T-cell stimulation with purified mycobacterial antigens in patients and healthy subjects infected with Mycobacterium leprae: secreted antigen 85 is another immunodominant antigen.

Peripheral blood leucocytes from 9 paucibacillary and 12 multibacillary leprosy patients, from 18 healthy controls and from 34 healthy leprosy contacts were stimulated with three mycobacterial heat shock proteins with respective molecular weights of 70, 65 and 18 kDa and with the secreted 30-32 kDa protein, also called antigen 85. Antigen 85 was found to be the most powerful T-cell antigen (as measured by lymphoproliferation and IFN-gamma secretion), eliciting a positive response in all (100%) paucibacillary patients and in all lepromin-positive controls and contacts. The three heat shock prot

Response of Mycobacterium habana vaccine in patients with lepromatous leprosy and their household contacts. A pilot clinical study.

Single dose vaccination was carried out with Mycobacterium habana vaccine, 31 lepromatous leprosy cases receiving 1.5 mg (1.5 mg = 6.27 x 10(8) bacilli) and 36 household contacts randomly receiving 1.5, 2.0, 2.5 mg vaccine intradermally. Duration of study was 18 weeks. Vaccination induced lepromin conversion in 100% of lepromatous leprosy cases and lepromin negative household contacts and augmentation of lepromin reactivity in 100% of lepromin positive household contacts, which was stable for the 15 weeks duration of follow-up. The maximum augmentation in lepromin reactivity was obtained with

Immuno-stimulatory effect of levamisole in borderline leprosy cases.

Eleven cases of borderline-borderline leprosy were subjected to levamisole therapy for 3 months. Levamisole was given 150 mg/day on three consecutive days every fortnight along with dapsone (DDS). Immuno-stimulation was assessed by lepromin test using Lepromin-A supplied by W.H.O. containing 4.0 X 10(7) bacilli per milli litre. It was found that there was statistically significant change in lepromin reaction after levamisole therapy.

Cell-mediated immunologic status of healthy members of families with a history of leprosy.

The cell-mediated immune status of 20 apparently healthy children from families with a history of leprosy has been studied. They have been compared with 20 age- and sex-matched controls from families with no history of leprosy. Lymphocyte transformation tests using PHA, PPD and lepromin and skin tests to lepromin, PPD and candida were carried out. No evidence of a depression of cell-mediated immunity in the children from families with leprosy was obtained. The only two children giving a negative Mitsuda lepromin skin test both subsequently developed leprosy in the succeeding 16 months. One was

[Epidemiological immunological studies on leprosy in Okinawa. 6. Lepromin, lymphocyte transformation, FLA-ABS and leproagglutination tests in the in-and out-patients with leprosy and the relationships among these tests and personal or family history or clinical findings of the patients].

In- and out-patients with leprosy in Okinawa were surveyed by using the following immunological tests: cutaneous reaction to Mitsuda's lepromin (40 million bacilli/ml) (LPR), lymphocyte transformation test with Dharmendara's antigen (DL-LTT), that with phytohemagglutinin (PHA-LTT), fluorescent leprosy antibody absorption test (FLA-ABS) and Leproagglutination test with cardiolipin-lecithin antigen (LAT). A correlation between two tests and a relationship between the test and personal or family history or clinical finding of the patients were evaluated by non-parametric statistics. Spearman's ra

Post-vaccination sensitization with ICRC vaccine.

ICRC vaccine is one of the candidate anti-leprosy vaccines under test in a large scale comparative vaccine in trial. The objectives of the present study was to study the sensitization potential, as measured by Rees' MLSA and lepromin, and reactogenicity of this vaccine preparation in the local population. The study included 368 'Работает' individuals aged 1-70 years. Each individual received either ICRC vaccine or normal saline (control) by random allocation. They were also tested with Rees' MLSA and lepromin-A, 12 weeks after vaccination. Reactions to Rees' MLSA were measured after 48 hours an

Does isopathic phenomenon occur in leprosy?

