Mammography of the postreconstruction breast.

Experience with mammographic examination of the reconstructed breast after mastectomy is limited. Typical mammographic changes associated with breast reconstruction are described. When these changes are identified on mammography, they can prevent patients from having unnecessary biopsies and can alleviate patient anxiety. A retrospective review of all mammograms performed in our breast center was made to identify typical as well as unusual findings on the postreconstruction mammogram. The findings were correlated with the type of reconstruction and the physical examination findings. Mammograph

Mammography of the male breast.

Mammography of the male breast accounts for less than 1% of mammographic examinations done in breast imaging centers [1]. The purpose of this pictorial essay is to familiarize readers with diseases of the male breast and their mammographic manifestations that were observed during a 40-month period at the VA Medical Center, West Los Angeles. The most common presenting signs and symptoms that lead to a request for a mammograph in these men included an enlarging breast, a palpable lump, and breast tenderness.

Mammography in younger women.

Breast imaging is not frequently performed in women aged 35 years or younger for several reasons. Approximately half of the radiographic examinations of a breast symptom give normal results because the expected density of the breast in younger women may obscure abnormalities. Results from biopsies performed subsequent to positive mammogram findings confirm the predominance of benign lesions; cancers are rarely observed. After reviewing the main breast diseases that occur in the 20- to 35-year-old age group, we evaluated the accuracy of mammography in diagnosing malignancy, and attempted to sum

Mammography: a surgeon's experience.

Most reports advocating mammography have been written by radiologists rather than by clinicians who are frequently confronted by women with breast complaints. The value of mammography in managing patients with breast problems was studied by reviewing its role in 1026 breast consultations; there were 129 patients with cancer. Mammography was performed in 95 patients. The procedure hastened the diagnosis of one comedocarcinoma, and in another patient was suspicious 3 years before she presented with a locally advanced lesion. It missed six cancers. In seven patients it recognized clinically obvio

Mammography--use or abuse.

Excluding the question of cost, at the present time the benefit of mammography associated with physical examination over the age of 50 appears proven as an intervention measure. With recent advances in radiological techniques, women under 50 appear to be advantaged by the performance of mammography but the benefit through the use of screening has yet to be proven by clinical trial. Screening might be restricted to the high risk group of patients with previous or present breast disorders and a family history of breast cancer in close relatives. Promotion of BSE and screening needs continuing ed

Current status of mammography.

Great progress has been made in recent years in the diagnosis and treatment of breast cancer; however, breast cancer continues to be the most common and lethal cancer in women today. Early diagnosis is essential in order for treatment to be given before the tumor spreads beyond the breast. The radiation risks of mammography have been greatly reduced with the use of newer low-dose techniques, and the benefits of mammography have increased because of a better understanding of the natural history of breast cancer, as well as improved methods of treatment. Radiologists must continue to take an act

Breast cancer mammography patterns.

Mammography studies, from 1963 through 1972, on 5,918 women over age 30 years with 327 breast cancer on initial studies, were prospectively categorized on a scale of 1 to 4 of increasing amounts of fibroglandular tissue. Approximately 60% of the cancers occurred in classes 1 and 2 breasts, about one-third of the patients, while 40% of the cancer were in the remaining two-thirds, comprising classes 3 and 4. There were 54 cancers that developed in breast that previously were free of symptoms, clinical signs, and x-ray abnormality. Up to 36 months one cancer was found in class 1, while 26 cancers

Clinical outcome assessment in mammography: an audit of 7,506 screening and diagnostic mammography examinations.

PURPOSE: To perform an audit of our routine mammographic practice and to compare our results to performance benchmarks. MATERIALS AND METHODS: We analyzed the outcomes of 7,506 consecutive examinations performed in 1 year. Screening and diagnostic cases were evaluated separately and mammographic assessments were based on the Breast Imaging Reporting and Data System (BI-RADS) classification. RESULTS: In 6,858 (91%) screening and 648 (9%) diagnostic cases, outcomes varied substantially. The recall rate was 10.9%. Estimated sensitivity and specificity were similar (100% vs. 98% and 88% vs. 94%)

Comparison of written reports of mammography, sonography and magnetic resonance mammography for preoperative evaluation of breast lesions, with special emphasis on magnetic resonance mammography.

