Ossification of the posterior longitudinal ligament of the spine (OPLL) is a common disease after the middle age. OPLL frequently causes serious neurological problems due to compression of the spinal cord and/or nerve roots. OPLL occurs in patients with monogenic metabolic diseases including rickets/osteomalacia and hypoparathyroidism; however most of OPLL is idiopathic and is considered as a multi-factorial (polygenic) disease influenced by genetic and environmental factors. Genomic studies for the genetic factors of OPLL have been conducted, mainly in Japan, including linkage and association

We present a unique variant of C1 fracture, which, at the best of our knowledge, has never been previously reported. This lesion consists in a lateral mass atlas fracture with a longitudinal separation of the transverse ligament that remains functionally preserved. We considered this lesion to be stable and treated the patient conservatively with good recovery. Even if many classifications of atlas fractures have been proposed, none of them ever described this uncommon variant. We debate about biomechanical aspects and therapeutic implication of the reported case.

OBJECT: Ossification of the posterior longitudinal ligament (OPLL) can result in significant myelopathy. Surgical treatment for OPLL has been extensively documented in the literature, but less data exist on conservative management of this condition.
METHODS: The authors conducted a systematic review to identify all reported cases of OPLL that were conservatively managed without surgery.
RESULTS: The review yielded 11 published studies reporting on a total of 480 patients (range per study 1-359 patients) over a mean follow-up period of 14.6 years (range 0.4-26 years). Of these 480 patients, 3

OBJECT: The authors assessed the clinical course in patients with a narrowed cervical spinal canal caused by ossification of the posterior longitudinal ligament (OPLL), but who have no or only mild myelopathy. Additionally, the authors analyzed the factors contributing to the development and aggravation of myelopathy in patients with OPLLinduced spinal canal stenosis.
METHODS: Between 1997 and 2004, the authors selected treatments for patients with cervical OPLL in whom the residual space available for the spinal cord was < or = 12 mm. Treatment decisions were based on the severity of myelopa

Ossification of the posterior longitudinal ligament in the cervical spine may be a cause of cervical myelopathy. This ossification has often been encountered in Japan, but only sporadically among the Caucasian races. It is therefore probable that racial factors are relevant to the pathology. During the past four years, due to the routine use of computerised axial tomography (CAT) in the pre-operative study of cervical myelopathy, we were able to show that in 13 cases stenosis of the vertebral canal was due to ossification of the posterior longitudinal ligament. This was visible in the standard

Ossification of the posterior longitudinal ligament (OPLL), which causes marked spinal cord compression, occurs more frequently in Japan than in other parts of the world. To investigate the mechanism of this ossification, we examined two cases of mixed type of OPLL inducing severe neurological signs due to spinal cord compression and three cases of segmental OPLL type without neurological signs. Ossification of the ligament begins at the upper end of posterior margin of the vertebral body and distends mainly downward along the posterior longitudinal ligament (PLL) and finally replaces the PLL

Ossification of the posterior longitudinal ligament (OPLL) is a progressive disorder characterized by abnormal ectopic bone formation along the posterior longitudinal ligament, often leading to spinal canal stenosis and cervical myelopathy. OPLL is increasingly recognized worldwide due to advancements in imaging technology. Surgical management remains the mainstay of treatment for symptomatic patients, but the optimal approach continues to be debated. This review aims to provide a practical workflow based on the current evidence on the pathophysiology, classification, diagnostic imaging, and t

Ossification of the posterior longitudinal ligament of the spine (OPLL) is an intractable disease leading to severe neurological deficits. Its etiology and pathogenesis are primarily unknown. The relationship between OPLL and comorbidities, especially type 2 diabetes (T2D) and high body mass index (BMI), has been the focus of attention; however, no trait has been proven to have a causal relationship. We conducted a meta-analysis of genome-wide association studies (GWASs) using 22,016 Japanese individuals and identified 14 significant loci, 8 of which were previously unreported. We then conduct

Ossification of posterior longitudinal ligament (OPLL) is a pathological process in which lamellar bone is deposited at the posterior longitudinal ligament and can lead to a limited range of cervical motion and spinal cord compression. A 64-year-old man presented with a 10-month history of worsening clumsiness in the hands and impaired gait, and he occasionally had a feeling of an electric shock in the limbs when the neck was flexed. Physical examination revealed atrophy of the intrinsic hand muscles, rapid reflexes in the lower extremities, and positive Hoffman sign and Babinski sign results.

Ossification of the posterior longitudinal ligament (OPLL) is a rare pathologic process of lamellar bone deposition that can result in spinal cord compression. While multiple genetic and environmental factors have been related to the development of OPLL, the pathophysiology remains poorly understood. Asymptomatic patients may be managed conservatively and patients with radiculopathy or myelopathy should be considered for surgical decompression. Multiple studies have demonstrated the morphology and size of the OPLL as well as the cervical alignment have significant implications for the appropri

The management of thoracic ossification of the posterior longitudinal ligament has been studied by many spinal surgeons. Indications for operative intervention include progressive radiculopathy, myelopathy, and neurological deterioration. The ideal surgery for decompression remains highly debatable as various methods of surgical treatment of ossification of the posterior longitudinal ligament have been devised. Although numerous modifications to the 3 main approaches have been identified (anterior, posterior, or lateral), the indication for each depends on the nature of compression, the morpho

The clinical material presented by the authors includes 24 observations of patients with ossification of the posterior longitudinal ligament. In all cases there was a cervical spine injury in the patients', past history. The diagnosis was confirmed roentgenologically and by the method of computer tomography. The treatment is surgical and consists in anterior decompression of the spinal cord. Recovery was observed in 10 patients (42 +/- 10%), partial regress of myelopathy in 9 patients (29 +/- 10%) and discontinuation (12 +/- 6%); there was no effect in 2 patients of progress of the disease in

An epidemiological study was carried out in connection with the multiphasic screening examination of 1,057 people (442 men and 615 women) in the village of Yachiho. The purpose of this study was to define the etiology of OALL (ossification of the anterior longitudinal ligament) and disc narrowing. The prevalence of disc narrowing increased with age, but OALL was not correlated with aging. OALL was found more frequently in men, but disc narrowing showed no difference between each sexes. The body height and weight-height index were higher in the OALL cases. The body height shrinkage by aging was

A case is presented of cervical myelopathy with acute posttraumatic onset, due to ossification of the posterior longitudinal ligament. This disease is rare outside Japan. The progressive ossification appears to be secondary to multiple ankylosing degenerative discopathies.

The cervical spine ligaments, including the anterior longitudinal ligament (ALL) and posterior longitudinal ligament (PLL), play a key role in maintaining spinal stability by limiting excessive movements. This study investigates how ageing affects the mechanical properties of these ligaments. We analysed 33 samples from 12 human cervical spines (15 ALL, 18 PLL), averaging data from the same donors for independent analysis, resulting in 18 final samples (8 ALL, 10 PLL). To explore age-related changes, we classified the samples into two groups-below and above 50 years old-aligning with the peak
