AIM: Sacral nerve stimulation has become a preferred method for the treatment of faecal incontinence in patients who fail conservative (non-operative) therapy. In previous small studies, sacral nerve stimulation has demonstrated improvement of faecal incontinence and quality of life in a majority of patients with low anterior resection syndrome. We evaluated the efficacy of sacral nerve stimulation in the treatment of low anterior resection syndrome using a recently developed and validated low anterior resection syndrome instrument to quantify symptoms.

OBJECTIVE: Endoscopic submucosal dissection (ESD) is increasingly preferred as an alternative to surgery and as an organ-preserving method for the treatment of rectal lesions. However, its impact on bowel function-particularly the development of low anterior resection syndrome (LARS)-remains poorly understood. This study aimed to evaluate the incidence of LARS following rectal ESD and to identify associated clinical and pathological factors.
METHODS: Data from 118 patients who underwent rectal ESD between January 2018 and December 2024 were retrospectively analyzed. Demographic characteristic

BACKGROUND: Functional morbidity remains an important survivorship concern after sphincter-preserving surgery for very low rectal cancer. We evaluated factors associated with major low anterior resection syndrome (LARS) and fecal incontinence after ileostomy reversal following hand-sewn intersphincteric resection (ISR).
METHODS: This retrospective cohort study included consecutive patients who underwent hand-sewn ISR with coloanal anastomosis and diverting ileostomy for low or ultralow rectal cancer at two tertiary centers between 2019 and 2024. Functional outcomes were assessed 12-15 months

BACKGROUND: Low anterior resection syndrome (LARS) is a series of bowel dysfunction symptoms, including altered bowel frequency, irregular bowel rhythms, fecal incontinence, and constipation. LARS occurs in 80% of patients undergoing sphincter-preserving surgery, affecting patients' quality of life along with social avoidance. Different measurements and treatments have been raised to deal with LARS, but no systematic standard has been developed.
OBJECTIVE AND METHODS: To promote the standardization of clinical trials and clinical management of LARS, this review summarizes the latest findings

Low anterior resection syndrome (LARS) is a collection of symptoms that can occur as a result of a low anterior resection for bowel cancer. Transanal irrigation (TAI) can be used to manage these symptoms. This article describes a retrospective audit of 15 patients who were using TAI to manage symptoms of LARS. The aim of the audit was to ascertain whether the use of TAI improved outcomes for these patients. The data suggest that TAI has reduced both the frequency of bowel movements and episodes of faecal incontinence. Those patients using higher volumes of water seem to have experienced more b

AIM: The low anterior resection syndrome (LARS) severely affects quality of life (QoL) after colorectal cancer surgery. There are no data about these complaints and the association with QoL in a reference population. The aim of this study was to assess LARS and the association with QoL in a reference population.
METHODS: Six hundred patients who visited the outpatient clinic because of general or trauma surgical indications were asked to participate in this study. They received an invitation letter containing three validated questionnaires to assess LARS (assessed with the LARS score) and bot

Low anterior resection syndrome (LARS) commonly develops after an anal sphincter-preserving operation (SPO). The etiology of LARS is not well understood, as the anatomical components and physiological function of normal defecation, which may be damaged during the SPO, are not well established. SPOs may damage components of the anal canal (such as the internal anal sphincter, longitudinal conjoint muscle, or hiatal ligament), either mechanically or via injury to the nerves that supply these organs. The function of the rectum is substantially impaired by resection of the rectum, division of the

BACKGROUND: Low anterior resection syndrome (LARS) is common after neoadjuvant chemoradiotherapy (nCRT) and sphincter-preserving surgery for rectal cancer and is associated with poor quality of life. However, reliable tools to identify patients at high risk remain limited. This study aims to develop and validate a prediction model for major LARS in patients with rectal cancer after nCRT and sphincter-preserving surgery.
PATIENTS AND METHODS: A total of 315 consecutive patients between 2019 and 2021 were retrospectively enrolled (training cohort: 213; independent validation cohort: 102). A dis

Low anterior resection syndrome (LARS) is a challenging complication following sphincter-preserving rectal surgery, characterized by bowel dysfunctions such as urgency, frequency, and incontinence. This case series investigates the potential role of lactose intolerance in exacerbating LARS symptoms. Three patients who developed LARS after rectal resections showed persistent symptoms despite conventional management with bulking agents, physiotherapy, and loperamide. However, all three experienced significant improvement in symptomatology, reflected by reductions in their LARS scores, after elim

BACKGROUND: Total intersphincteric resection (ISR) is the ultimate anus-preserving surgery for patients with ultra-low rectal cancer (ULRC), which can result in various degrees of anorectal dysfunction. Known as low anterior resection syndrome (LARS), it seriously affects the postoperative quality of life of patients. The aim of this study was to discuss the value of mesorectal reconstruction with pedicled greater omental transplantation (PGOT) to relieve LARS following total ISR in patients with ULRC, hoping to provide new ideas and strategies for the prevention and improvement of LARS.

BACKGROUND: As a series of bothersome bowel dysfunction symptoms, low anterior resection syndrome (LARS) has a high incidence after rectal cancer surgery, which grievously impairs health-related quality of life. There have been an increasing number of studies on biofeedback therapy (BFT) to recover intestinal function in patients following anus-preserving surgery. However, few systematic reviews or meta-analyses have been reported.
OBJECTIVE: The purpose of this systematic review with meta-analysis was to identify the short-term and long-term effects of BFT on subjective and objective indicat

PURPOSE: Serotonin (5-hydroxytryptamine [5-HT])3 receptor antagonists are effective for the treatment of diarrhea-predominant irritable bowel syndrome (IBS-D), in which exaggerated intestinal/colonic hypermotility is often observed. Recent studies have suggested that the motility disorder, especially spastic hypermotility, seen in the neorectum following sphincter-preserving operations for rectal cancer may be the basis of the postoperative defecatory malfunction seen in these patients. We investigated the efficacy of 5-HT3 receptor antagonists in patients suffering from severe low anterior re

BACKGROUND: The optimal intervention for managing low anterior resection syndrome (LARS) remains uncertain. This Bayesian network meta-analysis was conducted to compare and rank the effectiveness of various interventions on LARS.
METHODS: Randomized controlled trials (RCTs) addressing interventions for LARS were extracted from six electronic databases until September 2023. A network meta-analysis was performed using a Bayesian random-effects and consistency model. The results were presented as mean differences (MDs) with credible interval (CrI) or standardized mean differences (SMDs) with CrI

Many patients develop a variety of bowel dysfunction after sphincter preserving surgeries (SPS) for rectal cancer. The bowel dysfunction usually manifests in the form of low anterior resection syndrome (LARS), which has a negative impact on the patients' quality of life. This study reviewed the LARS after SPS, its mechanism, risk factors, diagnosis, prevention, and treatment based on previously published studies. Adequate history taking, physical examination of the patients, using validated questionnaires and other diagnostic tools are important for assessment of LARS severity. Treatment of LA

PURPOSE: Despite recent advances in multimodal treatment strategies, radical tumor resection remains a cornerstone in the management of patients with rectal cancer. However, a substantial proportion of patients develop postoperative functional impairment known as low anterior resection syndrome (LARS). Emerging evidence suggests that robotic-assisted surgery may offer superior functional outcomes compared to other approaches. Therefore, this study aimed to compare the incidence and severity of LARS after robotic and open surgery using a patient-reported outcome measure (PROM).
