Systemic Therapy for Biliary and Liver Neoplasms: Chemotherapy and Immunotherapy.

Liver and biliary tract cancers represent some of the most therapeutically challenging malignancies, with rising global incidence. Hepatocellular carcinoma remains the most common primary liver cancer, typically developing in the context of cirrhosis and chronic liver injury, whereas biliary tract cancers encompassing gallbladder, intrahepatic, perihilar, and distal subtypes, arises from the biliary epithelium demonstrating aggressive biological behavior. Systemic therapy has undergone substantial evolution over the last decade, shifting from minimally effective cytotoxic chemotherapy toward a

[Central segments resections in primary liver neoplasms--three clinical cases].

BACKGROUND/AIMS: The resections of the central paramedial hepatic sectors are most rarely reported due to their technical complexity. We present three cases in which we carried out this rare operative intervention for occasion of primary hepatic neoplasm in the central hepatic segments. MATERIAL AND METHODS: In three cases (two of primary hepatic cancer and one of sarcoma) we carried out resection of the central segments in the following steps: mobilization of the liver, subhepatic approach to the inferior v. cava, preparation for vascular isolation of the liver by Pringle's maneuver, dissect

Eosinophilic Biliary Cystic Neoplasm of the Liver: A Case Report of an Unrecognized Subtype of Intrahepatic Biliary Neoplasm.

We report a multicystic intrahepatic neoplasm in a 54-year-old Japanese woman, representing a previously unrecognized subtype. Grossly, the lesion was a well-demarcated multicystic tumor with focal papillary projections. Histologically, the cysts were lined by columnar to cuboidal neoplastic cells with brush border-like luminal microvilli, abundant granular eosinophilic cytoplasm, and small round nuclei. The cystic lumina contained colloid-like secretion, and bile-filled glands were occasionally observed. The septa were composed of thin hepatocellular parenchyma. Immunohistochemically, the tum

Undifferentiated (embryonal) liver sarcoma: synchronous and metachronous occurrence with neoplasms other than mesenchymal liver hamartoma.

Undifferentiated (embryonal) liver sarcoma (UELS) is a rare tumor that typically occurs in children. The association of UELS with neoplasm other than mesenchymal liver hamartoma is exceedingly rare. The aim of the study was to report 3 cases of UELS, 2 of them being interesting because of their association with another neoplasm, vaginal embryonal rhabdomyosarcoma in a teenage girl and B-acute lymphoblastic leukemia in a middle-aged woman. Besides, one of our cases of UELS, in a 58-year-old woman, is an extremely rare presentation of such a tumor in a middle-aged adult. The patient's clinical f

Solid pseudopapillary epithelial neoplasm of the pancreas with liver metastasis.

A girl in early adolescence presented with complaints of abdominal pain lasting for 4 months, along with a palpable lump in the epigastric region. A CT scan revealed a large solid-cystic mass lesion measuring 9.5×10.7×12.1 cm, involving the body and tail of the pancreas. Additionally, well-defined hypodense lesions in the liver, localised to segments VIII and IVa, were observed, with the aetiology being indeterminate. An endoscopic ultrasound-guided biopsy of the pancreatic mass confirmed a diagnosis of solid pseudopapillary epithelial neoplasm (SPEN). Ultrasonography (USG) of the liver sugges

MR imaging of the liver. Primary malignant neoplasms in the adult.

Primary malignant liver neoplasms of the adult are discussed with emphasis on hepatocellular carcinoma, the most common neoplasm. The clinical and pathologic aspects of the following neoplasms are correlated with imaging features: hepatocellular carcinoma, fibrolamellar carcinoma, intrahepatic cholangiocarcinoma, angiosarcoma, epithelial hemangioendothelioma, other sarcomas, and lymphoma. The complementary role of ultrasound, CT, and MR imaging in characterizing these lesions and determining resectability is highlighted.

Toxicity of targeted anticancer treatments on the liver in myeloproliferative neoplasms.

The liver has a central role in metabolism, therefore, it is susceptible to harmful effects of ingested medications (drugs, herbs, and nutritional supplements). Drug-induced liver injury (DILI) comprises a range of unexpected reactions that occur after exposure to various classes of medication. Even though most cases consist of mild, temporary elevations in liver enzyme markers, DILI can also manifest as acute liver failure in some patients and can be associated with mortality. Herein, we briefly review available data on DILI induced by targeted anticancer agents in managing classical myelopro

Molecular and Immunohistochemical Analysis of Mucinous Cystic Neoplasm of the Liver.

OBJECTIVES: Mucinous cystic neoplasm of the liver is characterized by neoplastic mucinous and/or biliary epithelium surrounded by ovarian-type stroma. Immunohistochemical studies have shown that the ovarian-type stroma expresses estrogen receptor, suggesting potential hormonal responsiveness. The molecular biology of mucinous cystic neoplasm of the liver remains poorly studied. METHODS: Transcriptome sequencing and immunohistochemistry were performed on a series of mucinous cystic neoplasms. RESULTS: Mucinous cystic neoplasm of the liver exhibited significantly increased RNA expression of ov

Laparoscopic left hepatectomy for mucinous cystic neoplasm of the liver.

BACKGROUND: Among liver cystic lesions, mucinous cystic neoplasm of the liver (MCN-L) constitutes a challenging issue in terms of management: preoperative diagnosis is often unachievable and this may mislead to inappropriate treatment [1-3]. We present the case of an otherwise healthy 29-year-old female who underwent laparotomic cyst unroofing in segment 4 and cholecystectomy in another institution. Post-operative course was complicated by biliary leakage that was endoscopically treated. Short term follow-up showed early recurrence with a volumetric enlargement of the cyst occupying most of th

Liver mucinous cystic neoplasm: a challenging diagnosis case report.

