Development of a progressive audiologic tinnitus management program for Veterans with tinnitus.

Tinnitus is the most prevalent service-connected disability awarded to Veterans. However, clinical protocols for management of tinnitus have been inconsistent across Department of Veterans Affairs (VA) medical centers. A study was funded to develop and pilot test a protocol to provide tinnitus services consistently across VA audiology clinics. Drawing on a series of prior VA and external research projects, a clinical model was formulated, supporting materials in multimedia were developed, and a pilot study was conducted. Five hierarchical levels of care were defined and labeled the Progressive

Contemporary Unplanned Readmission Trends Following Management of Type B Aortic Dissection.

PURPOSE: Large studies have demonstrated improved survival outcomes with thoracic endovascular aortic repair (TEVAR) at two and five years compared to medical therapy; however, early TEVAR for acute type B aortic dissection (TBAD) remains controversial. We aimed to evaluate trends and clinical predictors of hospital readmissions in patients undergoing medical management and TEVAR for acute TBADs. MATERIALS AND METHODS: The Nationwide Readmissions Database was queried for all 30-day and 90-day index readmissions (30D-IR and 90D-IR, respectively) after a diagnosis of a TBAD from January 2012 to

Application of tele-podiatry in diabetic foot management: A series of illustrative cases.

BACKGROUND AND AIMS: Telemedicine had been proposed as a tool to manage diabetes, but its role in management of diabetic foot ulcer is still evolving. The COVID-19 pandemic and related social restrictions have necessitated the use of telemedicine in the management of diabetic foot disease (tele-podiatry), particularly of patients classified as low-risk. MATERIALS AND METHODS: We present a report of three cases of varied diabetic foot problems assessed during the present pandemic using different forms of telemedicine for triaging, management of low-risk cases and for follow-up. RESULTS: Tele-

Background: The rapid development of multiple myeloma (MM) management underscores the value of real-world data. In our study we examined 509 adult MM patients treated with immunochemotherapy (ICT) with/without stem cell transplantation (SCT) from 2013 to 2019 in the Hospital District of Helsinki and Uusimaa, Finland. Materials & methods: Our study was based on computational analyses of data integrated into the hospital data lake. Results: After 2017, treatment pattern diversity increased with improved access to novel treatments. 5-year survivals were 74.4% (95% CI: 65.5-84.5) in SCT-eligible a

Timing of Cholecystectomy in Acute Cholecystitis.

BACKGROUND: Cholecystitis is an inflammation of the gallbladder that most commonly occurs as a result of obstruction of the cystic duct by gallstones. The current standard of treatment for acute cholecystitis is cholecystectomy. OBJECTIVE: Our goal was to discuss the benefits of and compare early laparoscopic cholecystectomy and delayed laparoscopic cholecystectomy in the treatment of acute cholecystitis. MATERIALS AND METHODS: A Medline literature search was performed dating from January 1982 to July 2015. We limited the search to human studies written in English and using the keywords "Acu

Curriculum evolution and documentary evidence of competency alignment in a hospital-embedded undergraduate health services and management programme in China: a longitudinal case study.

BACKGROUND: Undergraduate Health Services and Management (HSM) programmes are expected to integrate management, medicine, public health, digital capability and workplace learning. Hospital-embedded delivery may provide access to authentic organizational settings and interdisciplinary expertise, but distributed teaching can fragment curriculum ownership, competency progression and assessment. This study examined longitudinal evolution and documentary evidence of competency alignment in a hospital-embedded undergraduate HSM programme in China. METHODS: A longitudinal document analysis and compe

Surgical management and outcomes of endocarditis in adolescents and adults with congenital heart disease.

BACKGROUND: Adolescents and adults with CHD have increased risk for IE because of anatomic anomalies and burden of intracardiac prosthetic material. There are significant challenges in determining primary surgical or medical management along with limited guidelines to determine management. METHODS: A retrospective review was performed at both the University of Colorado Hospital and Children's Hospital Colorado of patients 12 years of age and older with CHD who were treated for IE between 2009 and 2019. RESULTS: Of 138 cases, the most common CHD lesions were bicuspid aortic valve (30%) and te

Developing a capability assessment tool for physiotherapists managing patients with blunt chest trauma.

PURPOSE: The aim was to develop a capability assessment tool (CAT) for physiotherapists managing patients with blunt chest trauma. MATERIALS AND METHODS: A nominal group technique incorporating a cross-sectional survey was used. A panel of 10 international physiotherapist experts developed and tested the content validity of the domains and items to be included in the tool. For each item, an item-level content validity index (I-CVI) was calculated based on the panel members' rating. Panel members rated each item for relevance to physiotherapy management of patients with blunt chest trauma. RE

Mortality rates for hip fracture patients managed surgically and conservatively in a dedicated unit in Singapore.

INTRODUCTION: At our hospital, an unusually high proportion of patients and families opted for conservative management of hip fractures. This study aimed to compare the mortality rates of patients with hip fractures treated conservatively to that of operatively managed patients in a dedicated hip fracture unit. MATERIALS AND METHODS: Retrospective analysis was done for patients who were treated for hip fractures between January 2015 and October 2017 in a Hip Fracture Unit at a tertiary hospital. Patients were managed non-operatively or surgically after discussion with the multi-disciplinary t

[Acute pain therapy in German hospitals as competitive factor. Do competition, ownership and case severity influence the practice of acute pain therapy?].

BACKGROUND: Due to the implementation of the diagnosis-related groups (DRG) system, the competitive pressure on German hospitals increased. In this context it has been shown that acute pain management offers economic benefits for hospitals. The aim of this study was to analyze the impact of the competitive situation, the ownership and the economic resources required on structures and processes for acute pain management. MATERIAL AND METHODS: A standardized questionnaire on structures and processes of acute pain management was mailed to the 885 directors of German departments of anesthesiology
