Crosstalk Between Liver and Extra-Liver Organs During Liver Regeneration in Mammals.

Liver diseases are leading causes of death and disability worldwide. Therefore, the development of new treatments for liver disease is urgent. An integral part of the treatment of any liver disease is the process of regeneration of its parenchyma. However, the current approach to studying mammalian liver regeneration is one-sided, focusing solely on the liver itself and neglecting its interactions with other organs. This includes organs of the functional excretory system, such as the lungs and kidneys, which can secrete factors, including HGF, EGF, TGFβ, IL6, and others, into the bloodstream,

Liver transplantation for rare liver diseases and rare indications for liver transplant.

Liver transplantation (LT) is currently considered the gold standard treatment for end-stage liver failure. Compared to the first decades of its use, LT is associated with lower comorbidity and mortality, with a 5-year survival over 70%. Worldwide, liver cirrhosis and hepatocellular carcinoma represent the major indications to LT. However, almost 1% of LT is performed for rare diseases or rare indications, which include non-hepatocellular malignancy, vascular disorders, metabolic and congenital liver disorders. These diseases can lead to hepatocellular necrosis, biliary tree abnormality and/or

Autoimmunity to a liver membrane lipoprotein and liver damage in alcoholic liver disease.

Antibodies reacting with a liver membrane lipoprotein (LSP) have been detected by radioimmunoassay in the sera of 15 (27%) of 55 patients with alcohol-related liver lesions. There was a close association between the presence of the anti-LSP antibody and the findings on liver biopsy of a lymphocytic infiltrate in the portal tracts together with piecemeal necrosis of periportal hepatocytes. These histological features are characteristically found in the autoimmune disorder of chronic active hepatitis, in which anti-LSP antibodies are almost invariably present. It is suggested that in these cases

Liver.

The liver's unique metabolism and relationship to the gastrointestinal tract make it an important target of the toxicity of drugs and xenobiotics. The developmental changes that occur in the liver's metabolic activity from birth to adolescence contribute to the varied sensitivity to toxins seen in the pediatric population. Hepatic drug metabolism, often with an imbalance between the generation of toxic metabolites and detoxification processes, can influence the degree of hepatotoxicity. The decreased capacity of the neonatal liver to metabolize, detoxify, and excrete xenobiotics explains the p

Liver Transplantation for Liver and Perihilar Neoplasms.

Transplantation offers curative potential for patients with select primary hepatobiliary malignancies when disease biology or hepatic reserve limit surgical resection. Contemporary practice for transplant for hepatocellular carcinoma and cholangiocarcinoma emphasizes selection through radiographic criteria, biomarker thresholds, and downstaging and neoadjuvant protocols. In carefully selected patients, transplantation is associated with increased overall survival and reduced recurrence compared to resection. Evolving allocation policies, living donor expansion, and integration of systemic ther

[Pathology of the explanted liver in liver transplantation].

The primary diseases of 209 explanted livers of the transplantation center of the University of Heidelberg are presented. Liver cirrhosis was found in 48.8% as the primary disease followed by the group of malignancies of the liver (28.2%) with HCC as the predominant tumor. Fulminant liver failure was found in 13.8% as the primary disease. Metabolic disorders comprised a small group of 11 cases (5.3%). 7 cases were transplanted for Budd Chiari syndrome (3.3%) and one case was transplanted for E. alveolaris. The examination of fulminant viral hepatitis revealed evidence of the involvement of the

Liver-directed therapies for colorectal liver metastases.

Colorectal cancer is the third most common cancer diagnosed in the United States, with an anticipated 53,000 deaths from this disease in 2025. Greater than one in five patients will present with synchronous colorectal liver metastases (CRLMs), and 25%-30% will develop CRLMs during the course of the disease. In addition, the rate of liver-specific recurrence is high, with recurrences in 50%-75% of patients who previously underwent resection for liver metastasis. Surgical resection remains the cornerstone of treatment for resectable CRLM, but a significant proportion of CRLMs are unresectable on

Liver Transplantation in Alcohol-Associated Liver Disease.

