Laxatives and faecal incontinence in long-term care.

High levels of faecal incontinence and laxative use in long-term care settings for older people are revealed in this study. Further research, together with more considered prescribing policies are needed, the authors suggest.

Long-term care insurance: a private sector challenge.

Powerful demographic and economic forces are fueling the looming crisis related to providing medical and social services for the elderly. However, the American public remains largely oblivious to the devastating implications of the need for institutional or home-based care among senior citizens. Employers have a key role to play in resolving the situation by evaluating competing long-term care policies, endorsing the program of a strong carrier, assisting in offering the policies through payroll deduction and educating employees on the importance of selecting policy riders that best meet their

Long-term ventilator-dependent care in nursing homes.

For some persons, being dependent on a ventilator is the only alternative to death. Most residents residing in New York state's approved ventilator-dependent beds have needed the support of a ventilator for more than a year. The quality of their lives since admission to specialized programs has been significantly different than it had been in hospitals. These residents are now actively participating in the life of the facility. Many who had not spoken during the entire time they were hospitalized are now speaking with the aid of special tracheostomy tubes. Many have been assisted out of bed fo

Peptic ulcer in geriatric long-term care medicine.

This study was based upon deceased patients in a geriatric university hospital with a high autopsy rate (81%). Of 6200 autopsied patients, 333 (5.4%) had had an active peptic ulcer; agonal and other acute erosions were not included. 257 cases were selected for the study (average age 83.8 yr). The diagnostic accuracy, and the symptoms of peptic ulcer in stationary, elderly, chronically ill patients were studied retrospectively. Only 16% of cases with duodenal ulcer and 29% with gastric ulcer had been correctly diagnosed antemortem. The clinical features of ulcer disease in the elderly may often

Long-term care of the patient with the atrial defibrillator.

With an aging population, atrial fibrillation is becoming an increasingly common cause of hospital admission. Patients with recurrent, symptomatic persistent atrial fibrillation often require repeated admissions to the hospital for cardioversion. The development of the atrial defibrillator has empowered such patients to take charge of their condition and perform cardioversion on themselves at home. This liberates them from the worry of hospitalization and can increase patient confidence. The implantation of an atrial defibrillator, however, has some disadvantages, and long-term use of the devi

Identifying Unexpected Deaths in Long-Term Care Homes.

OBJECTIVES: Predicting unexpected deaths among long-term care (LTC) residents can provide valuable information to clinicians and policy makers. We study multiple methods to predict unexpected death, adjusting for individual and home-level factors, and to use as a step to compare mortality differences at the facility level in the future work. DESIGN: We conducted a retrospective cohort study using Resident Assessment Instrument Minimum Data Set assessment data for all LTC residents in Ontario, Canada, from April 2017 to March 2018. SETTING AND PARTICIPANTS: All residents in Ontario long-term

Sexuality and physical intimacy in long-term care.

Sexuality and sexual needs in older adults remains a neglected area of clinical intervention, particularly so in long-term care settings. Because older adults in medical rehabilitation and long-term care beds present with significant frailties, and often significant neurocognitive disorders, it makes it difficult for occupational therapists and other staff to evaluate the capacity of an older adult resident to participate in sexual relationships. The current paper reviews the current literature on sexuality and aging, examines some of the clinical practices and guidelines regarding sexual expr

Biofilm-based infections in long-term care facilities.

The recent trend in the early admittance to long-term care facilities (LTCFs) of severely injured patients transferred from general hospitals has given a new dynamic to the incidence of healthcare-associated infections, including biofilm-based infections related to the implant of urinary and intravascular catheters, and the onset of pressure ulcers. Catheter-associated urinary tract infections lead in most of the surveys on LTCFs, approximately 80% of urinary tract infections in these settings being due to the short- or long-term insertion of a urinary catheter. Furthermore, the implantation o

Urinary tract infections in long-term care residents.

