KETAMINE-MEDETOMIDINE AND KETAMINE-MEDETOMIDINE-MIDAZOLAM ANESTHESIA IN CAPTIVE CHEETAHS (ACINONYX JUBATUS)-COMPARISON OF BLOOD PRESSURE AND KIDNEY BLOOD FLOW.

Six clinically healthy captive cheetahs ( Acinonyx jubatus ) were anesthetized twice using two different drug combinations to investigate if blood pressure and kidney blood flow are affected by medetomidine dosage. Protocol KM (2.0 mg/kg ketamine and 0.05 mg/kg medetomidine) was compared with protocol KMM (2.0 mg/kg ketamine, 0.02 mg/kg medetomidine, and 0.1 mg/kg midazolam). Heart rate (HR), respiratory rate (RR), body temperature, end-tidal carbon dioxide pressure (ETCO2), and anesthetic depth were monitored every 10 min. Noninvasive mean (MAP), systolic (SAP), and diastolic (DAP) arterial b

Ocular hypotensive effects of medetomidine and its analogs.

The ocular hypotensive effects of medetomidine, a relatively selective alpha 2-agonist, and its analogs were tested in rabbits and cats and their inhibition of adenylate cyclase in the isolated bovine ciliary process was also studied. It was found that topical unilateral administration of medetomidine (0.5-2.0%) to the normotensive rabbits produced a dose-dependent bilateral decrease in IOP with peak reduction in IOP at 2 hr in the treated eye and 1 hr in the untreated eye. A dose-dependent mydriasis was also observed in the treated eye. The dose-response curves of medetomidine and its analogs

Echocardiographic effects of medetomidine, butorphanol, and their combination in donkeys.

BACKGROUND: Standing sedation is frequently required in donkeys for minor surgical and diagnostic procedures, yet information on the cardiac safety of α2-adrenoceptor agonists and opioid combinations in this species is limited. This study evaluated the echocardiographic effects of intravenous medetomidine, butorphanol, and their combination in clinically healthy donkeys. MATERIALS AND METHODS: Sixty donkeys were randomly assigned to four groups (n = 15/group) to receive intravenous saline (control group, CG), medetomidine group (10 µg/kg, MG), butorphanol group (50 µg/kg, BG), or medetomidine

The Storm After the Calm: A Case of Medetomidine Withdrawal.

We report a case of a 30-year-old woman with a history of intravenous drug use, who presented to the emergency department with nausea, vomiting, agitation, and delirium. In order to protect her airway and to facilitate further evaluation, she received endotracheal intubation, but despite maximum pharmacologic sedation, she became increasingly tachycardic and hypertensive. A comprehensive blood toxicology panel revealed medetomidine, a sedative used in veterinary medicine and a known adulterant used in illicit fentanyl. While medetomidine has permeated the fentanyl supply in the middle Atlantic

Rise of illicit medetomidine use: A worrisome trend.

BACKGROUND AND OBJECTIVES: Medetomidine is a sedative approved by the FDA for veterinary use, but it is not authorized for human use. Its increasing presence alongside illegal opioids like fentanyl contributes to a rise in fatal overdoses across the U.S. While it is more potent than xylazine, knowledge surrounding medetomidine's role as an adulterant remains limited. This review investigates existing human data regarding its illicit use and overdose symptoms and highlights the evolving issue. METHODS: This review compiles case reports, law enforcement findings, and human studies to assess the

Accidental coadministration of medetomidine, vatinoxan and ketamine in a cat.

An 8-month-old, Domestic Shorthair male cat was presented for elective orchiectomy. Preanaesthetic clinical examination was unremarkable. The anaesthetic plan included intramuscular medetomidine (30 μg kg-1) and tramadol (2 mg kg-1) for premedication followed by intramuscular ketamine (8 mg kg-1) for induction of general anaesthesia. The premedication mixture, presumed to be medetomidine and tramadol, was given as a single injection. Approximately 15 minutes after premedication, the cat was profoundly sedated, with a heart rate of 140 beats minute-1. A catheter was placed in a cephalic vein. K

Novel combinations of nalbuphine and medetomidine for wildlife immobilization.

