Bilateral lens subluxation in a patient with homocystinuria: a case report.

PURPOSE: To report a case of bilateral lens subluxation associated with homocystinuria that had been previously misdiagnosed with high myopia, amblyopia, and keratoconus. OBSERVATIONS: A 19-year-old Asian female presented with progressive bilateral visual decline and hand tremors over 10 years. Slit-lamp examination, anterior segment optical coherence tomography, genetic and metabolic testing were performed to confirm the diagnosis of homocystinuria. Surgical intervention was undertaken to improve visual acuity. Anterior segment imaging documented bilateral lens subluxation and microspheropha

Bilateral Subluxation of Microspherophakic Lens in a Child with Cohen Syndrome.

Cohen Syndrome is a rare autosomal recessive condition characterized by facial abnormalities with or without microcephaly, non-progressive intellectual delay, hypotonia, ophthalmic abnormalities, and neutropenia. Due to its low incidence and variable presentation, much about the disorder, including ophthalmic manifestations, is not fully understood. Here, we present the first documented case of a 5-year-old Amish child with Cohen Syndrome who presented with bilateral subluxation of microspherophakic lenses - rare findings themselves, let alone coexisting in a patient with a rare genetic syndro

[Lens subluxation and glaucoma: indications and hazards of cataract surgery].

BACKGROUND: Iridodonesis is the clinical sign of lens subluxation. Acute glaucoma caused by a ciliolenticular block may develop. YAG iridotomy or peripheral iridectomy are rarely effective in controlling intraocular pressure. The results of phacoemulsification and posterior lens implantation for subluxated lens and glaucoma are reported. PATIENTS AND METHODS: Seven patients aged 69+/-9.5 years were followed up. All 10 eyes with subluxated lens and glaucoma had phacoemulsification and posterior chamber lens implantation. Two different groups relating to axial length could be analyzed. Intraope

[Laser fixation of the crystalline lens in its contused subluxation].

The authors report a nontraditional effective method of treating patients with contusion semiluxation of the lens. It consists in fixation of the lens at the expense of formation of posterior synechiae because of an inflammatory process induced by laser exposure of pigmented epithelium of the iris and ciliary body. Complex clinical evaluation of the organ of vision in 132 patients, including the remote period (after 3-10 years), showed good stabilization of visual functions and of the geometrical position of the incompletely dislocated lens after its laser fixation.

Pigment dispersion syndrome associated with spontaneous subluxation of crystalline lens.

Pigment dispersion syndrome (PDS) is an ocular condition characterized by a dispersion of iris pigment throughout the eye. This pigment is deposited in a characteristic manner on the corneal endothelium as Krukenberg's spindle, anterior surface of the iris, in the trabecular meshwork, on the lens and zonule and occasionally on the anterior hyaloid face. Even with deposition of pigment on zonular fibers, no zonular weakness, or zonular dehiscence has been reported in these cases. We report a unique case of PDS with bilateral spontaneous subluxation of crystalline lens. With characteristic findi

Intracapsular lens extraction for the treatment of pupillary block glaucoma associated with anterior subluxation of the crystalline lens.

PURPOSE: To report a case of pupillary block glaucoma associated with spontaneous crystalline lens subluxation into the anterior chamber in a 34-year-old man. METHODS: Dry vitrectomy was performed for securing enough retrolental space, and an intracapsular lens extraction was then performed via a corneolimbal incision. Additional endothelial cell damage was avoided with an injection of viscoelastics and gentle extraction of the crystalline lens. After deepening of the anterior chamber, scleral fixation of the intraocular lens was performed with an ab externo technique. RESULTS: Two months af

Transscleral fixation of foldable intraocular lens after pars plana lensectomy in eyes with a subluxated lens.

Two patients had a subluxated crystalline lens: one with Marfan's syndrome and the other with traumatic cataract. A pars plana lensectomy with transscleral fixation of a foldable silicone intraocular lens rather than a conventional poly(methyl methacrylate) lens was performed. Excellent, rapid visual rehabilitation was obtained using this small incision technique.

A case of iridofundal coloboma with persistent fetal vasculature and lens subluxation.

Persistent fetal vasculature and fundal coloboma are important congenital vitreoretinal disorders that can severely affect a child's visual acuity. Each disorder has its own set of potential complications. We discuss the case of a visually challenged child who presented with a combination of both these disorders, along with inferior lens subluxation.
