Maxillary sinus barotrauma with infraorbital nerve paraesthesia after breath-hold diving.

Barosinusitis, or sinus barotrauma, is a sinonasal injury and/or inflammation that results when the aerated spaces of the nose and sinuses are exposed to an uncompensated change in ambient pressure. We describe a 19-year-old male diver who presented to our clinic on the fourth day following a breath-hold diving session. During descent on a constant weight monofin dive at the South Cyprus World Championship he began to experience symptoms due to the inability to equalise the pressure, particularly in the Eustachian tubes and middle ear cavities. He felt pain and pressure in the upper left half

Resection of the Posterior, Middle, and Anterior Superior Alveolar Nerves and Infraorbital Nerve Neurolysis for Refractory Maxillary Pain.

Trigeminal injury can cause intractable facial pain. However, surgical approaches to the superior alveolar nerves have not been widely described. We report resection of the anterior superior alveolar nerve (ASAN), middle superior alveolar nerve (MSAN), and posterior superior alveolar nerve (PSAN) in a patient with refractory facial pain and outline an algorithmic approach to the treatment of trigeminal nerve injury. A 56-year-old woman presented with a 3-year history of refractory facial pain in the distribution of the right superior alveolar nerves after dental trauma. As a comorbidity, centr

Utility of Regional Maxillary Nerve Blocks in Improving Cleft Palate Postoperative Outcomes.

BACKGROUND: Palatoplasty procedures used to repair cleft palates are commonly associated with limiting postoperative pain. Regional anesthetic blocks have been utilized to improve pain outcomes and decrease opioid intake, yet additional data is needed to fully explore its utility in this setting. OBJECTIVE: To explore whether ultrasound-guided suprazygomatic maxillary blocks (SMB) improve postoperative pain, postoperative opioid use, time to oral feeding, and length of stay compared with a palatal field block in cleft palate repair. METHODS: In this retrospective chart review, 47 patients ag

Ultrasound-guided maxillary nerve block: an anatomical study using the suprazygomatic approach.

PURPOSE: Although a maxillary nerve (MN) block reportedly provides satisfactory analgesia for midface surgery and chronic maxillofacial pain syndromes, a safe and reliable MN block technique has not been reported. The goal of this anatomical study was to quantify the various angles and depth of the block needle, as well as to evaluate the impact of volume on the extent of injectate spread that might influence anesthetic coverage and block-related complications. METHODS: Following an ultrasound-guided suprazygomatic MN block with dye injection, a dissection was performed in the pterygopalatine

Surgical risk factors and maxillary nerve function after Le Fort I osteotomy.

PURPOSE: Data on intraoperative risk factors for long-term postoperative complications after Le Fort I osteotomy (LFO) are limited. The aim of this study was to describe prospectively the overall postoperative changes in maxillary nerve function after LFO, and to correlate these changes with a number of possible intraoperative risk factors. PATIENTS AND METHODS: Twelve men and 13 women (mean +/- standard deviation: aged 25+/-10 years) participated in 4 sessions: 1 before LFO (baseline), and the rest at 3, 6, and 12 months after LFO. At each session, somatosensory sensitivity was assessed for

[Varicella zoster virus infection involving the maxillary branch of the trigeminal nerve].

Herpes zoster is an infection caused by reactivation of the latent varicella virus in the sensory ganglia. The mechanisms responsible for Varicella zoster virus (VZV) reactivation are poorly understood. Yet, it is believed that decreased cellular immunity can be a trigger for it's reactivation. The occurrence of herpes zoster in young people may point to an underlying immunodeficiency. Therefore, the possibility of concomitant HIV infection must be eliminated. Herpes zoster manifests as a vesicular rash along a sensory dermatome, usually preceded by pain or paresthesia of the involved cutaneou

[A case of intracranial neurinoma arising from the maxillary branch of the trigeminal nerve].

The intracranial portion of the trigeminal branches is rarely involved in neurinomas. The clinical characteristics including symptoms, neuroradiological findings and results of surgical excision of the previously reported 11 neurinomas and the present case arising from the intracranial trigeminal branches are reviewed. A 59-year-old female patient complained of paresthesia and hypesthesia in the infraorbital region with no other neurological abnormalities. Neuroradiological examinations revealed a round cyst-like tumor located at the middle fossa with enlargement of the foramen rotundum and pt

Use of computed tomography for maxillary nerve block in the treatment of trigeminal neuralgia.