Twenty-five patients of leprosy classified according to the criteria of Ridley and Jopling (1966) were studied to verify the existence of isopathic phenomenon, using lepromin and autoclaved BCG as antigens. BCG as well as lepromin injection sites showed tuberculoid granulomas in all the cases of tuberculoid, borderline tuberculoid, mid-borderline and indeterminate leprosy. In lepromatous and borderline lepromatous cases BCG injection sites showed infiltration of foam cells containing AFB and foci of epithelioid cells, while lepromin injection sites showed infiltration of foam cells containing

Evidence that the Mitsuda reaction to Mycobacterium leprae can be mediated by lymphocytes responsive to Mycobacterium tuberculosis.

A positive Mitsuda skin test for delayed type hypersensitivity to Mycobacterium leprae is associated with a high level of protection against lepromatous leprosy, while the value of tuberculin sensitivity in leprosy is less pronounced. Cutaneous lymphocytes, isolated from the Mitsuda reaction of a PPD-positive individual not previously exposed to M. leprae, were cloned with Dharmendra lepromin and analysed for antigen specificity. Thirteen lepromin-responsive cell lines were derived, with greater than 95% certainty that the number of true clones was at least five and the number of functionally

Comparative Study of Leprom114 Reaction in Indeterminate Leprosy and Controls.

Lepromin test (LT) was done on 100 cases of indeterminate leprosy (IL) and in 100, age and sex matched controls. LT was negative in 98% of IL, but positive in 80% of the controls. Lepromin positive cases showed a tuberculoid picture histopathologically, though the clinical picture was that of IL. There was no co-relation between Fernandez reaction and Mitsuda reaction.

Immunotherapy of far-advanced lepromatous leprosy patients with low-dose convit vaccine along with multidrug therapy (Calcutta trial).

This report describes a promising mode of treatment of lepromin-unresponsive, far-advanced, lepromatous (LL) leprosy patients with antileprosy vaccines as an adjunct to multidrug therapy (MDT). The Trial Groups included 50 highly bacilliferous, lepromin-negative, untreated LL patients. They were given MDT for 2 years. Of them, 30 patients were administered a mixed antileprosy vaccine containing killed Mycobacterium leprae of human origin plus M. bovis BCG. The remaining 20 patients were given M. bovis BCG. Depending on the severity of lepromin unresponsiveness, they were given one to six inocu

Reversal reaction in lepromatous patients induced by a vaccine containing killed ICRC bacilli--a report of five cases.

Clinico-pathological features of five cases of lepromatous leprosy exhibiting "reversal" reaction with upgrading of lesions following vaccination with ICRC vaccine have been described. Two patients also developed ENL. Associated with the "reversal" reaction, the patients exhibited lepromin conversion. No evidence of fresh nerve lesions was observed in any patient. Besides lepromin conversion, occurrence of "reversal" reaction is yet another evidence of immunogenicity of the ICRC vaccine.

Blastogenic response with autologous plasma and Mitsuda's reaction in leprosy patients and their unaffected sibs.

Many investigators have evaluated the influence of genetic constitution on the susceptibility to leprosy in studies linked to different types of research. To determine the possible existence of a family trait linked to the lymphoproliferation and to lepromin reactivity we studied the blastogenic response to phytohaemagglutinin, lepromin and M. leprae and the Mitsuda's reaction in leprosy patients and their unaffected sibs. Sixty-eight individuals were studied, 34 were leprosy patients (17 lepromatous and 17 tuberculoid leprosy) and the remaining were their sibs previously matched by sex and ag

Immunogenic "subunit" of the ICRC antileprosy vaccine.

