Epidemiology and Management of Pediatric Fractures in Malawi.

BACKGROUND: Pediatric fractures are common in Malawi, and surgical care, when needed, remains inaccessible to many. Understanding which children in Malawi receive surgery or nonsurgical treatment would help set priorities for trauma system development. METHODS: We used multivariate logistic regression to evaluate associations between surgical treatment and age, sex, school enrollment, injury mechanism, fracture type, open fracture, referral status, hospital of presentation, delayed presentation (≥2 days), healthcare provider, and inpatient vs outpatient treatment. RESULTS: From 2016 to 2020,

Anticipatory decision-making for cholera in Malawi.

Climate change raises an old disease to a new level of public health threat. The causative agent, Vibrio cholerae, native to aquatic ecosystems, is influenced by climate and weather processes. The risk of cholera is elevated in vulnerable populations lacking access to safe water and sanitation infrastructure. Predictive intelligence, employing mathematical algorithms that integrate earth observations and heuristics derived from microbiological, sociological, and weather data, can provide anticipatory decision-making capabilities to reduce the burden of cholera and save human lives. An example

Clubfoot in Malawi: local theories of causation.

This paper examines local theories of the causation of clubfoot expressed by the guardians of children undergoing treatment at clinics run by the Malawi National Clubfoot Programme (MNCP). Core data was collected and analysed using qualitative methodologies of critical medical anthropology. Sixty detailed case studies were completed, each based on an extended open-ended interview with patient guardians. Five main theories of causation were put forward: God; the devil; witchcraft or curses; biological reasons; and inherited condition. Each was elaborated in a variety of ways. There is growing i

Self treatment of eye diseases in Malawi.

Self-treatment for eye diseases is very common in most developing countries yet there has been little investigation of such attitudes and practices. In many settings, people do not proceed beyond self-treatment and do not receive care from either traditional healers or Western eye care providers. Visual impairment and blindness can be the result. We conducted population-based survey of use of eye care services and self-treatment in two districts of Malawi. Adults were administered a detailed interview regarding their use of eye care services (Western and traditional as well as self-treatment)

Etiology of anemia in pregnancy in south Malawi.

BACKGROUND: Anemia in pregnancy is a major public health problem in developing countries. In sub-Saharan Africa, such anemia is generally accepted as resulting from nutritional deficiencies, particularly iron deficiency. OBJECTIVE: We comprehensively assessed the full spectrum of nutritional and nonnutritional factors associated with pregnancy anemia. DESIGN: Iron, folate, vitamin B-12, and vitamin A were measured in serum in a cross-sectional study of 150 pregnant women in Blantyre, Malawi. Bone marrow aspirates were evaluated, peripheral blood films were examined for malaria parasites, sto

Malaria infection in infancy in rural Malawi.

Malaria infection is thought to be relatively infrequent in infants less than 90 days of age in sub-Saharan Africa. In a rural area of Malawi with intense malaria transmission, we examined the occurrence of malaria infection during infancy and risk factors for parasitemia in the first three months of life in the cohort of infants delivered to women in the Mangochi Malaria Research Project. Among 3,915 liveborn singleton infants, 3,432 (87.7%) were seen at least once during infancy (first 12 months of life); of these, malaria blood smear results were available on 2,649 (77.2%). Overall, in a cr

The epidemiology of pityriasis versicolor in Malawi, Africa.

During a total population survey in 1988 and 1989 in Karonga district, northern Malawi, 4915/61735 (8.0%) people examined were found to have extensive pityriasis versicolor (PV). An additional 6085 people (9.9%) were diagnosed as having mild disease. The highest prevalence rates of extensive and mild PV were found among subjects aged 15-24 years. In this age group between 20% and 25% of people had extensive PV. Rates were generally higher among males than among females. PV was rarely found in prepubertal subjects.

The distribution of kwashiorkor in the southern region of Malawi.

Using existing nutritional survey data collected over an 11-month period during the recent drought, we sought to determine the distribution of kwashiorkor in the ten districts of the Southern Region of Malawi. Sampling and survey methods were identical and considerable efforts were made to ensure comparability. In 23 surveys, 25,824 children were weighed and measured and oedema was recorded. The district-adjusted prevalence of kwashiorkor was 18/1000. The northernmost districts of the Southern Region had rates five to ten times higher than the southernmost districts. Prevalence peaked at 18-23

Cryptococcal meningitis in Lilongwe and Blantyre, Malawi.

Infection with Human Immunodeficiency Virus is widespread in Malawi and cryptococcal meningitis is a common problem in those with AIDS. A review of microbiology laboratory records in Lilongwe and Blantyre between July 1991 and January 1993 identified 31 patients with cryptococcal meningitis. Diagnosis was based on a positive India ink stain of CSF and/or culture of Cryptococcus neoformans. There were 16 men (median age 38 years) and 15 women (median age 28 years) in the investigation. The median duration of symptoms was 2 weeks. The clinical presentation was varied, the most frequent features

Community perceptions of obstetric fistula in Malawi.

