Clinical infectious diseases : an official publication of the Infectious Diseases Society of America2024eng
BACKGROUND: Since 2019, the World Health Organization has recommended dolutegravir-based antiretroviral therapy (ART) as the preferred regimen for human immunodeficiency virus management. Large-scale programmatic transitioning to dolutegravir-based ART was subsequently implemented across Africa, often in the absence of recent viral load testing and without access to genotypic resistance testing (GRT) in case of viremia.
METHODS: This study assessed emerging dolutegravir resistance in the routine care of the Viral Load Cohort North-East Lesotho. We included pediatric and adult participants who

BACKGROUND: Discontinuation of ARV is a worldwide problem, with a discontinuation rate of 20% globally and 40-50% in Sub-Saharan Africa. In Lesotho, it ranges from 20 to 30% in men. To effectively tackle the HIV/AIDS epidemic in Lesotho, it is necessary to address the several complex problems that hinder adherence to ARVs among men.
METHODS: This study used a qualitative descriptive case study approach to collect data from men aged 18 years and above receiving health services at the Seventh-day Adventist clinic, using a homogeneous purposive sampling method. In-depth interviews were used to c

Child-headed households (CHHs) have become a salient feature of family life in parts of Southern Africa where HIV/AIDS, poverty, labour migration, and, more recently, COVID-19 have interacted to produce a syndemic that has eroded adult caregiving capacity. These intersecting structural pressures have contributed to sustained orphanhood, household instability, and the reorganisation of caregiving responsibilities within families. This article examines how children in CHHs in rural Lesotho navigate the intersecting effects of HIV/AIDS and COVID-19, negotiate caregiving responsibilities, and cons

BACKGROUND: The rubella virus is a major contributor to birth defects globally and is preventable by vaccination. In 2020, the world was supposed to be free of both rubella and Congenital Rubella Syndrome (CRS) however this goal has yet to be realized with only 93 out of 194 WHO member states confirmed rubella-free in 2020.
METHODS: A retrospective measles and rubella case-based surveillance data record review was conducted from 2018 to 2022 to document rubella epidemiology after the introduction of rubella vaccination in Lesotho and progress toward elimination. All samples submitted for surv

BACKGROUND: Although anthrax occurs globally, the burden of the disease remains particularly high in Africa. Furthermore, the disease anthrax has significant public health and economic implications. However, sufficient attention has not been given to the geographic distribution of anthrax outbreaks and cases in Lesotho. Therefore, this study investigates the spatial patterns of anthrax outbreaks and cases among livestock in Lesotho from 2005 to 2016.
METHODS: A cross-sectional study design was adopted to realise the objectives of this study using retrospective data of anthrax outbreaks and ca

In Lesotho's central hospital 55 (25%) of 218 admissions for severe PEM died during 1981 and 1982. Most deaths (62%) occurred in the first week. The most important causes of death were acute GE and pneumonia in marasmus and kwashiorkor, respectively. The cause of death remained obscure in 16 children, however. In marasmus a poor prognosis was significantly associated with the finding on admission of a temperature less than 36.5 degrees C (P less than 0.05), apathy (P less than 0.01) and a depigmented skin (P less than 0.05), while in marasmic kwashiorkor only the finding of the latter was sign

BACKGROUND: Lesotho has one of the highest rates of tuberculosis (TB) incidence and TB-HIV co-infection in the world. Our study aimed to assess the knowledge, attitude and associated factors towards TB in the general population of Lesotho.
METHODS: A cross-sectional analysis from the Lesotho Demographic and Health Survey (LDHS) 2014 was carried out among 9247 respondents. We used the chi-square test as well as univariate and multivariate logistic regression analyses to assess the associations of socio-demographic variables with respondent knowledge of and attitude towards TB.
RESULTS: The ov

BACKGROUND: Upper respiratory tract infections cause morbidity and a high burden on healthcare systems worldwide, especially in low- and lower middle-income countries. Recent studies throughout Africa indicate seasonal patterns that deviate from those previously described in settings with temperate climates. Currently, there are no data available on pathogens causing upper respiratory infections and their seasonal patterns in Lesotho, Southern Africa.
METHODS: This cross-sectional nested study enrolled a randomly selected sample of adults presenting at St. Charles Mission Hospital, Seboche, i

BACKGROUND: Community Health Workers locally known as Village Health Workers (VHW) in Lesotho are key members of the primary health workforce, and has been playing a significant role in building primary health systems in many countries and they can fill significant gaps in human resources as low and middle income countries work towards universal health coverage in the era of Sustainable Development Goals (SDGs). The 2014 Lesotho health reform restructured the VHW program to compensate, professionalize, and integrate VHWs into primary care services. We sought to document the ways in which the V

