Between September 25, 1987, and December 31, 1992, 75 patients (53 men, 22 women; average age, 52 years) underwent the maze procedure for the treatment of atrial fibrillation. Six patients had undergone a previous cardiac operation and 28% underwent concomitant cardiac procedures in addition to the maze procedure. One patient (1.3%) died 10 days after undergoing a combined maze procedure and Morrow procedure for the management of chronic atrial fibrillation and hypertrophic obstructive cardiomyopathy. Postoperative atrial pacemakers were required in 40%: 26% for preoperative sick sinus syndrom

The maze procedure was performed simultaneously with mitral valve replacement for atrial fibrillation and mitral stenosis in a 66-year-old female. On the first postoperative day, the cardiac status deteriorated suddenly due to a very rapid heartbeat which was later diagnosed as a 1:1 AV conduction of atrial flutter. The atrial flutter responded to an injection of verapamil and changed to a 2:1 conduction. Although atrial flutter had continued until the thirteenth postoperative day, the heart achieved sinus rhythm on that day. Since then it has continued without any antiarrhythmic drugs except

The Cox-Maze procedure is the cornerstone of surgical rhythm-control therapy for atrial fibrillation (AF). Developed to interrupt macro-reentrant conduction while preserving physiological atrial activation. It has evolved from the original cut-and-sew Maze I-III Operations, based on earlier strategies like a corridor surgery to today's energy-based Cox-Maze IV (CM-IV) and minimal invasive operations. Contemporary surgical ablation can be performed concomitantly with valve or coronary surgery, as a stand-alone operation, or via minimally invasive or hybrid strategies integrating electrophysiolo

A 68-year-old hypertensive diabetic woman with chronic atrial fibrillation presented with progressive congestive symptomatology. She was diagnosed with severe aortic stenosis, moderate mitral regurgitation, and critical right coronary artery stenosis. In addition to coronary revascularization and bioprosthetic aortic valve replacement, she underwent a mitral valve repair and a complete cryoMaze procedure through a transaortic approach. This technique obviates a separate left atriotomy for the mitral repair and Maze procedure. It affords excellent exposure, while reducing cross clamp and cardio

BACKGROUND: Patients undergoing mitral valve surgery with atrial fibrillation commonly have a concomitant Cox-maze IV procedure (CMP-IV). Secondary tricuspid regurgitation (TR) is frequently present. This study evaluated the impact of preoperative TR on outcomes after the CMP-IV.
METHODS: Between January 2002 and November 2023, 407 patients underwent concomitant mitral valve surgery with CMP-IV. Patients were grouped by preoperative TR severity: moderate or severe TR (TR group) and less than moderate (non-TR group). Propensity score matching was performed to balance baseline characteristics.

BACKGROUND: An association between mitral valve disease and atrial fibrillation is common. Modifications of energy sources have simplified surgical ablation. Left atrial size reduction should improve outcomes of the maze operation.
METHODS: Between 2004 and 2011, 236 patients with permanent atrial fibrillation and mitral valve disease underwent mitral valve surgery and modified biatrial radiofrequency ablation. The study evaluated cardiac rhythm, atrial size, atrial contractility, and survival.
RESULTS: The mitral valve was repaired in 88 (37.3%) patients and replaced in 148 (62.7%). The lef

INTRODUCTION: Atrial tachyarrhythmias, particularly atrial flutter and fibrillation, are commonly associated with congenital heart disease and are a major cause of morbidity and mortality. The Cox-maze procedure, introduced by Dr. James Cox in 1987, is effective at controlling atrial fibrillation in structurally normal hearts. Though the Cox-maze procedure has been used for atrial tachyarrhythmias in patients with congenital heart disease, few studies have looked at its effectiveness.
METHODS: A retrospective chart review was performed on 24 patients with congenital heart disease who underwen

BACKGROUND: The left atrial maze procedure is performed to treat atrial fibrillation (AF), mainly in patients with mitral valve disease. In this study, we assessed the midterm results of this procedure and clinically analyzed predicting factors for postoperative persistent AF.
METHODS: From June 1997 to May 2001, the left atrial maze procedure was performed on 31 patients (29 with mitral valve disease and 2 lone AF). For purposes of analysis, patients were divided postoperatively into those with persistent atrial fibrillation (AF) and those with sinus rhythm (SR), except 2 patients who requir

We present the case of an 80-year-old man with chronic atrial fibrillation associated with mitral regurgitation. The atrial fibrillation was successfully treated with the maze procedure combined with mitral valve replacement. The electrophysiological data are also reported. Recordings of sinus rhythm and intraatrial activity demonstrated the coexistence of sinus rhythm and fibrillation of both atria. This finding indicates that the sinus node was protected from segmental atrial fibrillation by entrance block, and this, in turn, is evidence of the efficacy of the maze procedure.