The administration of a vaccine containing ICRC bacilli, which is currently undergoing clinical trials in India, induces persistent lepromin conversion in lepromatous leprosy (LL) patients and lepromin-negative healthy subjects, with "upgrading" of tissue response in the former. A sonicate of ICRC bacilli, when subjected to gel-filtration chromatography using high-pressure liquid chromatography (HPLC), yields a high molecular weight glycolipoprotein (PP-I) with an apparent molecular weight of 10(6) daltons. PP-I, which brings about lepromin conversion in lepromin-negative healthy subjects, is

Reversal reaction and Mitsuda conversion in polar lepromatous leprosy: a case report.

A 22-year-old male polar lepromatous leprosy patient who became Mitsuda positive after 36 months of multidrug therapy (MDT) is reported. Lepromatous leprosy (LL) is a state of specific immunosuppression and is invariably irreversible. The finding of Mitsuda positivity in histopathologically proven polar lepromatous leprosy is extremely uncommon, and conversion of lepromin status following MDT has not so far been reported. This case reports confirms the observations made by Waters et al. regarding lepromin conversion in lepromatous patients.

T lymphocyte reactivity of leprosy patients and healthy contacts from a leprosy-endemic population to delipidified cell components of Mycobacterium leprae.

In this study, we measured in vitro proliferative responses of peripheral blood mononuclear cells from both leprosy patients across the clinical spectrum and also healthy contacts from a leprosy-endemic population to delipidified cell components of Mycobacterium leprae (DCC) and Dharmendra lepromin. Dharmendra lepromin was poor in inducing in vitro T cell proliferation in all the study groups, even though it elicited marked in vivo skin test reaction in tuberculoid leprosy patients and healthy contacts. In contrast, Dharmendra preparation of BCG induced marked T-cell response in tuberculoid as

A quick method of demonstrating bacillaemia in patients with lepromatous leprosy and ultrastructural studies of the circulating acid-fast bacilli.

While studying circulating immune complexes (CIC) in the sera of lepromatous patients by the polyethylene glycol (PEG) precipitation technique, we found (by light microscopy) abundant acid-fast bacilli (AFB), morphologically similar to those seen in slit skin smear preparations from these patients, precipitated with the PEG precipitated materials. Both solid and non-solid AFB could be readily identified. Ultrastructures of these AFB in the PEG aggregates showed some similarity with those detected in the PEG precipitates prepared from armadillo-derived lepromin under identical conditions. The m

Delayed hypersensitivity tests with Mycobacterium leprae purified protein derivative.

Skin tests were conducted on 3 lepromatous, 3 dimorphous and 6 tuberculoid leprosy patients and 3 others not suffering from leprosy with lepromin, purified protein derivative from M. leprae of armadillo origin and tuberculin. Results show that a delayed hypersensitivity reaction could be produced with PPD in 72 hours on all Mitsuda positive cases, with one anomalous exception, without cross reaction to tuberculin. The results were promising from the point of view of substituting lepromin with PPD in usual tests.

Preliminary trial of intervention levamisole therapy in persistently bacteriologically positive lepromatous leprosy.

The intervention treatment with the immuno-adjuvant levamisole was given in 14 persistently bacteriologically positive lepromatous cases who had had adequate chemotherapy for over 5 years. There was a temporary conversion of the early lepromin reaction in a majority of the patients with a corresponding improvement in clinical and bacteriological status. However, the lepromin response was short lived. Most patients had become bacteriologically negative at the end of 1 year. No adverse effects due to levamisole therapy were encountered.

Oral zinc as an adjunct to dapsone in lepromatous leprosy.

Oral zinc was tried in 15 cases of multibacillary leprosy as an immunostimulant in addition to conventional antileprosy drugs. Results were compared with those in ten similar cases treated with dapsone alone. Cases treated with zinc showed faster clinical improvement, regrowth of eyebrows, rapid fall in the bacterial index (BI) and in the bacterial index in the granuloma (BIG), early and greater influx in lymphocytes in the granuloma, and neovascularization. Upgrading occurred in 6 out of 15 patients taking zinc, but in only 1 out of 10 patients in the control group. Five out of six patients w