We formulated novel drug combinations of nalbuphine HCl and medetomidine HCl (NalMed), with or without azaperone tartrate, for use in immobilizing Rocky Mountain elk (Cervus elaphus nelsoni) and potentially for other wildlife species. Using the lowest tested nalbuphine dose (0.3 mg/kg) that produced sedation in elk, we initially evaluated a combination of nalbuphine, medetomidine, and azaperone (NalMed-A) for immobilizing adult elk. Based on initial success, we then conducted follow-up trials to assess alternative NalMed formulations successively modified to improve field usability, striving t

Comparative evaluation of anesthetic effects of medetomidine-midazolam-butorphanol and medetomidine-remimazolam-butorphanol administered subcutaneously and intraperitoneally in C57BL/6J mice.

Anesthesia management in experimental animals is essential for animal welfare and data reproducibility. In Japan, medetomidine-midazolam-butorphanol (MMB) is commonly used due to ketamine restrictions. We developed medetomidine-remimazolam-butorphanol (MRB) by replacing midazolam with remimazolam for faster metabolism and recovery. This study compared MMB and MRB administered intraperitoneally (i.p.) or subcutaneously (s.c.) in C57BL/6J mice of both sexes. Induction and recovery were assessed using immobilization, loss of righting reflex, and return of righting reflex. MRB i.p. produced the fa

Comparison of vatinoxan-medetomidine-ketamine-butorphanol and medetomidine-ketamine-butorphanol combinations for intramuscular anesthesia in New Zealand white rabbits (Oryctolagus cuniculus).

Rabbit perianesthetic morbidity and mortality are high due to unique physiologic and anatomic considerations. α2-adrenergic agonists are frequently used for sedation and anesthesia. However, these drugs can produce negative cardiovascular effects. A commercially available formulation licensed for sedation in dogs (Zenalpha®) combines medetomidine with vatinoxan, a peripherally acting α2-adrenergic antagonist. We evaluated this vatinoxan-medetomidine combination (2 mg/kg of vatinoxan-0.1 mg/kg of medetomidine) co-administered with ketamine (5 mg/kg) and butorphanol (0.5 mg/kg) (ZKB) in rabbits

Chiral sensitivity of medetomidine lateral flow immunoassay test strips.

Medetomidine has recently emerged in the illicit drug supply in the United States, and lateral flow immunoassay test strips are an inexpensive and easy-to-use field screening option for detecting this highly potent sedative. However, the drug is chiral, and the response of test strips to the different enantiomers has not been reported. This study evaluated the chiral sensitivity of 7 lots of medetomidine test strips produced by two manufacturers. Test strips were assessed using solutions of dexmedetomidine, levomedetomidine, and racemic medetomidine at varying concentrations, water types, and

Management of Patients with Medetomidine Withdrawal During Interfacility Transfer.

OBJECTIVES: Alpha-2 agonists including medetomidine are becoming increasingly prevalent as adulterants in the United States opioid supply. Complicated withdrawal following use of these agents is characterized by severe symptoms including agitation and nausea, often requiring treatment with high-dose dexmedetomidine infusion and transfer between community emergency departments and tertiary critical care units. We aimed to describe interventions and outcomes of patients undergoing interfacility transport while receiving a dexmedetomidine infusion for medetomidine withdrawal to inform safe transf

Emergence of Medetomidine in New York's Illicit Drug Supply.

AbstractNew York State and New York City health departments have closely monitored the emergence of medetomidine, a sedative that can cause prolonged sedation and a complex withdrawal syndrome, in the illicit drug supply. Drug checking and toxicology data first detected medetomidine in New York in mid-2024, and through 2025 it was identified in 25.1% of opioid samples analyzed. This Public Health Alerts report highlights state and local collaboration to detect emerging substances of concern in the illicit drug supply and to implement data-guided programmatic responses, including enhanced surve

Clinical Presentation of Medetomidine-Involved Opioid Overdoses-Chicago, 2024.

INTRODUCTION: Medetomidine, an alpha-2 adrenergic agonist, emerged as a U.S. drug adulterant in 2022. In May 2024, the Chicago Department of Public Health identified a cluster of opioid overdoses with unusual clinical features linked to medetomidine. METHODS: We performed a retrospective chart review of patients presenting to three Chicago emergency departments with suspected opioid-involved overdoses during May 11-17, 2024. We classified cases as confirmed, probable, or suspected based on independent clinical criteria and laboratory screening results. We performed descriptive statistics and