OBJECTIVE: To determine early term results of the modified Cox maze procedure for curing atrial fibrillation (AF) associated with mitral valve disease.
METHOD: Between January and December 2000, 10 consecutive patients with AF underwent the modified Cox maze procedure with mitral valve repair in 2 and replacement in 8. The associated procedure included 3 aortic valve replacements, 1 tricuspid annuloplasty, and 2 atrial septal defect closures. There were 5 males and 5 females, with ages ranging from 19 to 52 years (mean = 38.3 years). Pre-operative-existing AF time varied from 4 to 146 months

Twelve patients were operated on for mitral valve disease with concomitant chronic atrial fibrillation. Valve repair was performed in five patients and replacement in seven. Maze I and maze III procedures were applied in eight and four patients, respectively, and are compared. There was a regular rhythm in all maze I patients with a constant junctional rhythm in three and an alternating sinus and junctional rhythm in the remainder. In one case, part of the left atrium was in atrial fibrillation with the remaining atria in sinus rhythm. In the maze III group, one patient was always on a regular

BACKGROUND AND AIMS OF THE STUDY: The safety of combining mitral valve repair with the maze procedure for chronic atrial fibrillation in the surgical management of patients with mitral valve disease is not well elucidated. We present our operative results regarding mortality and morbidity after such combined surgery. As a comparison, our operative results after mitral valve repair in patients without chronic atrial fibrillation are presented.
METHODS: Between April 1993 and December 1994, 39 patients with chronic atrial fibrillation underwent mitral valve repair and concomitant maze procedure

A 34-year-old woman presented with palpitations and paroxysmal atrial fibrillation (AF). Workup revealed anterior mitral valve prolapse with severe mitral regurgitation. She was referred for surgical repair and underwent a mitral valve replacement, tricuspid valve repair, and bi-atrial cryoMAZE procedure with left atrial appendage ligation. Her postoperative course was complicated by inferior wall myocardial infarction. She subsequently presented with palpitations and underwent electrophysiology study and ablation. This case illustrates pitfalls associated with the surgical MAZE procedure and

Atrial fibrillation is associated with increased morbidity and mortality in patients undergoing mitral valve surgery. There is a growing consensus that patients with preexisting atrial fibrillation should undergo surgical ablation at the time of mitral valve surgery. Novel surgical ablation techniques, including cryoablation, have been developed to facilitate concurrent minimally invasive procedures. This video tutorial describes a combined cryo-maze procedure and mitral valve repair through an upper ministernotomy in a patient with long-standing persistent atrial fibrillation and severe mitra

Atrial septal defect is one of the most common congenital heart anomalies in adults. Patients with partial atrioventricular canal defects, previously known as ostium primum atrial septal defect, usually present at an early age, and only a few reach adulthood without surgical correction. Herein, we describe a young woman who presented with an ostium primum defect and severe symptomatic mitral and tricuspid regurgitation with paroxysmal atrial fibrillation. A complex repair was successfully done through a left atrial approach using robot-assistance.

BACKGROUND: Congenital heart disease (CHD) that causes right atrial dilatation is commonly associated with atrial flutter and/or fibrillation (AFl/F). To reduce late recurrence of AFl/F in patients undergoing repair of CHD, we utilized a concomitant right-sided maze procedure.
METHODS: From 1993 to 2003, 99 patients with CHD and associated AFl/F underwent a concomitant right-sided maze procedure at the time of CHD repair. Ages ranged from 9 to 72 years (median, 43 years). Atrial flutter and/or fibrillation was paroxysmal in 81 and chronic in 18; duration ranged from less than 1 month to 39.5

A 48-year-old man presented with typical features of Ehlers-Danlos syndrome, such as soft and hyperextensible skin, subcutaneous nodules, clubbed feet, and a mild degree of pigeon chest. In addition, his past history revealed congenital dislocation of the hip joint, retinal detachment, and repeated episodes of congestive heart failure due to severe mitral regurgitation and atrial fibrillation. We subjected the patient to surgical repair of the mitral valve combined with the maze procedure, which proved successful.