INTRODUCTION AND IMPORTANCE: Liver mucinous cystic neoplasm is a rare tumor with a challenging pre-operative diagnosis, as for our patient. PRESENTATION OF CASE: A 43-year-old woman complained of dull, aching pain in the left hypochondrial region. Abdominal ultrasound demonstrated a solitary hypoechoic cyst within the left hepatic lobe. The initial suspected diagnosis was a hepatic hydatid cyst due to the country's endemicity. Magnetic resonance imaging (MRI) was consistent with a hepatic mucinous cystic neoplasm. A left hepatectomy was performed. Histopathological examination of the surgical

Prognostic significance of hyperammonemia in neuroendocrine neoplasm patients with liver metastases.

Neuroendocrine neoplasms (NENs) are rare, usually slow-growing tumors, often presenting with extensive liver metastases. Hyperammonemia due to insufficient hepatic clearance has been described in NEN cases; however, no systematic evaluation of risk factors and outcomes of NEN-associated hyperammonemia exists so far. This case report and retrospective review of NEN patients developing hyperammonemia from the years 2000 to 2020 at the Erasmus Medical Center in Rotterdam, the Netherlands, aimed to describe these patients and determine prognostic factors to improve evaluation and treatment. Hand-assisted Laparoscopic Repeat Hepatectomy for Secondary Liver Neoplasm.

BACKGROUND: Hand-assisted laparoscopic surgery is a widely accepted alternative to an open approach. The use of this technique in repeat liver resection is limited due to technical difficulties caused by postsurgical adhesions. We aimed to assess the feasibility and safety of hand-assisted laparoscopic repeat hepatectomy (HALRH). MATERIALS AND METHODS: This was a retrospective study of the medical files of patients who had undergone HALRH between 2010 and 2017 in 2 university-affiliated medical centers. RESULTS: Sixteen patients with repeat hepatectomy were included with a median age of 67.5

Portal cavernoma in a case of alcoholic liver disease with myeloproliferative neoplasm.

Chronic liver disease is a major cause of portal vein thrombosis. Further screening for other thrombotic risk factors in patients with portal vein thrombosis and chronic liver diseases is often neglected. In this paper, we report a case of portal cavernoma in a patient who was diagnosed with alcoholic liver disease and myeloproliferative neoplasm.

Klebsiella Pneumoniae Liver Abscess as the Herald of Underlying Colorectal Neoplasm.

BACKGROUND: Klebsiella pneumoniae liver abscesses (KPLA) remain endemic to Taiwan. Hypervirulent strains in the gut can translocate across colonic mucosal defects into the portal circulation, causing subsequent liver abscesses. Occult colorectal cancer in patients with KPLA has been reported in a case series. Herein, we determined whether the diagnosis of KPLA serves as a harbinger of colorectal neoplasms. METHODS: Consecutive patients with KPLA treated at Taipei Veterans General Hospital in Taiwan between April 2011 and January 2024 were retrospectively enrolled. Hypervirulent K. pneumoniae

Neuroendocrine neoplasms of the biliary tree, liver and pancreas: a pathological approach.

Neuroendocrine neoplasms of the pancreatobiliary tract and liver are a heterogeneous group that encompass a spectrum of entities with distinct morphological, biological and clinical features. Although in the various anatomical sub-sites of this region they show specific characteristics, these tumors, as a whole, share several etiological and clinical aspects. This review systematically addresses NENs arising in the extrahepatic bile ducts, gallbladder, liver and pancreas, with the principal aim of pinpointing essential diagnostic and classification issues. In addition, the section on hepatic N

Mucinous cystic neoplasm of the liver with biliary prolapse mimicking intraductal papillary neoplasm of the bile duct: a case report.

Mucinous cystic neoplasm of the liver (MCN-L) is a rare cyst-forming epithelial neoplasm that typically lacks bile duct communication. Intraductal protrusion is uncommon and can mimic cystic intraductal papillary neoplasm of the bile duct (IPNB). A 39-year-old woman undergoing fertility treatment presented with recurrent hyperbilirubinemia and predominantly cholestatic liver test abnormalities. MRI demonstrated a 10.5-cm multilocular cystic lesion in segment 4 of the liver with downstream biliary dilatation, and internal septa were visible within the dilated bile duct on T2-weighted single-sho

Progression of Intraductal Papillary Mucinous Neoplasms of the Pancreas After Liver Transplantation.

BACKGROUND: Intraductal papillary mucinous neoplasms (IPMNs) are the most commonly identified pancreatic cystic neoplasms. Incidentally detected IPMNs are common among liver transplant recipients. The risk of IPMN progression to pancreatic cancer in transplant recipients and the impact of immunosuppression on the risk of malignant transformation of IPMN are unclear. METHODS: In this retrospective study of consecutive liver transplant recipients across Mayo Clinic over a 13-year period, patients were assessed for possible IPMN by automated chart review. Pancreatic cystic lesions were character

[Liver neoplasms in patients with glycogen storage disease. Diagnostic and therapeutic problems].

In a family composed by eight brothers, five had a type I glycogen storage disease and three presented with liver tumors complicated with hemorrhage or malignant transformation, during the follow up. The periodic ultrasonographic control allowed the early diagnosis of these neoplasms. We describe the clinical picture and treatment of two patients. Metabolic alterations were corrected during the preoperative period with parenteral nutrition. Resection was successful in both cases. It is concluded that a close follow up and early treatment of these lesions is effective and avoids complications.