Urinary tract infection (UTI) is common in long-term care (LTC) residents; however, most infections are asymptomatic and do not require treatment. Differentiating asymptomatic from symptomatic UTI is challenging, because LTC residents typically have chronic genitourinary complaints, multiple comorbid illnesses, and communication barriers. Although consensus guidelines have been proposed to improve the accuracy of identifying symptomatic UTIs and minimize treatment of asymptomatic UTIs, diagnostic accuracy is not yet optimized. Strategies for prevention of UTI are unsatisfactory and require fur

Telehealth for wound management in long-term care.

Telehealth facilitates provision of healthcare services at distant sites using electronic communications. A pilot project utilizing telehealth was initiated in a long-term care community to assess the wounds of geriatric residents. Over a 1-year period, nine patients requiring a total of 21 visits were evaluated by an offsite University of Virginia Health System wound nurse practitioner via telecommunication. Traditionally, frail elders are seen in the local wound care clinic, necessitating often costly and physically strenuous transport. The technology in this program incorporated use of a vi

[Management of scabies in long term care facilities].

Regularly, cases of scabies are diagnosed. Old institutionalized persons with depressed immunity are particularly sensitive to this type of infestation. Scabies is an ectoparasitose related to Sarcoptes scabiei. Clinical picture can be highly variable depending of the type of scabies (typical scabies, profuse one or norvegian scabies). Even if the etiologic agent is the same, each type has its own characteristics and is different as far as the contagiosity is concerned. Diagnostic of scabies can be difficult due to atypical lesions. Delayed diagnostic, particularly in collectivities of old per

Urinary incontinence treatment preferences in long-term care.

OBJECTIVE: To elicit preferences for different urinary incontinence (UI) treatments in long-term care (LTC) from groups likely to serve as proxy decision makers for LTC residents. DESIGN: A descriptive, comparative study of preference for UI treatments of frail older adults, family members of nursing home (NH) residents, and LTC nursing staff. Surveys were mailed to families and self-administered by staff. Older adults were interviewed. SETTING: Four LTC facilities and two residential-care facilities in Los Angeles. PARTICIPANTS: Four hundred and three family members of incontinent NH resid

Long-term care in dementia: patients and caregivers.

General principles of managing chronic, age-associated diseases apply as much to Alzheimer's disease (AD) and other late-life dementing disorders as they do to congestive heart failure or osteoarthritis. Beyond efforts to maintain residual tissue or organ function, important physician roles include promoting general well-being and helping patients and their caregivers adjust to disease-related limitations. Physicians provide essential information to patients and their families about the disease, its social and legal ramifications, and community resources to facilitate care. Therefore, physicia

Ethical challenges in geriatrics and long-term care.

BACKGROUND: The Canadian population, like that of other western countries, is aging. OBJECTIVE: To review ethical issues related to clinical practice, research, and resource allocation in the geriatric and long-term care population. METHODS: Through the use of case histories and relevant literature review, we explore issues in geriatrics and long-term care. CONCLUSIONS: It is incumbent on health-care providers involved in geriatrics and long-term care to be sensitive to the ethical issues that surround decision-making in the later years of life. It is through this process that we can be hum

Play it safe in long-term care facilities.

Instead of caring for patients, nurses and other healthcare workers at long term facilities are increasingly becoming patients themselves. Over the past decade, the injury rate among nursing home workers jumped by 55%, with more than 200,000 of these workers injured annually. This article discusses the types of injuries suffered and how they can be prevented.

Catheter-associated bacteriuria in long-term care facilities.

Catheter-associated bacteriuria is the most common infection acquired in long-term care facilities. Complications include fever, acute pyelonephritis, bacteremia, catheter obstruction, urinary stones, chronic interstitial nephritis, renal failure, and death. The closed-catheter system has been the only innovation in this traditional method of care that has led to prevention of bacteriuria. Antimicrobial agents rarely are indicated to prevent or treat bacteriuria in long-term catheterized patients, except for those with symptomatic infection. Alternative devices are available and often may be p
