Lewy Body Dementias: Dementia With Lewy Bodies and Parkinson Disease Dementia.

PURPOSE OF REVIEW: This article provides an overview of the clinical features, neuropathologic findings, diagnostic criteria, and management of dementia with Lewy bodies (DLB) and Parkinson disease dementia (PDD), together known as the Lewy body dementias. RECENT FINDINGS: DLB and PDD are common, clinically similar syndromes that share characteristic neuropathologic changes, including deposition of α-synuclein in Lewy bodies and neurites and loss of tegmental dopamine cell populations and basal forebrain cholinergic populations, often with a variable degree of coexisting Alzheimer pathology.

Visual dysfunction in dementia with Lewy bodies.

PURPOSE OF REVIEW: To review the literature on visual dysfunction in dementia with Lewy bodies (DLB), including its mechanisms and clinical implications. RECENT FINDINGS: Recent studies have explored novel aspects of visual dysfunction in DLB, including visual texture agnosia, mental rotation of 3-dimensional drawn objects, and reading fragmented letters. Recent studies have shown parietal and occipital hypoperfusion correlating with impaired visuoconstruction performance. While visual dysfunction in clinically manifest DLB is well recognized, recent work has focused on prodromal or mild cogn

Cerebral microbleeds in dementia with Lewy bodies.

INTRODUCTION: Cerebral microbleeds (CMB) are associated with cognitive impairment and hypertensive or cerebral amyloid angiopathy. The pathophysiology and clinical significance of CMB in dementia with Lewy bodies (DLB) are not well understood. Our study aimed to investigate the prevalence of CMB in DLB and to estimate the magnitudes of their clinical associations. METHODS: Twenty participants with DLB (mean age 74 ± 5 years) were included in this cross-sectional study. All participants underwent 3 T magnetic resonance imaging. CMB number and location were assessed on susceptibility-weighted i

Pharmacological Management of Dementia with Lewy Bodies.

Dementia with Lewy bodies (DLB) is a complex disease that involves a variety of cognitive, behavioral and neurological symptoms, including progressive memory loss, visual hallucinations, parkinsonism, cognitive fluctuations and rapid eye movement sleep behavior disorder (RBD). These symptoms may appear in varying combinations and levels of severity in each patient who is seen in the clinic, making diagnosis and treatment a challenge. DLB is the third most common of all the neurodegenerative diseases behind both Alzheimer's disease and Parkinson's disease (PD). The median age of onset for DLB (

Clinical imaging in dementia with Lewy bodies.

Dementia with Lewy bodies (DLB) is a common neurodegenerative dementia in older people; however, the clinical features, particularly cognitive fluctuations and rapid eye movement sleep disorder, are often hard to elicit, leading to difficulty in making the diagnosis clinically. Here we examine the literature for the evidence behind imaging modalities that could assist in making the diagnosis. Dopamine transporter (DAT) imaging remains the best modality for differentiation from dementia of Alzheimer's type with high sensitivity and specificity reported based on pathological diagnoses. 123Iodine

Dementia with lewy bodies in meige syndrome.

Meige syndrome is a rare form of segmental dystonia characterized by blepharospasm and oromandibular dystonia. A few case reports of Meige syndrome have been associated with Lewy body pathologies, and the syndrome was also proposed for inclusion in the spectrum of Lewy body disease. We report a case of an elderly gentleman with a history of Meige syndrome for more than 10 years who developed dementia with Lewy bodies. Updated clinical and pathological evidence of linkages between these two conditions is also presented.

[Autonomic disorders in dementia with Lewy bodies].

Fifty patients diagnosed with dementia with Lewy bodies (DLB) according to criteria of I. MacKleith et al (2000), mean age 71.1 +/- 6.1 years, have been studied. Severity of dimentia was evaluated using a battery of neuropsychological scales and tests. A state of autonomic nervous system function was determined during clinical examination, the results of which were used as a basis for the Scale of autonomic disorders severity, and by instrumental methods. Most of patients with DLB had autonomic disorders, such as orthostatic hypotension, heat intolerability, dryness of cutaneous epithelium and

Dementia with Lewy bodies: diagnosis and management.

OBJECTIVE: To summarize the clinical, pathological, imaging and treatment aspects of dementia with Lewy bodies (DLB). METHOD: Review of literature (MEDLINE). RESULTS: DLB is the second most common form of degenerative dementia, accounting for up to 20% of cases in the elderly. It is characterized by fluctuating cognitive impairment, spontaneous parkinsonism and recurrent visual hallucinations. Consensus clinical criteria have been published and have been shown to have high specificity, but they may still lack sensitivity. Pathologically, DLB may be classified as a Lewy body (LB) disorder and

Dementia with Lewy Bodies and Sleep Dysfunction.

Neurodegeneration in dementia with Lewy bodies affects all crucial networks responsible for sleep control and as a result, the sleep cycle is heavily disturbed. Certain sleep syndromes such as rapid eye movement sleep behavior disorder and hypersomnia are particularly common and characteristic features of the disease, but patients also suffer from insomnia, sleep disordered breathing, movement disorders during sleep, or nighttime urinary dysfunction. Several treatment options are available nowadays; however, more trials on efficacy and safety in this population are still needed.

Gut microbiota in dementia with Lewy bodies.

Gut microbiota and fecal bile acids were analyzed in 278 patients with α-synucleinopathies, which were comprised of 28 patients with dementia with Lewy bodies (DLB), 224 patients with Parkinson's disease (PD), and 26 patients with idiopathic rapid eye movement sleep behavior disorder (iRBD). Similarly to PD, short-chain fatty acids-producing genera were decreased in DLB. Additionally, Ruminococcus torques and Collinsella were increased in DLB, which were not changed in PD. Random forest models to differentiate DLB and PD showed that high Ruminococcus torques and high Collinsella, which presuma

Tactile hallucinations in dementia with Lewy bodies.

BACKGROUND: Visual hallucinations (VH) are one of the most common psychological symptoms of dementia with Lewy bodies (DLB). It is generally considered that the VH that occur in DLB usually disappear when patients try to touch imaginary objects. However, DLB patients also sometimes experience tactile hallucinations (TH). To the best of my knowledge, this is the first report to comprehensively describe the TH experienced by DLB patients. METHODS: I searched the literature for case reports that described TH in Parkinson's disease (PD), PD dementia, or DLB. There were four reports regarding TH i

Molecular Imaging of Dementia With Lewy Bodies.

Dementia with Lewy bodies (DLB) is the second most common cause of neurodegenerative dementia. The core clinical features of DLB include fluctuating cognition, visual hallucinations, rapid eye movement sleep behavior disorder, and parkinsonism. Molecular imaging is a powerful tool to assess the brain function in vivo. In this chapter, we reviewed the positron emission tomography, single-photon emission computed tomography, and [123I]-metaiodobenzylguanidine scintigraphy studies evaluating the pathological processes underlying DLB, including altered brain metabolism and neurotransmitter pathway

LRRK2 variation and dementia with Lewy bodies.

INTRODUCTION: The leucine-rich repeat kinase 2 (LRRK2) gene contains several variants that cause Parkinson's disease (PD) and others that modify PD risk. However, little is known about the role of LRRK2 in dementia with Lewy bodies (DLB). Aims of this study were to screen DLB patients for pathogenic LRRK2 variants and to evaluate associations between common LRRK2 variants and risk of DLB. METHODS: 417 clinical DLB patients and 1790 controls were included in the primary analysis. Additionally, 355 Lewy body disease patients assessed as having a high likelihood of clinical DLB based on neuropat
