Posterior Mediastinal Paravertebral Müllerian cyst (cyst of Hattori): literature review.

Mediastinal cysts are typically of bronchogenic, thymic or neurenteric origin, but may also represent oesophageal duplication. Posterior paravertebral mediastinal Müllerian cysts of undetermined pathogenesis are very rare occurrences. The first case of a ciliated cyst arising in the mediastinum, of probable Müllerian origin, was reported by Hattori in 2005, which gave rise to the name cyst of Hatorri (COH) The number of reported cases in literature of a similar nature have since then increased significantly. One of the main concerns about this pathology is the possibility of malignant transfor

[A variety of mediastinal bronchogenic cyst: the broncho-alveolar cyst].

Congenital bronchogenic cysts are usually located in the mediastinum or develop as intrapulmonary cysts. Gross examination of excised bronchogenic cysts shows them to be unilocular; histologically, these lesions are characterized by the presence of respiratory-type pseudostratified epithelium as well as small islands of cartilage and seromucinous glands. We report a case of a mediastinal bronchogenic cyst having pulmonary parenchyma within the cyst wall. The rarity of our case, the pathological and clinical features and the embryological development of bronchogenic cysts are briefly discussed.

Non-Neoplastic Mediastinal Cysts.

Primary mediastinal cysts are infrequent lesions that can arise from a variety of mediastinal organs or structures. Most of these are congenital in origin and incidental findings during investigations for unrelated conditions. Histologically, the cysts may be composed of various tissues, including bronchogenic, pericardial, thymic, enteric, Müllerian, lymphatic, and parathyroid types. Mediastinal cysts typically demonstrate a benign clinical course and patients are cured after complete surgical resection. In this review, the embryogenesis, clinical, radiologic, and pathologic characteristics o

[Mediastinal cysts, surgical management].

INTRODUCTION: Mediastinal cysts are common lesions, affecting children, as well as adults. They account for 20-30 % pathological mediastinal masses. The treatment is surgical, including extirpation, respectively enucleation of the cyst. AIM OF THE STUDY: Assessment of the author's patient group and overview of current literature data. MATERIAL AND METHODS: Retrospective analysis of a group of 11 patients (9 female and2 male subjects, the mean age was 47.6 years) indicated for scheduled procedures for mediastinal cysts from 2006 to 2010. RESULTS: Histological examination confirmed pericardia

Mediastinal foregut duplication cysts.

Foregut duplication cysts are rare congenital anomalies of enteric origin. In majority of the patients, the diagnosis is made in infancy. The authors report 4 cases of mediastinal foregut duplication cyst in children diagnosed on CT/MRI and confirmed on histopathology. In none of the cases the cysts had intraspinal extension nor heterotopic gastric mucosa.

Mediastinal parathyroid cysts revisited.

BACKGROUND: A case of a functioning mediastinal cyst is presented. METHODS: A comprehensive review of the literature found 93 patients in whom a parathyroid cyst or cysts extended into, or was completely contained within, the mediastinum. Including our patient, there were 46 men and 45 women, and the gender was not recorded in three. RESULTS: The cysts were located in the anterosuperior region in 56 patients, in the middle region of the mediastinum in 26, and in the anterior, prevascular region in 12. Thirty-nine patients had functioning cysts associated with hyperparathyroidism of varying s

[Pulmonary and mediastinal bronchogenic cysts].

Bronchogenic cysts are relatively rare anomalies with a congenital origin resulting in an abnormal branching of the tracheo-bronchial tree. They either develop in the pulmonary parenchyma or in the mediastinal region. Between 1975 and 1987, 21 cases of bronchogenic cysts were operated upon. Eighteen of these cysts had a pulmonary localisation and 3 were mediastinal. Only 8 of these were diagnosed pre-operatively by bronchoscopy. The major reason for delay in diagnosis is persistent or recurrent pulmonary infection. Eight cases were treated by cystectomy, 4 by simple wedge resection, 4 by segme

Primary mediastinal hydatid cysts.

PURPOSE: Hydatid disease is endemic in many parts of the world. Mediastinal hydatidosis is seen less than 0.1% of all hydatid diseases. We want to report our primary mediastinal hydatid cysts. MATERIALS AND METHODS: In this retrospective study, from January 2010 to December 2012, 158 patients with intrathoracic hydatid cysts were operated in our thoracic surgery clinic. Nine of 158 (5.69%) patients had mediastinal hydatid cyst. Chest X-ray and computed tomography (CT) were used as diagnostic tools. RESULTS: Hydatid cyst was confirmed surgically and pathologically in all the patients. Anterio

Mediastinoscopy in the treatment of mediastinal cysts.

OBJECTIVE: Primary cysts constitute 25% of all masses in the mediastinum. Because radiological investigations are often inconclusive, many adults require mediastinoscopy, thoracotomy, video-assisted thoracic surgery, or computed tomography-guided transbronchial, transesophageal, or transcutaneous aspiration to confirm the cystic nature of these lesions. Minimally invasive procedures fail when the cyst contents are gelatinous and mucoid (failure to aspirate) or when the cyst wall continues to secrete fluid. Though Pursel reported mediastinoscopic extirpation of benign cysts 35 years ago, it rem

Clinical spectrum of mediastinal cysts.

