Extracorporeal shock wave lithotripsy (ESWL) is one of the most frequently applied procedures for the treatment of urolithiasis. ESWL breaks and splits stones by the means of repeated acoustic shock waves. Despite its non invasive nature, ESWL has been intuitively associated with potential complications, mostly related to residual stone fragments. While non stone-related complications are rare (< 1 %), awareness and identification of these complications could help clinicians to prevent and manage them safely and effectively. The current study reviews the pathophysiology, predicting factors and

Symptomatic salivary stones in the middle or proximal parotid duct have traditionally been treated by gland excision, which is associated with a 3% to 27% risk to the facial nerve in the pediatric population. Minimally invasive approaches to the management of salivary duct calculi have been introduced over the last several years. Fluoroscopically guided basket retrieval, lithotripsy, and intraoral stone removal under general anesthesia have found favor with many surgeons. Our patient had extracorporeal lithotripsy to his parotid gland with complete disintegration of the stone. He has had no ev

PURPOSE: This article describes a technique of salivary gland endoscopy using a 2.1-mm endoscope with a 1-mm working channel. The technique allows intracorporeal lithotripsy under endoscopic control. Two types of energy to produce calculi fragmentation were analyzed.
PATIENTS AND METHODS: Of 39 patients who presented with obstructive sialoadenitis, endoscopic treatment was possible in 27. Eighteen had a diagnosis of sialolithiasis. Intracorporeal lithotripsy was done under endoscopic control in these patients. Laser energy was used to produce fragmentation in 3 cases and pneumobalistic energy

Extracorporeal shock-wave lithotripsy (ESWL) has been widely introduced in the treatment of nephro-uretero-lithiasis during the last decade as it provides a noninvasive removal of the stones from the upper urinary tracts. The paper covers the history, techniques, indications, contraindications, anesthesia, complications and results of the method. Positive and negative characteristics of ESWL equipment (lithotriptors) are considered. The experience with 5000 lithotripsies led the authors to the conclusion that ESWL does not solve all the problems in the treatment of nephro-uretero-lithiasis and

Over a 13-month period (November 1988 to December 1989), we performed our first 104 laparoscopic cholecystectomies using an intracorporeal ultrasonic lithotripsy technique. The procedure in three of these patients was converted to an open operation because of hemorrhage with unexpected findings of cirrhosis in two patients and dense subhepatic adhesions in a third. Endoscopic retrograde cholangiopancreatography was successfully used in three other patients in whom common bile duct stones were identified. We encountered only three postoperative complications out of all the laparoscopic cholecys

Percutaneous nephrostomy and stone removal was performed 240 times in 84 patients during a five-year period. Surgery was necessary in only two cases; hemorrhage requiring blood transfusion occurred in four. Perforation of the renal pelvis in five patients healed without sequelae. Final control of the nephrostomy was achieved in almost all cases. Indications and technical points regarding percutaneous lithotripsy are discussed.

BACKGROUND: In addition to currently available modalities of intracorporeal lithotripsy (ultrasonic, electrohydraulic, and laser), a new ballistic lithotriptor known as the Swiss Lithoclast has recently gained attention. This study reports our experience with the Swiss Lithoclast in the endoscopic management of urinary calculi.
METHODS: A total of 51 patients with urinary calculi were treated with the Swiss Lithoclast; one patient with a renal calculus, 28 with ureteral calculi, and 22 with lower urinary tract (bladder, urethra and Kock pouch) calculi.
RESULTS: The Lithoclast successfully fr

Noninvasive or minimally invasive techniques for the treatment of biliary stones have been significantly advanced in the past two decades. The least invasive, extracorporeal shock-wave lithotripsy, continues to be used successfully, not only at its earliest reported site of use in Munich, Germany, but also in numerous other centers in the world. Lithotriptors that utilize a spark gap electrode or other sources of energy to generate sound waves and imaging by radiographic and ultrasonographic targeting of stones have proved to be effective in fragmenting biliary stones (90% to 95%) that are too

