We present a woman with distinct mammahypertrophy and simultaneous catheter-associated axillary-subclavian venous thrombosis characterized only by chronically dilated collateral vein over the chest. Despite this massively dilated subcutaneous vein covering the chest on the affected side, bilateral reduction mammaplasty of modified Pitanguy method was successfully performed among protection of this collateral vein.

BACKGROUND: Breast reduction surgery is common in females; however, in males it is mainly due to gynecomastia. After weight reduction following obesity surgery, it is a problem in women, but also in some men.

A case of galactorrhea immediately following a reduction mammaplasty is presented. Prolactin levels were elevated postoperatively and could be reduced to normal by a prolactin inhibitor. No other disease or disorder could be found. Based on a review of the literature, it seems that various factors have to contribute to galactorrhea, including stress-related prolactin secretion, rebound phenomenon after discontinuation of birth control pills, and hypersensitivity to prolactin receptors. Their coincidental occurrence, resulting in abundant milk production after surgery, is discussed.

A modification of the original inferior flap reduction mammaplasty is presented using a dermal pedicle based on the inferior flap to support the nipple and areola complex. The technique is extremely versatile, and the breast contour is excellent. The nipple shows excellent projection and generally improved sensation over the preoperative state.

Galactorrhea complicating wound healing following reduction mammaplasty occurs rarely; only isolated cases have been reported in recent years. We report the case of a 25-year-old woman who presented with delayed healing and dehiscence of surgical wounds 3 weeks following vertical scar reduction mammaplasty. AIM: The aim of this study was to report the incidence of occult cancer in reduction mammaplasty (RM) specimens.
METHODS: A retrospective study of 314 women who underwent RM from February 1996 to August 2001.
RESULTS: Occult carcinomas were detected in three patients, in two of which the lesions were invasive. Atypical ductal or lobular hyperplasia was found in five cases and other carcinoma risk lesions in four patients. Other benign changes were identified in totally 133 patients.
CONCLUSION: Incidental carcinomas will occasionally be found in RM procedures.

Darier disease is an autosomal-dominant inherited genodermatosis. A woman is described who had Darier disease and hereditary spherocytosis whose inframammary Darier disease had dramatic and sustained improvement following reduction mammaplasty. The postoperative resolution of the dermatosis beneath the patient's breasts introduces the possibility that, when appropriately indicated, breast reduction surgery may have a potential role in the management of severe inframammary Darier disease.

A 26-year-old woman underwent breast augmentation with fat injections from trochanteric liposuction. After one year firm lumps developed in both breasts. Surgical treatment included excising the cysts through an inframammary approach. Volume replacement consisted of a silicone gel prosthesis inserted through an axillary retropectoral approach. In a second stage, a supra-areolar dermopexy was done to centralize the nipple-areola complex.

Thirty-five patients treated for 37 cases of breast cancer after augmentation mammaplasty were analyzed. All augmentations were performed with silicone gel prostheses at a mean age of 38 years (range 24 to 59 years).
