Risk factors for isolated periventricular leukomalacia.

Periventricular leukomalacia, a major cause of neurologic disabilities in preterm infants, can be isolated or associated with intraventricular and periventricular hemorrhage. To determine the risk factors for isolated periventricular leukomalacia, we retrospectively studied the characteristics of all very low birth weight infants affected by isolated periventricular leukomalacia who were delivered over a 5-year period and compared them with a control group of very low birth weight infants, matched within 2 weeks for gestational age, with no central nervous system pathology, and born during the

Cerebral visual impairment in periventricular leukomalacia.

Infants with cerebral palsy (CP) frequently present cerebral visual impairment (CVI) often caused by damage to retrochiasmatic pathways. This is particularly true of subjects with damage to the periventricular white matter. Thirty-eight preterm infants with periventricular leukomalacia (PVL) diagnosed by MRI were examined to correlate binocular visual acuity with neuroradiological findings. Binocular visual acuity was evaluated using Teller Acuity Cards and a complete ophthalmological examination was also performed. Three infants with ROP III were excluded from the sample. The age of observati

[Severe periventricular leukomalacia: characteristic electroencephalographic features].

Periventricular leukomalacia is an infrequent but severe lesion in the premature neonate. The presence of positive rolandic sharp waves (PRSW) on the electroencephalogram, regardless of their morphology, is a reliable marker of periventricular ischemia, justifying systematic EEG monitoring during the first three weeks of life for infants born at less than 34 weeks of amenorrhea. A retrospective study of 32 premature neonates with extensive cavitary periventricular leukomalacia confirmed the early occurrence of positive rolandic sharp waves in 84% of premature newborns. The mean rate of PRSW wa

Periventricular leukomalacia, glial development and myelination.

In perinatal leukomalacia, the brain pathology exhibits several different distribution patterns, according to cerebrovascular and glial maturity or various causal factors. Periventricular leukomalacia occurs in the prenatal as well as the postnatal period, and is caused by, in addition to predisposing factors, cerebral hypoperfusion which is in turn caused by systemic hypotension or intracranial vascular constriction and circulatory disturbance. Oligodendroglial damage or diffuse astrogliosis associated with leukomalacia may lead to delayed or reduced myelination in the cerebral white matter.

Abnormal venous drainage in periventricular leukomalacia.

Age-matched six control subjects and 14 selected paediatric patients with periventricular leukomalacia were prospectively studied by MR imaging, and MR angiography. MR angiograms were acquired utilizing either the three-dimensional phase-contrast (3D-PC) or the two-dimensional time-of-flight (2D-TOF) techniques, or both, in an axial slab centred to the centrum semiovale. Abnormal draining veins (slightly enlarged and long vessels with abnormal courses) were found in eight of 14 patients (57%). In five of these eight patients (62.5%) abnormal vessels were identified on conventional MR imaging (

[Ultrasound morphologic changes in periventricular leukomalacia].

Therapeutic advances in neonatology cause fewer hypoxic conditions. In relation with this we observe fewer high grade brain haemorrhages. Therefore, ischemical brain changes through brain oedema (12%) and resulting colliquation necroses become the focal point of diagnostic and therapeutic work. 7.3% of intensively treated neonates showed the sonographic picture of a periventricular leucomalacy (PVL). The polycystic areas can be seen on the eight day after birth at the earliest and at six months they turn into a glia scar which is not very obvious on the sonograph. We found a relation between b

Periventricular leukomalacia: ultrasonic and neuropathological correlations.

Ultrasound scans of a preterm neonate (27 weeks gestation) at seven weeks of age showed periventricular echo-free cavities, but these were no longer visible at 15 weeks, three weeks before the infant died. At autopsy, a linear glial scar, extending from the periventricular white-matter into the white axis of the parasagittal gyrus, was found in the area occupied by the periventricular cysts. The larger cavity was reduced to a slit-like excavation in the midst of glial tissue. Unsuspected focal infarcts in the cerebral cortex were also found. This observation demonstrates that transient echo-fr

Periventricular leukomalacia: evaluation with MR imaging.

We reviewed 24 magnetic resonance (MR) images of 12 infants diagnosed as having periventricular leukomalacia based on ultrasound results. Abnormalities were identified on the MR images of every patient and were divided into three categories: distribution of low-signal-intensity areas within the cerebral white matter, degree of ventriculomegaly and brain atrophy, and extent of myelination. Follow-up examinations on six patients were also compared for significant changes. Abnormalities noted on MR images that most consistently related to poor outcome for the patient were low-signal-intensity les

Sudden infant death with periventricular leukomalacia.

