Combination of dasatinib and venetoclax in newly diagnosed chronic phase chronic myeloid leukemia.

BACKGROUND: The dual inhibition of the BCR::ABL1 tyrosine kinase and BCL-2 could potentially deepen the response rates of chronic myeloid leukemia in chronic phase (CML-CP). This study evaluated the safety and efficacy of the combination of dasatinib and venetoclax. METHODS: In this phase 2 trial, patients with CML-CP or accelerated phase (clonal evolution) received dasatinib 50 mg/day for three courses; venetoclax was added in course 4 for 3 years. The initial venetoclax dose was 200 mg/day continuously but reduced later to 200 mg/day for 14 days, and to 100 mg/day for 7 days per course once

Asymptomatic chronic-phase chronic myeloid leukemia BCR-ABL. (+) without splenomegaly: A case report.

Chronic Myeloid Leukemia (CML) is one of the myeloproliferative disorders characterized by Philadelphia chromosome presence. Contrary to Western countries, most CML patients in Indonesia present symptoms with splenomegaly as the most common finding. It is rare to find asymptomatic CML patients. We present a case of asymptomatic chronic-phase chronic myeloid leukemia BCR-ABL (+) without splenomegaly who achieved a hematologic and molecular response to imatinib.

[Aseptic meningitis as paraneoplastic syndrome related to chronic myeloid leukemia in chronic phase].

Chronic myeloid leukemia (CML) is a clonal hemopoietic stem cell disorder characterized by reciprocal translocation between the long arms of chromosomes 9 and 22 that produces the fusion BCR-ABL1 gene. Major manifestations in CML patients are increased white cell count and splenomegaly. In this case, the patient presented with aseptic meningitis and showed symptoms, such as disorientation, double vision, and neurogenic bladder disorder. Pulse steroid and antibiotic treatment was ineffective for these symptoms; however, the combination therapy with these drugs and dasatinib was very effective.

Impact of additional cytogenetic aberrations at diagnosis of chronic phase chronic myeloid leukemia.

OBJECTIVE: This study aimed to investigate the correlation between additional cytogenetic abnormalities (ACAs) at diagnosis and their clinical consequences in 337 Chinese chronic myeloid leukemia (CML) patients. DESIGN: Retrospective observational cohort study. METHODS: Response criteria were applied according to the European LeukemiaNet. Event-free survival (EFS) and progression-free survival (PFS) were analyzed. Independent predictors of PFS were assessed using Cox regression analysis. RESULTS: At diagnosis, ACAs were identified in 41 patients (12.2%), with 24 exhibiting high-risk ACAs. P

Imatinib dose reduction after major molecular response in chronic-phase chronic myeloid leukemia.

BACKGROUND: In people with chronic-phase chronic myeloid leukemia (CML) receiving imatinib and achieving major molecular response (MMR), dose reduction may decrease adverse events but may be associated with a loss of molecular response. Whether digital droplet polymerase chain reaction (ddPCR) can identify persons in whom dose reduction might be unsuccessful is unknown. METHODS: Data from 716 consecutive subjects who achieved MMR after initial imatinib therapy (400 mg/day) were obtained. A total of 486 subjects remained on full-dose imatinib, whereas 230 subjects had their dose reduced to 300

Atypical presentation of patients with chronic myeloid leukemia in chronic phase-Case report.

The presence of the translocation t(9;22)(q34;q11), leading to the BCR::ABL1 fusion transcript, is the hallmark of chronic myeloid leukemia (CML). Nevertheless, atypical presentation at diagnosis can be challenging. However, although most patients with CML are diagnosed with the e13a2 or e14a2 BCR::ABL1 fusion transcripts, about 5% of them carry rare BCR::ABL1 fusion transcripts, such as e19a2, e8a2, e13a3, e14a3, e1a3, and e6a2. In particular, the e6a2 fusion transcript has been associated with clinically aggressive disease frequently presenting in accelerated or blast crisis phases. To date,

[Chronic-phase chronic myeloid leukemia with intracranial hemorrhage complicated with tumor lysis syndrome].

A 14-year-old male with autism was admitted to our hospital owing to altered consciousness and gait disturbance. Blood tests showed a white blood cell (WBC) count of 728,600/µl, and brain computed tomography revealed intracranial hemorrhage and a midline shift of the brain. The chronic phase of chronic myeloid leukemia (CML) was confirmed as per bone marrow aspiration findings. The patient underwent emergency craniotomy for hematoma removal, and he subsequently received hydroxyurea and rasburicase combination therapy. However, he developed tumor lysis syndrome (TLS) and died on the second day

Olverembatinib in accelerated-phase chronic myeloid leukemia: efficacy and safety evaluation.