Alcohol-associated liver disease (ALD) is the foremost cause of liver-related mortality in the United States comprising a spectrum of conditions from simple hepatic steatosis to more severe alcohol-associated cirrhosis and alcohol-associated hepatitis. There has been growing acceptance and application of early liver transplantation (eLT) for ALD. There is robust evidence for excellent patient and graft survival rates for eLT for ALD. Nevertheless, recidivism remains a major concern. This article aims to explore the recent trends in liver transplantation for ALD, as well as the advances in prac

Steatotic liver disease after pediatric liver transplantation.

Metabolic dysfunction-associated steatotic liver disease is becoming an increasingly frequent cause of chronic liver disease in children. It has been reported as a complication of liver transplantation in adults but remains poorly evaluated in liver-transplanted children. The aim of this study was to assess the prevalence and characteristics of steatotic liver disease in a large cohort of liver-transplanted children and to identify factors associated with it. In this single-center study of patients with pediatric liver transplants (n=122) with a median follow-up time of 14.0 years, steatosis w

Liver transplantation for alcohol-associated liver disease.

Alcohol-associated liver disease (ALD) is a major cause of morbidity and mortality worldwide, and a leading indication for liver transplantation (LT) in many countries, including the United States. However, LT for ALD is a complex and evolving field with ethical, social, and medical challenges. Thus, it requires a multidisciplinary approach and individualized decision-making. Short-term and long-term patient and graft survival of patients undergoing LT for ALD are comparable to other indications, but there is a continued need to develop better tools to identify patients who may benefit from LT

Blue Liver: Case Report of Blue Liver.

BACKGROUND Although many cases of unusual liver discoloration exist, such as blue liver syndrome which is linked to oxaliplatin-based chemotherapy, our finding was seen in a patient who was not on chemotherapy. A 39-year-old male who presented with jaundice was found to have blue liver discoloration. CASE REPORT A 39-year-old male presented with jaundice of one-month's duration evidenced by elevated total and direct bilirubin. An ultrasound and magnetic resonance cholangiopancreatography (MRCP) demonstrated thickened gall bladder wall but no common bile duct stones. A robotic-assisted laparosc

Ultrasound liver elastography beyond liver fibrosis assessment.

Several guidelines have indicated that liver stiffness (LS) assessed by means of shear wave elastography (SWE) can safely replace liver biopsy in several clinical scenarios, particularly in patients with chronic viral hepatitis. However, an increase of LS may be due to some other clinical conditions not related to fibrosis, such as liver inflammation, acute hepatitis, obstructive cholestasis, liver congestion, infiltrative liver diseases. This review analyzes the role that SWE can play in cases of liver congestion due to right-sided heart failure, congenital heart diseases or valvular diseases

HYPERVASCULAR LIVER LESIONS IN RADIOLOGICALLY NORMAL LIVER.

BACKGROUND: The hypervascular liver lesions represent a diagnostic challenge. AIM: To identify risk factors for cancer in patients with non-hemangiomatous hypervascular hepatic lesions in radiologically normal liver. METHOD: This prospective study included patients with hypervascular liver lesions in radiologically normal liver. The diagnosis was made by biopsy or was presumed on the basis of radiologic stability in follow-up period of one year. Cirrhosis or patients with typical imaging characteristics of haemangioma were excluded. RESULTS: Eighty-eight patients were included. The average

Liver fibrosis markers in alcoholic liver disease.

Alcohol is one of the main factors of liver damage. The evaluation of the degree of liver fibrosis is of great value for therapeutic decision making in patients with alcoholic liver disease (ALD). Staging of liver fibrosis is essential to define prognosis and management of the disease. Liver biopsy is a gold standard as it has high sensitivity and specificity in fibrosis diagnostics. Taking into account the limitations of liver biopsy, there is an exigency to introduce non-invasive serum markers for fibrosis that would be able to replace liver biopsy. Ideal serum markers should be specific for

Autoimmune liver disease, autoimmunity and liver transplantation.

Primary biliary cirrhosis (PBC), primary sclerosing cholangitis (PSC) and autoimmune hepatitis (AIH) represent the three major autoimmune liver diseases (AILD). PBC, PSC, and AIH are all complex disorders in that they result from the effects of multiple genes in combination with as yet unidentified environmental factors. Recent genome-wide association studies have identified numerous risk loci for PBC and PSC that host genes involved in innate or acquired immune responses. These loci may provide a clue as to the immune-based pathogenesis of AILD. Moreover, many significant risk loci for PBC an
