While GPT-3.5 is unable to pass the Physician Licensing Exam in Taiwan, GPT-4 successfully meets the criteria.

BACKGROUND: This study investigates the performance of ChatGPT-3.5 and ChatGPT-4 in answering medical questions from Taiwan's Physician Licensing Exam, ranging from basic medical knowledge to specialized clinical topics. It aims to understand these artificial intelligence (AI) models' capabilities in a non-English context, specifically traditional Chinese. METHODS: The study incorporated questions from the Taiwan Physician Licensing Exam in 2022, excluding image-based queries. Each question was manually input into ChatGPT, and responses were compared with official answers from Taiwan's Minist

Comparison of GPT-4o and o3-Mini on Otolaryngology USMLE-Style Questions.

Outcomes were to compare the accuracy of 2 large-language models-GPT-4o and o3-Mini-against medical-student performance on otolaryngology-focused, USMLE-style multiple-choice questions. With permission from AMBOSS, we extracted 146 Step 2 CK questions tagged "Otolaryngology" and stratified them by AMBOSS difficulty (levels 1-5). Each item was presented verbatim to GPT-4o and o3-Mini through their official APIs; outputs were scored correct/incorrect. Historical, de-identified student responses to the same items served as the comparator. Accuracy (%) was calculated per difficulty tier. Group dif

A comparison of physician examiners and trained assessors in a high-stakes OSCE setting.

BACKGROUND: The Medical Council of Canada (MCC) administers an objective structured clinical examination for licensure. Traditionally, physician examiners (PE) have evaluated these examinees. Recruitment of physicians is becoming more difficult. Determining if alternate scorers can be used is of increasing importance. METHOD: In 2003, the MCC ran a study using trained assessors (TA) simultaneously with PEs. Four examination centers and three history-taking stations were selected. Health care workers were recruited as the TAs. RESULTS: A 3x2x4 mixed analyses of variance indicated no significa

Can all roads lead to competency? School levels effects in Licensing examinations scores.

At the foundation of research concerned with professional training is the idea of an assumed causal chain between the policies and practices of education and the eventual behaviours of those that graduate these programs. In medicine, given the social accountability to ensure that teaching and learning gives way to a health human resource that is willing and able to provide the healthcare that patients and communities need, it is of critical importance to generate evidence regarding this causal relationship. One question that medical education scholars ask regularly is the degree to which the u

Three large language models demonstrate competitive performance in Traditional Chinese Medicine national medical licensing examinations over two years.

Large language models (LLMs) are increasingly integrated into medical education and assessment. However, their performance in high-stakes, non-English medical licensing examinations-particularly within culturally distinct medical systems such as Traditional Chinese Medicine (TCM)-remains insufficiently evaluated. This study aimed to systematically compare the performance of Gemini 3 Pro, DeepSeek V3.1, and GPT-5.2 in the Chinese National Traditional Chinese Medicine Licensing Examination (TCMLE) over two consecutive years (2023 and 2024), and to assess their potential implications for TCM educ

National Learning Objectives in Dermatology and Venereology for the New Swedish Medical Programme.

Effective medical education relies on clearly defined learning objectives that foster deep and meaningful learning. This article presents a consensus-driven proposal for national learning objectives in dermatology and venereology within Sweden's new medical programme qualifying for licensure. The primary aim is to harmonize educational standards across all medical faculties and to establish a common understanding of the expected level of knowledge and competence at the point of medical licensure. Using a structured approach, existing curricula were analysed and categorized according to a modif

Performance of large language models in medical licensing examinations: a systematic review and meta-analysis.

PURPOSE: This study systematically evaluates and compares the performance of large language models (LLMs) in answering medical licensing examination questions. By conducting subgroup analyses based on language, question format, and model type, this meta-analysis aims to provide a comprehensive overview of LLM capabilities in medical education and clinical decision-making. METHODS: This systematic review, registered in PROSPERO and following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, searched MEDLINE (PubMed), Scopus, and Web of Science for relevant

The USMLE Step 1 Pass/Fail Reporting Proposal: Another View.

The Association of Program Directors in Radiology recently issued a statement endorsing continued reporting of results of the United States Medical Licensing Examination (USMLE) as a three-digit score. While this position was approved by the Association of Program Directors in Radiology Board of Directors, it does not reflect the opinions of all radiology program directors. Here, we present an argument in support of reporting USMLE results as pass/fail. As a psychometric instrument, the USMLE Step 1 is designed to assess basic science knowledge and intended to inform a binary decision on licen

An evaluation of outcomes following the replacement of traditional histology laboratories with self-study modules.