The international journal of tuberculosis and lung disease : the official journal of the International Union against Tuberculosis and Lung Disease2007eng
Tuberculosis (TB) remains the leading infectious killer of adults with human immunodeficiency virus (HIV) globally. Lesotho has the third highest prevalence of HIV and the fourth highest prevalence of TB worldwide, and the majority of TB patients are co-infected with HIV. This paper describes an antiretroviral treatment (ART) program instituted in a health center in a mountain region of Lesotho. Although the main goal of the program was to increase HIV detection and initiate ART for patients, the program also resulted in a ten-fold increase in the detection of TB among patients with and withou

An evaluation of the impact of a nationwide clinic-based growth monitoring (GM) programme was done in Lesotho to determine if clinic attendance was associated with improved maternal knowledge of weaning practices and diarrhoea. A total of 907 mothers from eight clinics were included in the study. Our results showed that mothers who had attended the clinics knew more about the appropriate timing for introducing animal protein-rich foods in the child's diet and about the use of oral rehydration salts for diarrhoea, than those who had not. The difference in knowledge between previous clinic atten

INTRODUCTION: Limited research has examined dementia prevalence and associated factors in Lesotho. This study investigates dementia prevalence and the associated factors in Lesotho.
METHODS: A survey in Lesotho included 1738 participants screened for dementia and potential associated factors with a focus on modifiable factors. Associations were evaluated using logistic regression models.
RESULTS: The median age was 66 years, with 54.83% women. The prevalence of dementia was 4.89%. Those with depressive symptoms (adjusted odds ratio [aOR]: 3.97, 95% confidence interval [CI]: 1.39-11.30), age

The effects of water quantity and sanitation, alone and in combination with each other, on infant weight gain and length gain were examined. Data on 119 infants were collected from 20 villages in rural Lesotho between July 1984 and January 1985. The interactions between sanitation and increased water usage for weight gain (p = 0.007) and length gain (p = 0.006) were significant after potential confounding was controlled. The biggest growth effects were dependent on families possessing a latrine and increasing their use of water during the warm, wet season. Infants gained 1.031 kg (95% confiden

Information was obtained on infants and children admitted for severe protein energy malnutrition (PEM) to Lesotho's central hospital in 1981 and 1982 as regards feeding pattern, socio-economic, environmental factors and vaccination status. Acute gastroenteritis (GE) appeared to be a major precipitating cause in the development of severe PEM. Of children 13-24 months of age only 13% was still breast fed. Compared to kwashiorkor significantly more children with marasmus or marasmic kwashiorkor had never been breast fed, or were early weaned and bottle fed. Above age nine months, children had a s

Perinatal mortality is a major public health challenge in many developing countries and has profound social, economic, and health implications for families and broader society. Despite its serious and devastating effects, there are a few studies that describe the spatial distribution and factors associated with perinatal mortality in Lesotho. Understanding the burden and geographic variation is crucial for implementing targeted interventions, allocating resources efficiently, and achieving the Sustainable Development Goals. Therefore, this study explores the spatial variations and predictors o

BACKGROUND: Since 2020, the world has been battling the coronavirus disease 2019 (COVID-19) pandemic. The mortality and morbidity at the height of the pandemic sparked generalised fear and uncertainty about the future. Concerns were raised about the psychological impact of the pandemic on workers in healthcare systems globally. This study was conducted to establish the degree of psychological impact of the pandemic on frontline health workers in Lesotho.
METHODS: The study used a quantitative cross-sectional survey design. The Kessler psychological distress screening tool (K-10) and the post-

Despite extensive global efforts, sub-Saharan Africa remains disproportionately affected by the HIV epidemic. This generalized epidemic can be seen in Lesotho which in 2014 the HIV prevalence rate of those aged 15-49 years was 24.6%, with and incidence of 1.9 new infections per 100-person-year exposures. To better understand the impact of Lesotho's national HIV response and significant predictors associated with HIV infection, the Lesotho Population-based HIV Impact Assessment was conducted. This survey provided a nationally representative sample of individuals aged 15-59 years old in which pa

OBJECTIVE: To examine changes in vaccination of children younger than 1 year during the coronavirus disease 2019 (COVID-19) pandemic (March 2020-August 2021) in Haiti, Lesotho, Liberia and Malawi.
METHODS: We used data from health management information systems on vaccination of children aged 12 months or younger in districts supported by Partners In Health. We used data from January 2016 to February 2020 and a linear model with negative binomial distribution to estimate the expected immunization counts for March 2020-August 2021 with 95% prediction intervals, assuming no pandemic. We compare