Patients with abnormal breast findings (n = 413) were examined by mammography, sonography and magnetic resonance (MR) mammography; 185 invasive cancers, 38 carcinoma in situ and 254 benign tumours were confirmed histologically. Sensitivity for mammography was 83.7%, for sonography it was 89.1% and for MR mammography it was 94.6% for invasive cancers. In 42 patients with multifocal invasive cancers, multifocality had been detected by mammography and sonography in 26.2%, and by MR mammography in 66.7%. In nine patients with multicentric cancers, detection rates were 55.5, 55.5 and 88.8%, respect

Risk-Based Screening Mammography for Women Aged <40: Outcomes From the National Mammography Database.

OBJECTIVE: There is insufficient large-scale evidence for screening mammography in women <40 years at elevated risk. This study compares risk-based screening of women aged 30 to 39 with risk factors versus women aged 40 to 49 without risk factors in the National Mammography Database (NMD). METHODS: This retrospective, HIPAA-compliant, institutional review board-exempt study analyzed data from 150 NMD mammography facilities in 31 states. Patients were stratified by 5-year age intervals, availability of prior mammograms, and specific risk factors for breast cancer: family history of breast canc

One more hurdle to increasing mammography screening: pubescent, adolescent, and prior mammography screening experiences.

BACKGROUND: Approximately $8.1 billion dollars is spent each year in the United States alone on the treatment of breast cancer. Survival rates are dependent on access to, and utilization of, early detection services. The primary reason for disparity in breast cancer mortality is the delay in time to diagnosis, resulting in poor prognosis. Despite ongoing research to understand barriers to mammography screening, recent studies report a decrease in mammography screening among all racial groups. METHODS: A qualitative approach was used to elicit information from 36 White non- Hispanic, African-A

Measuring preparedness for mammography in women with intellectual disabilities: a validation study of the Mammography Preparedness Measure.

BACKGROUND: Women with intellectual disabilities have similar breast cancer rates as the general population, but lower rates of regular mammography and higher breast cancer mortality rates. Although prior qualitative work demonstrates that women with intellectual disabilities face unique, disability-specific barriers to mammography, the present authors lack standardized, validated instruments for measuring knowledge of breast cancer screening in this population. In addition, much research related to adults with intellectual disabilities focuses on family or carer perspectives, rather than invo

Growing concern following compression mammography.

A patient without clinical symptoms had a mammogram in October 2008. The procedure caused intense persistent pain, swelling and development of a haematoma following mediolateral left breast compression. Three months later, a 9×11 cm mass developed within the same region. Core biopsies showed a necrotizing high-grade ductal carcinoma, with a high mitotic index. Owing to its extensive size, the patient began chemotherapy followed by trastuzumab and later radiotherapy to obtain clear margins for a subsequent mastectomy. The mastectomy in October 2009 revealed an inflammatory carcinoma, with 2 of

Mammography-oncogenecity at low doses.

Controversy exists regarding the biological effectiveness of low energy x-rays used for mammography breast screening. Recent radiobiology studies have provided compelling evidence that these low energy x-rays may be 4.42 +/- 2.02 times more effective in causing mutational damage than higher energy x-rays. These data include a study involving in vitro irradiation of a human cell line using a mammography x-ray source and a high energy source which matches the spectrum of radiation observed in survivors from the Hiroshima atomic bomb. Current radiation risk estimates rely heavily on data from the

[Workflow in digital screening mammography].

The overriding goal of an organized mammography screening program is to offer high level medical care to everyone at a justifiable cost. The following overview will demonstrate how both aspects of screening, quality and cost efficiency, are supported by a fully digital workflow. Digital mammography systems allow for a constant high image quality and repeat examinations due to overexposure or underexposure can be avoided. Dedicated mammography screening workstations with integration of image viewing and reporting enable efficient softcopy reading. Many aspects of the screening workflow, such as