OBJECTIVES: Congenital cysts of the mediastinum are an uncommon but important diagnostic group, representing 12 to 30% of all mediastinal masses. The purpose of this study was to review our institutional experience with congenital cysts of the mediastinum, emphasizing the clinical spectrum of the disease. DESIGN: Retrospective study. OBJECTIVES: University hospital unit of general thoracic surgery. METHODS: We retrospectively reviewed the records of 105 patients with cysts of the mediastinum (50 male and 55 female patients) who comprised 13.0% of mediastinal masses over the past 50 years.

Non-neoplastic mediastinal cysts.

OBJECTIVE: The non-neoplastic mediastinal cysts (NNMCs) form a group of uncommon benign lesions of a congenital origin. The significant controversy regarding these cysts is whether to manage with observation or surgical resection. The aim of this study is to analyse the utility of thoracic computed axial tomography (CT) in imaging diagnosis of the NNMCs and the results of surgery in these lesions. PATIENTS AND METHODS: Twenty NNMCs underwent surgery between 1980 and 2000. The preoperative study of mediastinal cystic masses includes a complete blood test, chest radiography (CR) and, for the la

Imaging of mediastinal foregut cysts.

This study was undertaken to determine the value of magnetic resonance imaging (MRI) in the diagnosis of mediastinal foregut cysts. The MRI scans of 56 patients (with a total of 57 mediastinal masses) were reviewed. The patients (26 women and 30 men) had been examined between March 1986 and April 1991 at a tertiary-care hospital. T1-weighted and T2-weighted spin-echo images were available in all cases. The signal intensity of the lesions was compared with that of normal fat and muscle; the size, shape and location of the mediastinal masses were also noted. The 57 lesions consisted of 15 lympho

Symptomatic mediastinal thymic cysts.

The case of a symptomatic, hemorrhagic mediastinal thymic cyst associated with idiopathic thrombocytopenic purpura is reported and the literature reviewed. Contrary to previous reports, mediastinal thymic cysts are frequently symptomatic (39%), presenting with complaints of tracheal and esophageal compression. The age distribution is the same in symptomatic and asymptomatic patients. Complete excision remains the treatment of choice.

Diagnostic dilemmas of mediastinal cysts.

Children with foregut cysts of the mediastinum can present at any age with nonspecific respiratory symptoms or dysphagia. Chest radiograph and barium esophagram are recommended for initial evaluation, but they fail to identify some lesions. In other cases, an obvious mass may be confused with a solid neoplasm. We have operated upon 34 infants and children with mediastinal bronchogenic cysts and esophageal duplications from 1968 through 1985. This review of their clinical course and radiographic imaging studies emphasizes some of the diagnostic pitfalls that lead to operative delay. Twelve (35%

Primary Mediastinal Hydatid Cyst Causing Diaphragmatic Palsy.

Hydatid cystic disease is a significant clinical problem in endemic countries. Hydatid cysts are most commonly located in the liver and lungs. Primary mediastinal hydatid cyst is a rare clinical entity. The diagnosis must be considered in a patient with a mediastinal mass, particularly in endemic regions. Mediastinal hydatid cysts causing paralysis of phrenic and recurrent laryngeal nerves have been rarely reported. We describe a rare case of primary mediastinal hydatid cyst associated with diaphragmatic palsy caused by compression of the left phrenic nerve, which was successfully treated with

Mediastinal bronchogenic cyst mimicking congenital lobar emphysema.

Bronchogenic cyst (BC) is a rare congenital malformation of the lung. Most patients remain asymptomatic until adulthood while some are symptomatic in the first few years of life. However, symptoms in newborn period are rare. We report a case of a 3-day-old preterm baby with respiratory distress diagnosed as congenital lobar emphysema on chest X-ray. A CT scan revealed a mediastinal cyst causing obstructive lobar emphysema. The cyst was excised and pathological examination was suggestive of BC.

Robotic resection of a mediastinal parathyroid cyst.

Mediastinal parathyroid cysts (PC) are rare, benign lesions, reported in fewer than 150 cases worldwide. Although most are asymptomatic and discovered incidentally on imaging, symptoms of dyspnea, dysphagia, hoarseness, palpitations, hypercalcemia, and innominate or jugular venous thrombosis have been reported. Sternotomy or thoracotomy has traditionally been the approach used to resect mediastinal PCs. We describe the first reported case of a robot-assisted resection of a mediastinal PC.

An unusual cause of posterior mediastinal cyst.

Cystic lesions of the mediastinum may be congenital or acquired. The differential diagnosis depends on their location in the mediastinum. Cysts in the posterior mediastinum are generally developmental cysts and are neurogenic or of foregut origin. We report the case of a 14-year-old boy, who presented with dry cough and progressively increasing breathlessness, and was found to have a cystic lesion in the posterior mediastinum. Fine needle aspiration from the cyst helped make a diagnosis of tuberculosis.