BACKGROUND AND OBJECTIVES: Maxillary nerve block has traditionally been performed by using external anatomic landmarks. However, the classic approach to the nerve may be confounded because of anatomic variability. We describe a technique for the block using the suprazygomatic route guided by computed tomography (CT). CASE REPORT: A 90-year-old woman had a 30-year history of episodic pain in her right maxillary region. We attempted to block the maxillary nerve with classic technique, but we could not identify the nerve by eliciting paresthesia. In addition, bleeding was noted after repeated at

Sensory action potentials of the maxillary nerve: a methodologic study with clinical implications.

PURPOSE: Recently, recording of sensory nerve action potentials (SNAPs) of the inferior alveolar nerve (IAN) was described and is used as a diagnostic test of traumatic neuropathic trigeminal disorders. The technique is limited to IAN damage; therefore, we adapted the technique to the maxillary nerve, which is also frequently injured by either trauma or orthognathic surgery. PATIENTS AND METHODS: Fourteen healthy volunteers participated in this methodologic study in which the infraorbital nerve (ION) was stimulated with 2 needle electrodes. The SNAPs were recorded from the maxillary nerve wit

Orbital apex syndrome associated with cranial nerve V neuritis complicating bacterial maxillary sinusitis.

Maxillary sinusitis is a common pathology of the paranasal sinuses, most frequently caused by bacterial infections. Although typically benign, it can occasionally lead to severe complications when the infectious process extends beyond the sinus cavity. One such rare but serious complication is orbital apex syndrome (OAS), also known as Jacod syndrome, characterized by dysfunction of multiple cranial nerves at the orbital apex. OAS may result from various etiologies, including infectious, traumatic, and neoplastic causes. Among infectious origins, bacterial maxillary sinusitis is an uncommon ye

Infraorbital Nerve Located Medially to Postoperative Maxillary Cysts: A Risk of Endonasal Surgery.

BACKGROUND/AIMS: This study aimed to examine variations in the location of the infraorbital nerve relative to postoperative maxillary cysts to assess the potential risk of nerve injury during endonasal marsupialization. METHODS: Coronal computed tomography images of 130 patients (162 sides) with postoperative maxillary cysts who visited our clinic between 2003 and 2014 were reviewed from the viewpoint of the anatomical relationship between the infraorbital nerves and cysts. RESULTS: The proportions of the six locations were as follows: upside 45.1% (n = 73), separate 13.0% (n = 21), medial 5

Maxillary herpes zoster with nasociliary nerve involvement and Klebsiella superinfection: A rare occurrence.

Herpes zoster is an acute cutaneous viral disease resulting from reactivation of dormant varicella-zoster virus. The maxillary nerve is the least frequently affected branch of the trigeminal nerve. Rarely, cutaneous lesions can be secondarily infected with Klebsiella species. This report discusses a case of maxillary zoster with nasociliary nerve involvement and Klebsiella superinfection.

Research Progress on Radiofrequency Treatment of Primary Trigeminal Neuralgia in the V2 Maxillary Nerve.

Primary trigeminal neuralgia (TN) involving the V2 maxillary nerve presents unique therapeutic challenges. Traditional radiofrequency techniques at the semilunar ganglion often lack selectivity and carry risks of injuring adjacent nerves. The recently refined CT-guided foramen ovale puncture radiofrequency thermocoagulation technique has significantly improved outcomes for V2 TN. This narrative review systematically explores the evolution of radiofrequency treatment for V2 TN, with a particular focus on comparing the efficacy and safety of different puncture pathways. We emphasize the superior

Detailed evaluation of the risk of infraorbital nerve injury in postoperative maxillary cyst surgery.

OBJECTIVE: Postoperative maxillary cyst (POMC) develops as a delayed complication after radical surgery. The infraorbital nerve (ION) is a terminal branch of the trigeminal nerve and is one of the low-frequency collateral injuries that may occur in maxillary sinus surgery. The risk of ION injury may increase during POMC surgery due to anatomical variations: however, this has not yet been examined in detail. Therefore, we herein investigated variations in the ION in POMC cases with a focus on its relationship with the aperture site of the cyst, to clarify the risk of injury. METHODS: A multifa
