Mandibular reconstruction with free vascularized fibular flap.

Although various sources of vascularized bone/composite tissue have been used for mandibular reconstruction, the vascularized fibular flap has been reported to have many advantages over the others and to be the most suitable for bridging a long-span mandibular defect. In this paper, positive experience of 14 consecutive cases in which a free vascularized fibular graft with or without a skin paddle has been used to reconstruct a long mandibular defect is reported. Half of the cases were primarily reconstructed, the rest secondarily. The average length of the fibula graft harvested was 16.7 cm,

A "no-touch-technique" in mandibular reconstruction with reconstruction plate and free flap transfer.

OBJECTIVE: Mandibular reconstruction with a mandibular reconstruction plate (MRP) and free flap transfer can involve serious plate-related complications. The aim of our study is to present our new "no-touch-technique" which keeps an MRP not contaminated to saliva during the entire operation for the reduction of plate-related complications. PATIENTS AND METHODS: Data were retrospectively collected on 29 patients who had undergone segmental mandibulectomy for head and neck tumor and mandibular reconstruction with an MRP and free flap transfer from 2004 to 2013; 12 patients were reconstructed wi

Transverse Mandibular Distraction Technique for Complex Craniofacial Reconstruction.

A very important method in the management of maxillomandibular discrepancies with pronounced hypoplasia of the upper or lower jaw is distraction osteogenesis. With distraction osteogenesis, it is possible to correct functional abnormalities, improve problems related to airway obstruction, and enhance overall soft tissue harmony in ways that would otherwise not be possible with traditional osteotomy and fixation techniques, particularly in the growing skeleton. For patients with severe transverse mandibular discrepancies, distraction osteogenesis is a viable option for correcting the mandibular

[Accuracy Verification of Robot-assisted Mandibular Reconstruction Surgery].

Mandible is an important bone of the head and neck. Mandibular defects not only affect patient's face, but also impede patient's daily functions, such as chewing, speech, and so on. Fibular transplantation for mandibular reconstruction is the common method, which requests high accuracy of bone positioning and posture adjustment. Therefore, a robotic system for mandibular reconstruction surgery with fibula flaps was designed to assist surgeons to hold and locate bones, and the model comparison experiments were conducted. The results showed that the robotic system can assist surgeons for mandibu

Evaluation of Temporomandibular Joint Movement After Mandibular Reconstruction.

Mandibular head dislocation and problems with mouth opening may develop after mandibular reconstruction. The authors investigated dislocation of the mandibular head and amount of protrusive sliding (excursion) and their effect on mouth opening. The authors divided 55 mandibular reconstruction patients into 3 groups on the basis of the extent of masticatory muscle and mandibular resection and investigated mandibular head dislocation. On the other hand, the authors focused on mandibular head protrusive excursion as a function of a reconstructed mandible. Protrusive excursion was measured by plai

A-O plate mandibular reconstruction: a complication critique.

Immediate A-O plate mandibular reconstruction was used in 31 patients, following the excision of malignant disease. This five-year retrospective study shows that 48% (15/31) of the patients are living (one with recurrence), with a mean follow-up of 30 months (range nine to 60 months). The mean survival of the deceased patients was 11 months (range two weeks to 46 months). Postoperative complications are evaluated with respect to the mandibular defect, radiation status, dentition, adjuvant onlay bone grafting and T stage. Briefly, 11/31 patients (36%) enjoyed an uneventful recovery, 9/31 (29%)

Digital mandibular reconstruction design guided by condylar position.

Mandibular defects following tumor resection can severely affect facial form, occlusal stability, and temporomandibular joint (TMJ) function. We developed a condyle-oriented digital workflow designed to optimize postoperative condylar position and improve functional reconstruction. Preoperative CT data were used to generate mirrored mandibular models based on the ideal condylar relationship, followed by fibular segment planning and the creation of a full set of patient-specific guides incorporating a shared screw-hole strategy for precise intraoperative alignment. Seventeen patients underwent

Factors associated with plate fractures in mandibular reconstruction.

PURPOSE OF REVIEW: Mandibular plate fracture (MPF) is a serious complication of mandibular reconstruction, leading to postoperative morbidity and potential revision surgeries. Recently, there has been an increased utilization of virtual surgical planning (VSP) with patient-specific reconstruction plates to decrease operative time and improve postoperative outcomes. It is critical for surgeons performing mandibular reconstruction to have a comprehensive understanding of the contributing factors and strategies for minimizing the likelihood of MPF. RECENT FINDINGS: This review examines the patie

[Individual combined endoprosthesis for reconstruction of mandibular defects].

Substitution of mandibular defects resulting from injuries and inflammatory bone diseases as well as surgical interventions for tumors is an important medical and scientific problem. The article presents the results of surgical treatment of 8 patients with the use of combined endoprosthesis of the mandible consisting of individual titanium metal structure and titanium silk. This solution provides the possibility for reliable fixation of the soft tissue structures of the lower face region by suturing, including facial and masticatory muscles, improving the biological integration of the endopros

Iliac flap versus fibula flap in mandibular reconstruction.

The mandible is one of the important parts of the body in terms of facial appearance and function. Vascularized free bone grafts have become the criterion standard in mandible reconstruction. With flap survival rates not too much distant to 100%, the focus is now on function and aesthetics. With the various free-flap options now available, reconstruction of the mandibular defect has achieved significant improvement in both functional and aesthetic results. However, few studies have compared the results of these options. The fibula is defined as the most popular flap in mandibular reconstructio

Microvascular reconstruction of mandibular defects in paediatric patients.

BACKGROUND: During mandibular reconstruction in paediatric patients special consideration must be given to the issues related to growth when restoration of the mandibular and maxillary relationship is essential for correct development of the facial skeleton, a dynamic process whereby mandibular and maxillary growth are strongly correlated. Wide resections, irradiated fields, infection at the recipient site, and scars are strong indications for using revascularized bone-containing free flaps. METHODS: Five patients were reviewed, with a follow-up ranging from 5 to 11 years. Measurements were p

Microvascular transfer of burned tissue for mandibular reconstruction.

We present a case of mandibular reconstruction using a free-vascularized fibula osteocutaneous flap after invasive infection of the mandible with mucormycosis. Unique to this case is the skin paddle that previously sustained full-thickness burn injury treated with cultured skin substitute (CSS). A 10-year-old boy sustained a 96% total body surface area burn. CSS was used to obtain wound closure. However, he developed invasive mucormycosis of the mandible requiring resection. The best option for mandibular reconstruction was the use of a free-vascularized fibular osteocutaneous flap with CSS as

Mandibular microsurgical reconstruction in patients with hemifacial microsomia.

BACKGROUND: Although hemifacial microsomia is a relatively common craniofacial malformation, there is some debate regarding the ideal treatment of severe mandibular hypoplasia. Traditionally, patients with severe mandibular deficits have been treated with iliac or costochondral bone grafts followed by distraction osteogenesis, with mixed results. The authors present their experience with the use of the fibula osteocutaneous free flap for mandibular reconstruction in severe hemifacial microsomia patients. METHODS: From 1999 to 2006, 10 patients aged 3 to 10 years (mean, 7.2 years) underwent 10