Changes in medical school curricula often require educators to develop teaching strategies that decrease contact hours while maintaining effective pedagogical methods. When faced with this challenge, faculty at the University of Cincinnati College of Medicine converted the majority of in-person histology laboratory sessions to self-study modules that utilize multiple audiovisual modalities and a virtual microscope platform. Outcomes related to this shift were investigated through performance on in-house examinations, results of the United States Medical Licensing Examination® (USMLE® ) Step 1

DeepSeek R1 Distilled Fails to Perform Well Against the USMLE and Other LLMs with and Without Semantics.

DeepSeek-R1 Distilled Large Language model (LLM) was touted to be almost as good and much cheeper to generate than traditional LLMs. We compared its ability to answer medical questions from the United States Medical Licensing Examinations (USMLE) and found that the performance drop from the original model to DeepSeek was significant. DeepSeek is not yet a good alternative for medical question answering.

Fitness to drive and seizures - Recent medicolegal considerations to tell or not to tell? - THAT IS THE QUESTION!

This paper reviews a Coroner's inquest into the deaths of two people. The deaths were caused when a driver had a seizure, and lost control of his car, which then plowed into a café. The Coroner reviews the roles and responsibilities of primary and tertiary caregivers, with special focus on the question of who has the responsibility to inform the Drivers' Licensing Authority of a patient's unfitness to drive - the patient or the practitioner? The Coroner recommends the establishment of a multi-agency committee to advance work in this area. The paper notes a separate but parallel development - a

Afghanistan, poppies, and the global pain crisis.

The World Health Organization has reported that somewhere between 30-86 million people suffer from moderate to severe pain due to cancer, HIV/AIDS, burns, wounds and other illnesses annually and do not have access to proper opiate anesthetics to control the pain [1]. The vast majority of these people live in poor nations where medicinal opiates are either too expensive or not readily available. In this paper, it is argued that access to adequate healthcare is a human right and that adequate healthcare includes management of pain. The solution to this problem may be in Afghanistan, a country no

Impact of New York State's new licensure regulations for laboratory professionals on staffing of clinical laboratories.

Licensing of laboratory professionals has been a controversial issue for the individuals working in these professions for many years. In New York State (NYS), licensing of laboratory professionals has been debated for over three decades and did not become law until 2005. The NYS licensure law stipulates specific educational requirements that include course work as well as curricular content areas. In addition to these educational requirements, the licensure law stipulates successful completion of a certification examination for new licensure applicants. To determine if the new legislation in N

International medical graduates' experiences of clinical competency assessment in postgraduate and licensing examinations: A scoping review protocol.

An international medical graduate (IMG) is a doctor who has received their basic medical qualification from a medical school located in a different country from that in which they practice or intend to practice. IMGs are known to face difficulties in their working lives, including differential attainment in assessment. The objective of this review is to map key concepts and types of evidence in academic and gray literature relating to international medical graduates' experiences of clinical competency assessment and to identify knowledge gaps on this topic by systematically searching, selectin

Early prediction of the risk of scoring lower than 500 on the COMLEX 1.

BACKGROUND: The Comprehensive Osteopathic Medical Licensing Examination of the United States Level 1 (COMLEX 1) is important for medical students to be able to graduate. There is a glaring need to identify students who are at a significant risk of performing poorly on COMLEX 1 as early as possible so that extra assistance can be provided to those students. Our goal is to produce a reliable predictive model to identify students who are at risk of scoring lower than 500 on COMLEX 1 at the earliest possible time. METHODS: Academic data from medical students who matriculated at Rocky Vista Univer

Continuing medical education: viewpoints of Canadian psychiatric residents.

Continuing Medical Education (CME), relicensure and recertification are controversial issues for physicians today. The viewpoints of 142 Canadian psychiatric residents from 12 training centres across Canada were surveyed. The majority (73%) of the residents did not think that CME would pose a problem for them after certification. In fact, a large majority (77%) felt CME should be required after certification. Close to half of the residents favoured monitoring of CME by the Canadian Psychiatric Association. In contrast to these results, few favoured mandatory recertification. A formal relicensu

Shelf help: what powers medical student subject exam performance in the United States.

BACKGROUND: Following the change in 2022 of a scored United States Medical Licensing Examination (USMLE) Step 1 to a binary pass-fail grading system, greater emphasis has been placed on the Step 2 Clinical Knowledge (Step 2 CK) exam. Shelf or subject exams offered by the National Board of Medical Examiners (NBME) help prepare students for Step 2CK and therefore deserve the attention of clerkship directors. This paper reviews the literature pertaining to the factors that positively and negatively impact medical students' performance on the shelf exams, including but not limited to performance i
