Medicaid and fulfillment of desired postpartum sterilization.

OBJECTIVE: We sought to assess fulfillment of sterilization requests while accounting for the complex interplay between insurance, clinical and social factors in a contemporary context that included both inpatient and outpatient postpartum sterilization procedures. STUDY DESIGN: This is a retrospective single-center cohort chart review study of 1331 women with a documented contraceptive plan at time of postpartum discharge of sterilization. We compared sterilization fulfillment within 90days of delivery, time to sterilization and rate of subsequent pregnancy after nonfulfillment between women

Ectopic pregnancy rates in the Medicaid population.

OBJECTIVE: The Centers for Disease Control and Prevention last estimated a national ectopic pregnancy rate in 1992, when it was 1.97% of all reported pregnancies. Since then rates have been reported among privately insured women and regional health care provider populations, ranging from 1.6-2.45%. This study assessed the rate of ectopic pregnancy among Medicaid beneficiaries (New York, California, and Illinois, 2000-03), a previously unstudied population. STUDY DESIGN: We identified Medicaid administrative claims records for inpatient and outpatient encounters with a principal International

Medicaid reimbursement and the uptake of adolescent vaccines.

BACKGROUND: In light of low adolescent vaccination rates, state-level policies that could improve vaccine coverage should be evaluated. Approximately 1/3 of adolescents are eligible, primarily through Medicaid enrollment, to receive vaccines from state-administered Vaccines for Children (VFC) programs. We investigated whether Medicaid reimbursement, the scope of implementation of VFC programs (i.e. limited or universal purchase), and/or presence of school-based vaccine mandates were associated with adolescent vaccination levels. METHODS: We performed a cross-sectional analysis of state-level

The little engine that could: Medicaid at the millenium.

The Medicaid program is little understood and, often, even less liked. It has suffered from poor design, unintended consequences, massive cost increases, provider hostility, and the slings and arrows of political fortune. Nonetheless, it provides at least some coverage to 1 in 8 Americans and has been a hotbed of health policy experimentation--often for better, sometimes for worse. As it stands at yet another crossroads, its fate is uncertain; yet there does not seem to be anything that could replace it, save a level of social cruelty this nation is so far not ready to accept. The question, th

Access of Medicaid recipients to outpatient care.

BACKGROUND: Visits to the emergency department by Medicaid recipients for nonemergency problems are common and contribute to rising health care costs. However, such patients may have few alternatives. We conducted a telephone survey of 953 ambulatory care sites in 10 cities to determine the availability of appointments for Medicaid recipients with common problems. METHODS: Research assistants telephoned all ambulatory care clinics and a stratified sample of private primary care practices in the catchment area served by the hospital emergency department in each city. The assistants identified

Medicare and Medicaid fraud and abuse regulations.

Specific business arrangements that are protected under legislation and regulations governing parties doing business with Medicare or Medicaid are discussed. Regulations implementing the Medicare and Medicaid Patient Protection Act of 1987 specify practices and activities that are not subject to criminal penalties under the antikickback provisions of the Social Security Act or to exclusion from Medicare or state health-care programs. As of July 29, 1991, all organized health-care settings that receive payments from either Medicare or state health-care programs must comply with these regulation

Examining Racial/Ethnic Disparities in Tobacco Dependence Treatment Among Medicaid Beneficiaries Using Fifty State Medicaid Claims, 2009-2014.

In the USA, low-income racial/ethnic minority groups experience higher smoking rates and greater smoking-related disease burden than their White counterparts. Despite the adverse effects, racial/ethnic minorities are less likely to access tobacco dependence treatment (TDT). Medicaid is one of the largest payers of TDT in the USA and covers predominantly low-income populations. The extent of TDT use among beneficiaries from distinct racial/ethnic groups is unknown. The objective is to estimate racial/ethnic differences in TDT use among Medicaid fee-for-service beneficiaries. Using a retrospecti

Potential adult Medicaid beneficiaries under the Patient Protection and Affordable Care Act compared with current adult Medicaid beneficiaries.

