Severe obstructive sleep apnea (OSA) is linked to adverse cardiovascular outcomes. While continuous positive airway pressure (CPAP) is the standard treatment, mandibular advancement devices (MADs) offer an alternative. This substudy of a randomized trial compared the effectiveness of MADs versus CPAP on 24-h ambulatory blood pressure (BP), sleep-related quality of life, myocardial remodeling, ambulatory heart rhythm, and biomarkers in severe OSA. A total of 144 participants were randomized to MAD (n = 73) or CPAP (n = 71) for 12 mo. Median nightly usage was 5.4 (2.9-6.5) h for the MAD group (≥

INTRODUCTION: Mandibular advancement (MA) has emerged over the last decade as an alternative solution to nasal continuous airway pressure (nCPAP) for the treatment of obstructive sleep apnea syndrome (OSAS).
OBSERVATION: We report the case of a patient with history of chronic atrial fibrillation and moderate supine-dependent OSAS in whom central sleep apneas developed during treatment by a bi-bloc MA device. Central apneas increased with the level of MA and preferentially occurred in the supine position. We hypothesized that mouth opening under excessive mandibular advancement in supine posit

The cephalometric analysis of two groups: 30 sleep apnoea patients, and 30 age- and sex-matched control patients, established predictors for the orthopaedic mandibular advancement by means of the Esmarch device (ED) in treating obstructive sleep apnoea (OSA). Polysomnographic sleep and ventilation data gathered for each patient with and without mandibular advancement by means of the ED were compared. Cephalometric data and treatment efficacy for each patient were submitted to a regression analysis. Maximal efficacy is predicted by: 1. The combination of an orthognathic to prognathic maxilla (S

Ten of 12 adolescents treated with surgical mandibular advancement showed postsurgical mandibular growth, as indicated by an increase in the distance from condylion to pogonion. In all cases, the growth was expressed vertically relative to the cranial base, so that the chin did not come forward. None of the patients had significant increments of anterior maxillary growth postsurgically. Several patients had vertical maxillary growth, which was compensated by vertical mandibular growth, so that the anteroposterior position of the chin was maintained. Forward growth of the maxilla is minimal aft

This study examines the short-term stability of the mandible following mandibular advancement surgery in which skeletal suspension wires were used in addition to dental maxillomandibular fixation. Twenty adults underwent sagittal ramus osteotomies. No concomitant surgical procedures were performed. Maxillomandibular fixation consisted of wiring between the upper and lower orthodontic brackets and circummandibular wires connected to the piriform aperture or anterior nasal spine wires for eight weeks. Cephalograms were analyzed during this period to evaluate skeletal stability. A statistically i

BACKGROUND: The efficacy of mandibular advancement devices (MAD) and maxillomandibular advancement (MMA) in improving upper airway (UA) patency has been described as being comparable to continuous positive airway pressure (CPAP) outcomes. However, no previous study has compared MAD and MMA treatment outcomes for the upper airway enlargement. This study aimed to evaluate three-dimensionally the UA changes and mandibular rotation in patients after MAD compared to MMA.
METHODS: The sample consisted of 17 patients with treated with MAD and 17 patients treated with MMA matched by weight, height, b

Drug-induced sleep endoscopy (DISE), performed in the operating room, improves airway obstruction phenotyping in obstructive sleep apnea syndrome (OSAS). It pharmacologically approximates human sleep and allows dynamic maneuvers, notably including mandibular advancement, the results of which are predictive of those with a mandibular advancement device (MAD): when obstruction is resolved by mandibular advancement under DISE, a MAD may be indicated. However, individual maximal mandibular advancement in the waking state has to be taken into account, and the MAD should not exceed 75% of this value

The oromandibular limb hypogenesis syndrome is a group of anomalies affecting the mandible, tongue, and maxilla with or without reductive limb anomalies. It was first described by Hanhart in 1950. In severe syndromic cases of mandibular hypoplasia, a number of techniques have been described for mandibular advancement including sagittal split osteotomies, segmental osteotomies, or distraction osteogenesis just to name a few. A 25-year-old male patient presented to our clinic with symptoms including difficulty in speech and eating, disability in opening the mouth, together with hand and foot abn

Mandibular advancement devices (MADs) represent the main non-continuous positive airway pressure (non-CPAP) therapy for patients with obstructive sleep apnoea (OSA). The aim of the European Respiratory Society Task Force was to review the evidence in favour of MAD therapy. Effects of tongue-retaining devices are not included in this report. Custom-made MADs reduce apnoea/hypopnoea index (AHI) and daytime sleepiness compared with placebo devices. CPAP more effectively diminishes AHI, while increasing data suggest fairly similar outcomes in relation to symptoms and cardiovascular health from the

This randomized placebo-controlled cross-over trial assessed the effectiveness of a mandibular advancement appliance (MAA) in managing obstructive sleep apnoea (OSA). Twenty-one adults, with confirmed OSA, were provided with a maxillary placebo appliance and a MAA for 4-6 weeks each, in a randomized order. Questionnaires at baseline and after each appliance assessed bed-partners' reports of snoring severity (loudness and number of nights per week), and patients' daytime sleepiness (Epworth Sleepiness Score, ESS). The Apnoea Hypopnoea Index (AHI) and Oxygen Desaturation Index (ODI) were measure

Treatment options for obstructive sleep apnea (OSA) may involve potential side effects or discomfort; nasal continuous positive airway pressure (CPAP) may not be tolerated by 25% of patients. We therefore sought to determine the efficacy of mandibular advancement as a treatment for OSA, and to investigate whether clinical and radiographic parameters can predict the response to this treatment. Sixteen male and 3 female subjects with documented OSA who had failed or been unable to tolerate nasal CPAP underwent baseline polysomnography and cephalometry, and were then fitted with a removable Herbs