Periventricular leukomalacia (PVL) is a form of cerebral infarction occurring in neonates, particularly in low-weight and premature infants. PVL is well-known to neonatologists, but generally considered nonfatal. Many infants with PVL die in the hospital with multiple medical problems. Those infants with PVL who survive because of intensive care will have serious motor and sensory deficits, but these problems are rarely recognized before one year of age. When infants with PVL die at home, death seems sudden and unexpected. However, it is important to distinguish death caused by PVL from the Su

Periventricular leukomalacia: a correlation study between real-time ultrasound and autopsy findings. Periventricular leukomalacia in the neonate.

The aim of this study was to validate the accuracy of real-time ultrasound (US) in the diagnosis of periventricular leukomalacia (PVL). US changes of PVL were correlated with autopsy results. During a 12-month period, all premature infants of 34 weeks' gestation or less (group A) and all neonates of more than 34 weeks' gestation who presented with abnormal neurological signs (group B) were studied with an ATL mechanical sector scanner (5 and 7.5 MHz). The overall incidence of PVL was 13.3%. In group A (n = 83), 13 infants had PVL and 3 died. In group B (n = 36), three developed PVL and two die

[Periventricular leukomalacia prevalence in premature newborn].

BACKGROUND: the periventricular leukomalacia (PLM) is considered a lesion of the central nervous system secondary to a hypoxic-ischemic insult, and affects more to premature than term neonates. However, the reported rates in the literature are between 2 % to 25 % in premature neonates. The objective was to determine the prevalence of PLM in premature neonates. METHODS: a two year prospective screening program with transfontanel ultrasound in premature neonates weighing < 2000 g was carried out at 4 weeks of age. RESULTS: from 38 355 consecutive deliveries 562 were premature neonates weighted

Periventricular leukomalacia and placental histopathologic abnormalities.

OBJECTIVE: To estimate whether there are placental histopathologic abnormalities associated with neonatal periventricular leukomalacia (PVL), a major precursor of cerebral palsy. METHODS: This is a case-control study of 167 neonates born between 23 and 34 weeks of gestation diagnosed with PVL by head ultrasonography within 6 weeks of birth, and 167 control neonates without neurologic morbidity matched by gestational age. Placentas for both case neonates and control neonates were reviewed by two perinatal pathologists who were blinded to neonatal course. RESULTS: Neonates with PVL were signif

Periventricular leukomalacia: overview and recent findings.

Periventricular leukomalacia (PVL), the main substrate for cerebral palsy, is characterized by diffuse injury of deep cerebral white matter, accompanied in its most severe form by focal necrosis. The classic neuropathology of PVL has given rise to several hypotheses about the pathogenesis, largely relating to hypoxia-ischemia and reperfusion in the sick premature infant. These include free radical injury, cytokine toxicity (especially given the epidemiologic association of PVL with maternofetal infection), and excitotoxicity. Among the recent findings directly in human postmortem tissue is tha

Unexpected versus anticipated cystic periventricular leukomalacia.

The objective of this study is to contrast the clinical profile of premature infants who develop cystic periventricular leukomalacia (cPVL) following a severe hemodynamic event (anticipated) with infants whose cPVL is not preceded by such an event (unexpected). The study design is a Retrospective Cohort. The study setting is a Neonatal Intensive Care Unit. Infants born at a gestational age of 23-32 weeks received serial cranial ultrasounds to screen for cPVL. Infants were divided into two groups based upon whether or not they had experienced a hemodynamic event of sufficient severity to potent

[Visuo-constructive disorders in periventricular leukomalacia].

INTRODUCTION: Cerebral palsy involves not only motor impairment but also the impairment of cognitive functions. Visuo spatial impairment has often been reported in preterm children with spastic diplegia due to periventricular leukomalacia. PATIENTS AND METHODS: Eight children with periventricular leukomalacia diagnosis. When examined their ages were between 3 and 13 years. RESULTS: We found visuo spatial anomalies in all subjects. These anomalies don't result in reading and writing learning difficulties. CONCLUSION: Constructional dyspraxia is frequently found in children with periventricul