It was suggested to use contact ultrasonic lithotripsy in lumen of common bile duct during laparoscopic surgery in order to preserve the sphincter apparatus of major duodenal papilla. Waveguide with original structure was used for lithotripsy performing. Waveguide's total length is 400 mm. It has a radially curved 40 degrees broken part with diameter of 6 mm cone rolling in working portion with diameter of 4 mm and length of 60 mm for introduction in common bile duct. There is concave lens on waveguide working portion end with diameter of 1 mm. A lens permits concentrating the waves beam in lo

BACKGROUND AND OBJECTIVE: The objective of this study was to evaluate clinical use of an Alexandrite laser lithotripter for intracorporeal lithotripsy of urinary calculi.
STUDY DESIGN/MATERIALS AND METHODS: We prospectively evaluated a flash lamp pumped, Q-switched Alexandrite solid-state laser for use in conjunction with ureteroscopy (30 cases) or percutaneous nephrolithotripsy (2 cases). The laser operates at a wavelength of 755 nm in a pulsed mode with pulses of 150-800 ns duration at energy settings of 30-80 mJ.
RESULTS: The fragmentation rate with the Alexandrite laser alone was 50% (16

OBJECTIVE: This study aimed to comparatively analyze the indications, treatment outcomes, and reliability levels of the intraductal pneumatic lithotripsy (IPL) and holmium laser-assisted lithotripsy (HLL) methods that are used to sialendoscopically separate stones into smaller pieces in submandibular gland sialolithiasis (SMGS) patients. To the best of the author's knowledge, there is no study that compared these 2 methods in the literature in English.
METHODS: The retrospectively designed study included 51 patients with SMGS. The IPL was used to break up 32 stones in 28 patients, while HLL w

The aim of this retrospective study was to evaluate the efficacy of ureteroscopic lithotripsy (URSL) and extracorporeal shock wave lithotripsy (ESWL) in the treatment of middle and lower ureteric stones. From January 1996 to March 1997, 61 patients treated by URSL and 49 patients treated by ESWL were studied, both were conducted as outpatient procedures. URSL using Holmium laser and semirigid ureteroscope (Fr.8.5) performed under general anaesthesia had single session stone clearance rates of 100% and 95% for middle and lower stones respectively. There were 6 complications including 5 readmiss

INTRODUCTION: Pancreatic duct stones (PDS) pose a significant clinical challenge, and choosing treatment modality is crucial to achieving optimal outcomes. Extracorporeal shock wave lithotripsy (ESWL) has long been regarded as the primary intervention for PDS. However, per-oral pancreatoscopy-guided lithotripsy (POP), both electrohydraulic lithotripsy (EHL-POP) and laser lithotripsy (LL-POP), has emerged as a promising endoscopic alternative. This meta-analysis compares the efficacy and safety profiles of EHL-POP, LL-POP, and ESWL for treating PDS.

The major limitations of biliary extracorporeal shock wave lithotripsy (ESWL) relate to adequate stone fragmentation and clearance of the stone fragments. We hypothesized the removal of small fragments with concomitant catheter cholecystostomy during ESWL would improve its efficacy. ESWL with aspiration or flushing through three different cholecystostomy catheters was performed on pigs with surgically implanted gallstones. Twenty-two percent and 46% of the pre-ESWL stone weight were aspirated through the 10 and 12 French catheters respectively. The clearance, size, and visualization of fragmen

Management of heavily calcified lesions during percutaneous coronary intervention (PCI) is often associated with a high incidence of complications and long-term adverse outcomes as the suboptimal coronary stent expansion due to calcified lesion is one of the strongest predictors of adverse outcomes. Shockwave intravascular lithotripsy (S-IVL) is a new technique used in the optimal debulking and preparation of severely calcified coronary artery stenoses, but there is few data on its efficacy in acute or postponed optimization of underexpanded stents. We report a case of a 66-year-old male patie

Background: Acute calculous cholecystitis is the obstruction of the cystic duct by a gallstone that leads to inflammation of the gallbladder necessitating cholecystectomy. Case Series: We present the cases of 2 patients with acute calculous cholecystitis who were deemed ineligible candidates for cholecystectomy because of their complicating medical histories. Both patients initially underwent cholecystostomy and drain placement with interventional radiology for management of acute calculous cholecystitis. Their large gallstones remained refractory to attempts at removal by electrohydraulic lit
