BACKGROUND: The anterior cruciate ligament (ACL) is a major sagittal plane stabilizer of the knee joint. Even if anterior laxity can be brought under control by utilizing modern surgical techniques, internal rotational instability may not always be controlled adequately. Various surgical techniques are used to prevent this coronal or rotational instability, such as lateral extra-articular tenodesis (LET). Despite an abundance of articles in recent decades discussing LET in ACL reconstruction, no definitive indicators for extra-articular tenodesis have been described in the literature.

BACKGROUND: Transarticular screw fixation is a common surgical treatment for tarsometatarsal ligamentous (Lisfranc) injuries. Iatrogenic damage to articular cartilage from screw placement, however, has been thought to potentially lead to increased risk of tarsometatarsal (TMT) joint arthritis after initial injury. To date, no study has evaluated the effect of weightbearing on articular cartilage after screw fixation. The aim of this study was to create a Lisfranc injury and quantify and compare articular damage due to screw fixation before and after simulated weightbearing.
METHODS: A ligamen

PURPOSE: The purpose of this study was to evaluate a series of intra-articular distal radius fractures (DRFs) to determine whether patients without radiographic evidence of scapholunate (SL) ligament injury have a difference in outcomes in comparison with patients with radiographic evidence of SL ligament injury and no ligament repair or reconstruction. Our hypothesis is that there are no significant differences in outcomes between patients after treatment of their intra-articular DRF.
METHODS: A retrospective analysis of patients from a single institution who sustained an intra-articular DRF

Posterior cruciate ligament (PCL) injuries commonly occur in association with participation in sporting or recreational activities or due to a direct trauma. Cartilage and meniscal lesions are prevalent in PCL-injured knees with increasing likelihood and severity based on extent and duration of trauma to the knee. As such, comprehensive diagnostics should be performed to ascertain all related pathology, and patients should be thoroughly educated regarding treatment options, likely sequelae including posttraumatic osteoarthritis, and associated outcomes. Treatments should address the joint as a

BACKGROUND: A modified Lemaire lateral extra-articular tenodesis (LET) is a procedure that is designed to address anterolateral complex (ALC) deficiency. The procedure is performed as an augmentation to anterior cruciate ligament reconstruction (ACLR) to reduce anterolateral rotatory laxity. Studies have demonstrated improved rotational control and reduced failure rates of ACLR when LET is added. This is particularly helpful in young patients with high-grade rotatory laxity returning to contact pivoting sport, and in the revision ACLR scenario.
DESCRIPTION: A 6-cm skin incision is placed just

Tenosynovial giant cell tumors originate from the synovial tissue of the joints, tendon sheaths, mucosal bursas, and fibrous tissues adjacent to tendons. The disease presents in localized and diffused forms. Large joints, such as the knee, are not frequently affected. Magnetic resonance imaging has been reported to be the best noninvasive technique to diagnose these tumors. Magnetic resonance imaging diagnosis has to be confirmed by histopathological examination. Few reports exist of tenosynovial giant cell tumors arising from the posterior cruciate ligament. This article describes a case of a

We report a rare case of late femoral interference screw migration in the posterior compartment of the knee after anterior cruciate ligament reconstruction. The graft was intact with no signs of damage. The screw was successfully removed through the posteromedial portal site and the patient regained full function of the knee.

OBJECTIVE: The trapeziometacarpal (TMC) joint, a common site of osteoarthritis (OA) in postmenopausal women, relies on ligamentous structures for stability due to minimal bony constraint. However, the role of ligamentous constraint in OA pathomechanics, specifically its relationship with joint kinematics and surface morphology across disease stages, remains poorly understood.
METHOD: Forty-six cadaveric female TMC joints across OA stages were analyzed. Articular surface topography was assessed via space distance, curvedness, congruence, and global curvature. Kinematics were recorded via 3D mo

BACKGROUND: Anterior cruciate ligament (ACL) injury in children is an increasingly common occurrence. Historically, nonsurgical treatment has been the main treatment option; however, the indication for surgical reconstruction is increasing, as the histological characteristics of the immature skeleton are better known.
INDICATIONS: The extra-articular surgical technique for ACL reconstruction is a good option for Tanner I patients, aged up to 8 years, with knee instability and recurrent pain. Its advantage in skeletally immature patients is due to the fact that it avoids the bone growth plates

BACKGROUND: The anterior cruciate ligament (ACL) is a primary restraint to anteroposterior as well as rotatory knee laxity. In case of concomitant lesion of menisci or other ligamentous structures, further dynamic instability is encountered. A lateral extra-articular tenodesis (LET) augmentation has been proposed by the Authors to treat or prevent residual laxity.
INDICATIONS: ACL reconstruction is recommended in young athletes involved in pivoting sports, non-contact pivoting injuries, high-grade pivot shift, deep notch sign and double bone bruise, meniscal loss, and revision of previous bon

Lateral extra-articular tenodesis and other anterolateral procedures improve patient outcomes when combined with anterior cruciate ligament reconstruction. Failure rates are >50% lower in young patients at high risk of reinjury. We defined patients at high risk as those aged 14 to 25 years with 2 or more factors of the following criteria: 1) returning to contact pivoting sport, 2) high-grade anterolateral rotatory laxity, as measured by pivot shift test (grade 2 or higher), and 3) generalized ligamentous laxity (Beighton score greater than 4) or knee hyperextension recurvatum of greater than 1

BACKGROUND: Arthroscopic techniques have gradually become popular for anterior talofibular ligament (ATFL) repair. However, the reliability of the repair and the risk of nerve injuries are of concern. The aim of this study was to evaluate the fully intra-articular lasso-loop stitch technique for arthroscopic ATFL repair.
METHODS: From October 2018 to December 2019, 43 cases with chronic lateral ankle instability were treated with this fully intra-articular lasso-loop stitch technique using the Arthro-Pierce instrument. The Karlsson-Peterson score, Cumberland Ankle Instability Tool (CAIT) scor

BACKGROUND: The pathoanatomy and biomechanics of elbow instability have been previously reported; however, few researchers have dealt with the operative management and clinical consequence of recurrent elbow instability. Recent studies on arthroscopic lateral collateral ligament (LCL) complex repair have reported successful outcomes similar to those achieved by open repair. We aimed to determine the validity of arthroscopic repair of the LCL complex in elbows with unstable dislocation with or without intra-articular fracture.
METHODS: Eighteen consecutive patients who had undergone arthroscop

OBJECTIVE: It is unclear if anterior cruciate ligament (ACL) reconstruction can prevent the onset of degenerative changes in the knee. Previous studies were inconclusive on this subject. The aim of this study was to systematically review all studies on the effect of ACL reconstruction on articular cartilage in animals.
DESIGN: Pubmed and Embase were searched to identify all original articles concerning the effect of ACL reconstruction on articular cartilage compared with both its positive (ACL transection) and negative (sham and/or non-operated) control in animals. Subsequently a Risk of bias

BACKGROUND: Revision anterior cruciate ligament (ACL) reconstruction has been documented to have worse outcomes compared with primary ACL reconstructions.
PURPOSE/HYPOTHESIS: The purpose of this study was to determine if the prevalence, location, and/or degree of meniscal and chondral damage noted at the time of revision ACL reconstruction predicts activity level, sports function, and osteoarthritis symptoms at 2-year follow-up. The hypothesis was that meniscal loss and high-grade chondral damage noted at the time of revision ACL reconstruction will result in lower activity levels, decreased