We studied 130 consecutive subjects who presented with (N=29) or transformed to (N=101) accelerated-phase chronic myeloid leukemia (CML) and who received olverembatinib. Sixty-two were in second chronic phase. All failed ≥1 tyrosine kinase inhibitor (TKI) and 91 had BCR::ABL1T315I. Median follow-up was 28 months (interquartile range, 10-74 months). The 6-year cumulative incidences of major cytogenetic response (MCyR), complete cytogenetic response, major molecular response and molecular response 4.0 were 59% (95% confidence interval [CI]: 49- 69), 53% (95% CI: 42-62), 52% (95% CI: 41-62) and 4

Myelomastocytic transformation in chronic myeloid leukemia blast phase: A case report.

Myelomastocytic leukemia (MML) presenting as a blast phase manifestation of Chronic Myeloid Leukemia (CML) is exceptionally rare, with limited documented cases in the literature. Understanding its distinct clinicopathologic features and treatment outcomes is crucial for optimal patient management. A 45-year-old male with a history of CML since 2016, previously treated with imatinib and dasatinib, presented after treatment interruption with leukocytosis (WBC 27.3 × 103/μL) and 58% circulating blasts showing metachromatic granulation. Bone marrow examination revealed 30% blast cells with strong

Myeloid Sarcoma as a Presentation of Acute Myeloid Leukemia and Blastic Phase in the Course of Chronic Myeloid Leukemia: A Case Report and Literature Review.

BACKGROUND: Myeloid sarcoma (MS), also known as granulocytic sarcoma or chloroma, is a rare disease entity characterized by the emergence of an extramedullary tumor, which may be antecedent, coexisting, or manifest secondarily to an ongoing malignancy of lymphoid origin. Owing to its low prevalence, scientific reports addressing this matter comprise mainly retrospective studies with a limited number of participants, rather low-quality research, and only few case reports. Despite MS's rarity, the need for enhancing their diagnostic tools and refinement of their therapeutic regimens is broadly r

Pharmacokinetics and pharmacodynamics of dasatinib in the chronic phase of newly diagnosed chronic myeloid leukemia.

PURPOSE: Dasatinib is a novel, oral, multi-targeted kinase inhibitor of breakpoint cluster region-abelson (BCR-ABL) and Src family kinases. The study investigated pharmacokinetic (PK) and pharmacodynamic (PD) analyses of dasatinib in 51 newly diagnosed, chronic phase, chronic myeloid leukemia patients. METHODS: The dasatinib concentration required to inhibit 50 % of the CrkL (CT10 regulator of kinase like) phosphorylation in bone marrow CD34+ cells (half maximal (50 %) inhibitory concentration (IC50)CD34+cells) was calculated from each patient's dose-response curve using flow cytometry. A case report of a 33-year chronic phase survivor of chronic myeloid leukemia.

Here, we report a Philadelphia chromosome-positive chronic myeloid leukemia case with the longest chronic phase and overall survival to our knowledge ever reported in the medical literature. During the 33-year chronic phase, he was asymptomatic without any treatment and had normal blood cell values. BCR-ABL silencing might be referred to the uncommon long-term survivor.

Sudden Intracranial Hemorrhage in a Patient With Atypical Chronic Myeloid Leukemia in Chronic Phase.

A 16-year-old boy was incidentally found to have hyperleukocytosis during a school physical examination. He was diagnosed with atypical chronic myeloid leukemia in chronic phase. Although treatment with hydoxyurea was started, his white blood cell count increased and he eventually developed lethal intracranial hemorrhage. Although very rare, intracranial hemorrhage should be considered as a possible complication in patients with atypical chronic myeloid leukemia, even in chronic phase, if they have hyperleukocytosis and thrombocytopenia.

Blast phase of chronic myeloid leukemia presenting lymphoid phenotype with a chronic phase of extremely short duration.

Chronic myeloid leukemia (CML) is generally diagnosed in the chronic phase. We have recently encountered two CML-blastic phase (BP) cases, a 71-year-old woman and a 74-year-old man, who resembled de novo acute leukemia. The complete blood count was normal at least 11 and 13 months before the presentation, respectively. The leukemic cells showed predominant lymphoid phenotype. The blasts and granulocytes were positive for BCR-ABL, indicative of CML-BP. Both patients were successfully treated with prednisone and vincristine, followed by Imatinib. Our cases indicate rare presentations of CML-BP w