PURPOSE: Under health care reform, states will have the opportunity to expand Medicaid to millions of uninsured US adults. Information regarding this population is vital to physicians as they prepare for more patients with coverage. Our objective was to describe demographic and health characteristics of potentially eligible Medicaid beneficiaries. METHODS: We performed a cross-sectional study using data from the National Health and Nutrition Examination Survey (2007-2010) to identify and compare adult US citizens potentially eligible for Medicaid under provisions of the Patient Protection and

Administrative Burden Documented in Medicaid Care Coordination.

IMPORTANCE: Administrative burdens, the time, effort, and stress of navigating bureaucratic requirements, impede Medicaid beneficiaries and persist even when care is obtained, yet their frequency and patient time costs are largely invisible in claims data. OBJECTIVE: To measure administrative burdens documented in Medicaid care coordination notes by type, quantify the patient time costs they entail at multiple wage valuations, and characterize differences by race and ethnicity. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study of Medicaid managed care beneficiaries in a single co

Opioid Treatment Programs' Medicaid Patient Retention Rates.

IMPORTANCE: Retention on methadone and buprenorphine has been shown to be associated with reduced overdose and death. Opioid treatment programs (OTPs) are the only health care facilities in the US licensed to dispense methadone for opioid use disorder (OUD), and they increasingly prescribe buprenorphine. By measuring and comparing their patient retention rates, after adjusting for patient characteristics, OTPs can determine if their retention rate is low relative to other OTPs, which may motivate efforts to improve patient engagement and outcomes. OBJECTIVE: To develop a standardized case mix

Re-Evaluating the Medicaid Sterilization Consent Process.

Currently, individuals with federally-funded health insurance who desire sterilization are required to sign the Consent for Sterilization form (Title XIX form) at least 30 days before the date of the procedure. The form remains valid for 180 days with the only exceptions being cases in which emergency abdominal surgery or premature delivery is warranted. These tenets were constructed in response to public outcry over sterilization abuses that had occurred during the early 20th century. Although the intention of the Medicaid sterilization consent process lay in protecting the reproductive right

Medicaid Continuous Coverage Requirement and Postpartum Hospitalization.

IMPORTANCE: In 2020, all US states adopted the Families First Coronavirus Response Act Medicaid continuous coverage requirement (CCR), which prevented disenrollment from Medicaid. This policy effectively extended pregnancy-related Medicaid eligibility beyond the previous end date of 60 days post partum. OBJECTIVE: To determine whether the CCR was associated with a change in postpartum hospitalization rates for mothers covered by Medicaid during their delivery. DESIGN, SETTING, AND PARTICIPANTS: This cohort study was conducted from December 2023 to March 2025 at all nonfederal acute care hosp

Utilization of varenicline among Kansas Medicaid enrollees.

BACKGROUND: Smoking prevalence among Medicaid beneficiaries is twice that of privately insured adults, yet effective smoking cessation treatments remain underutilized. Understanding varenicline use, the most effective smoking cessation medication, can inform efforts to improve tobacco treatment for Medicaid enrollees. OBJECTIVES: To describe varenicline utilization among Kansas Medicaid (KanCare) enrollees from 2014-2021, including changes related to the FDA's December 2016 removal of the black box warning, and explore changes and demographic differences in its use. Inpatient Medicaid Utilization at Select Children's Hospitals.

Medicaid and the Children's Health Insurance Program together provide health insurance for 78.6 million low-income Americans, including 37.3 million children. This study describes inpatient utilization at select U.S. children's hospitals by Medicaid-insured patients. Legislation passed in July 2025 changed Medicaid eligibility requirements. For this study, RAND researchers processed hospital cost report data for policymakers to analyze.

Contextualizing Medicaid reimbursement rates for abortion procedures.

OBJECTIVE: Low Medicaid reimbursement rates have been cited as a key threat to abortion clinic sustainability in the United States. This study examines differences between Medicaid and Medicare reimbursements for abortion and miscarriage management procedures under a fee-for-service (FFS) model. STUDY DESIGN: Using 2017 Medicaid and Medicare Physician fee schedules, we extracted reimbursement data for the two most commonly-billed abortion procedures and two miscarriage management procedures for 45 states and the District of Columbia (DC). We compared Medicaid and Medicare reimbursement rates