It is well-documented that obstetric fistula, a severe birth injury, is caused by a prolonged obstructed labour that has not been relieved on time. Lay people often understand causation differently. This study sought to explore the awareness and local meanings attached to obstetric fistula in the rural parts of Malawi. We conducted interviews with key informants and focus group discussions with community members in purposively selected communities in the central region of Malawi. We categorised data using Nvivo 10 and conducted a thematic analysis. Findings indicate that there is considerable

Incidence of induced abortion in Malawi, 2015.

BACKGROUND: In Malawi, abortion is legal only if performed to save a woman's life; other attempts to procure an abortion are punishable by 7-14 years imprisonment. Most induced abortions in Malawi are performed under unsafe conditions, contributing to Malawi's high maternal mortality ratio. Malawians are currently debating whether to provide additional exceptions under which an abortion may be legally obtained. An estimated 67,300 induced abortions occurred in Malawi in 2009 (equivalent to 23 abortions per 1,000 women aged 15-44), but changes since 2009, including dramatic increases in contrac

Sexual abuse in Malawi: patterns of disclosure.

Potential human immunodeficiency virus transmission makes prompt disclosure of child sexual abuse in Africa critical. The pattern of disclosure of 133 children presenting to the largest hospital in Malawi were analyzed. Eighty percent presented early enough for effective use of HIV postexposure prophylaxis. Seventy-five percent of children made a disclosure of child sexual abuse; 29% spontaneously and 47% after prompting. Disclosures were most commonly made to a parent, and age did not affect the pattern of disclosure. The number of children reporting child sexual abuse is increasing, possibly

Utilization of ultrasound in medical inpatients in Malawi.

BACKGROUND: Ultrasound utilization studies in the developing world are important to support appropriate use. METHODS: A prospective, cross-sectional study in the medical wards at Queen Elizabeth Central Hospital, Blantyre, Malawi, was performed which aimed to assess referrals, reports and usefulness of scans to develop local recommendations. The primary outcome of a 'useful scan' was based on scan results and utilization of the report. Indication, quality of requests and reports, and operator were documented. Recommendations for request and report writing were developed. RESULTS: During 28 A

Tackling Malawi's human resources crisis.

In sub-Saharan Africa, health systems are fragile and staffing is grossly inadequate to meet rising health needs. Despite growing international attention, donors have been reluctant to undertake the significant investments required to address the human resources problem comprehensively, given social and political sensitivities, and concerns regarding sustainability of interventions and risks of rising donor dependency. In Malawi, one of the poorest nations in Africa, declining human resource levels have fueled an accelerating collapse of public health services since the late 1990s. In an effor

Molecular epidemiology of cryptosporidiosis in children in Malawi.

Few studies have examined the molecular epidemiology of cryptosporidiosis in developing countries. In this study, DNA of 69 microscopy-positive human fecal samples collected from Malawi were examined by multilocus genetic analyses. From 43, 27 and 28 of the samples, the SSU rRNA, 70 kDa heat shock protein (HSP70) and 60 kDa glycoprotein (GP60) genes, respectively, were successfully PCR-amplified. Restriction analysis of the SSU PCR products showed that 41 of the 43 PCR-positive samples had C. hominis and 2 had C. parvum. Sequence analysis of the HSP70 and GP60 gene confirmed the species identi

The distribution and epidemiology of bovine trypanosomosis in Malawi.

A survey to update the distribution and clarify the epidemiology of bovine trypanosomosis in Malawi was conducted between 1995-97. Use was made of parasitological and serological (anti-trypanosomal antibody-detection Enzyme-Linked Immunosorbent Assay (ELISA) diagnostic methods. Trypanosomal infections were detected in cattle sampled adjacent to known tsetse foci. The distribution of cattle with anti-trypanosomal antibodies indicated that the distribution of bovine trypanosomosis was more widespread than the parasitological prevalence data suggested. This is attributed to the seasonal movement

Intensive nursing care of kwashiorkor in Malawi.

The case fatality rate for children with kwashiorkor in central hospitals in Malawi was 30.5% (275/901) in 1995. The purpose of this study was to determine whether improved case management with intensive nursing care could lower this case fatality rate. A total of 75 children admitted with kwashiorkor in Blantyre, Malawi, received intensive nursing care. This included nursing in individual clean beds with blankets, a nurse:child ratio of 1:3, supervised feedings every 2 h, a paediatrician with expertise in treating kwashiorkor always available for consultation, laboratory evaluation for system
