aquatic bacteria like Legionella pneumophila are able to colonize man-made water systems. Poorly maintained systems or those that are seldom used provide ideal sites for growth and often also provide the means for aerosolizing the organism. Compounds leached from construction materials and the by-products of other organisms can be used as food by the Legionellaceae and can thus aid their growth. Keeping water systems clean and well serviced, keeping hot water at or above, and cold water below, recommended temperatures and additionally in cooling towers maintaining the required leve

The routine use of monoclonal antibodies against L. pneumophila and other Legionellaceae has not yet reached the clinical microbiology laboratory. Most of these antibodies are still under evaluation and are not available commercially. However, it is obvious that some of them will be available in the next few years. Because of the numerous subtypes and serogroups of L. pneumophila already identified, extreme caution must be used before they are adopted routinely by a given laboratory. Testing against a large number of strains will have to be performed, and even then unidentified subtypes may no

We examined the factors involved in the occurrence of Legionellaceae in a hospital water system and the recontamination by Legionella pneumophila after a thermal disinfection procedure was studied. Three months after the heat treatment (70 degrees C), the regrowth of the two prevalent Legionella strains (L. pneumophila serogroup 1 [Oxford-like] and L. pneumophila serogroup 2) reached the original level of cell numbers. Genomic analysis (pulsed-field gel electrophoresis) revealed the strains to be survivors of the decontamination. Temperature tolerance experiments showed that the serogroup 1 st

Besides other bacterial genera legionellae are a major causative agent of severe pneumonia. Therefore, it is essential to include legionnaires' disease into the diagnostics of pneumonia of unknown origin. The appreciable number of different species and serogroups of legionellaceae, most of them pathogenic for humans, render this task more difficult. The cultural isolation of legionellae from patient or environmental samples is easily performed, the identification of the isolates may be difficult, however. Structural features of the bacteria as determined by biochemical techniques may support t

Ever since the 1976 Philadelphia epidemic and the isolation of the causative organism by MacDade in 1977, numerous clinical, epidemiological and bacteriological papers have stressed the significance of the "new" causative organisms responsible for serious cases of pneumonia. On the basis of knowledge available at present, the Legionellaceae family accounts for these bacteria. There are five different species in the genus Legionella: L. pneumophila, L. micdadei, L. bozemannii, L. dumoffii and L. gormanii. L. pneumophila occurs most frequently and has six serogroups (Serogroups 1-6), the first o

Legionnaires' disease is an acute bacterial pneumonia caused by an organism of the family of Legionellaceae. L. pneumophila serotype 1 is responsible for most infections. The incidence of Legionnaires' disease in Belgium is not known because the diagnosis cannot be made based on clinical or radiological grounds and because there is a microbiological underdiagnosis. Legionellae are widely distributed in nature, as free living organisms, in protozoa, and in biofilms. As a consequence, these bacteria are particularly resistant to disinfection and heating. We recently investigated an outbreak of l

This article reviews the classification and identification of the 23 described species, the 11 unnamed species, and the 49 serogroups presently known in the family Legionellaceae and the genus Legionella. The events leading to the isolation and classification of the first species, Legionella pneumophila, are summarized; these include the outbreak of legionnaires' disease at the 1976 American Legion Convention in Philadelphia, the five outbreaks preceding the Philadelphia outbreak, and the isolation of three Legionella species before 1976. The phenotypic characteristics of legionellae are descr

Chlamydiae are intracellular bacterial parasites of eukaryotes, ranging from amoebae to humans. They comprise many novel members and are investigated as emerging pathogens. Environmental studies highlighted similarities between the ecologies of chlamydiae and legionellae, both groups being important agents of respiratory infections. Herein, we analyzed nasal samples from healthy persons, searching for the presence of amoebae, chlamydiae and legionellae. From a total of 25 samples, we recovered by PCR eight samples positive to chlamydiae and six samples positive to legionellae. Among these samp

Legionellosis is a contagious disease occurring either sporadically or more frequently in outbreaks, the infections being caused by members of the family Legionellaceae. The disease may show two clinical forms, i.e.: 1) the pneumonic form characterized by fever, pulmonary, neurologic and often hepatic and renal affections, severe evolution and high mortality rate; and 2) non-pneumonic form similar to influenza infection. Occurrence of the disease was mentioned almost all over the world its incidence in Europe reaching 1-3%. The infection sources are: warm water supply systems of hospitals, hot

An outbreak of a potentially fatal form of pneumonia in 1976 and in the annual convention of the American Legion was the first time that Legionella spp. was identified. Thereafter, the term Legionnaires' disease (LD) was established. The infection in humans is transmitted by the inhalation of aerosols that contain the microorganisms that belong to the Legionellaceae family and the genus Legionella. The genus Legionella contains genetically heterogeneous species and serogroups. The Legionella pneumophila serogroup 1 (Lp1) is the most often detected strain in outbreaks of LD. The pathogenesis of

During the summer of 2012, a major Legionella pneumophila serogroup 1 outbreak occurred in Quebec City, Canada, which caused 182 declared cases of Legionnaire's disease and included 13 fatalities. Legionella pneumophila serogroup 1 isolates from 23 patients as well as from 32 cooling towers located in the vicinity of the outbreak were recovered for analysis. In addition, 6 isolates from the 1996 Quebec City outbreak and 4 isolates from patients unrelated to both outbreaks were added to allow comparison. We characterized the isolates using pulsed-field gel electrophoresis, sequence-based typing

A novel Legionella species was identified based on analysis of 16S rRNA and mip (macrophage infectivity potentiator) gene sequences, cellular fatty acids, isoprenoid quinones, biochemical reactions, antigens and quantitative DNA-DNA hybridization. Strain CDC-1796-JAP-E(T) was isolated from well water at the Nagasaki Municipal Medical Center, Japan. Two strains, CDC-3041-AUS-E and CDC-3558-AUS-E, were isolated from water samples during an outbreak of legionellosis in South Australia. Strain CDC-5427-OH-H was isolated from a 66-year-old female patient diagnosed with Legionnaires' disease in the

BACKGROUND: There has never been a cross-Canada surveillance project to determine the rate of Legionella species as a cause of community-acquired pneumonia requiring hospitalization and to determine whether there are any regional differences in the rates of Legionnaires' disease in Canada. Anecdotally, Legionnaires' disease is thought to be uncommon in Western Canada.
METHODS: From January, 1996 through to October 31, 1997, a prospective study of the etiology of community acquired pneumonia requiring admission to 15 tertiary care hospitals in eight Canadian provinces was conducted. A urine sa

The study of outbreaks of Legionella pneumophila has been essential in understanding the organism, the disease, and its pathogenesis. Early epidemics defined the clinical spectrum: Pontiac fever is an acute, self-limited, febrile illness with an attack rate of 95% to 100% and an incubation period of 36 hours. In contrast, legionnaires' disease is a life-threatening bronchopneumonia with an attack rate of 2% to 7% and an incubation period of two to ten days. Three times as many males as females are affected with legionnaires' disease, and age, cigarette smoking, and chronic medical disease (par

BACKGROUND: While Legionella is a common cause of pneumonia, extrapulmonary infections like arthritis are scarce. Here, we describe a case of monoarthritis due to Legionella bozemanii, with no history of pneumonia. We provide a literature review of the 9 previously published Legionella arthritis and highlight a dichotomous epidemiology suggesting different physiopathological pathways leading to joint infection.
CASE PRESENTATION: A 56-year old woman under immunosuppressive treatment by oral and intra-articular corticosteroids, methotrexate, and tocilizumab for an anti-synthetase syndrome was
