Potentially Inappropriate Medications in a Psychogeriatric Inpatient Ward: An Audit Based on Beers Criteria.

INTRODUCTION: Older adults are often prescribed a high number of medications. Overcoming inaction in health care is difficult but essential to deprescribing potentially inappropriate medications (PIMs). Curbing the use of PIMs is urgent in older adults suffering from psychiatric illness, as cognitive, emotional and psychosocial adversity makes them an especially vulnerable group. We aimed to audit PIM usage and deprescribing in an old-age psychiatry inpatient service of a university-affiliated public hospital. METHODS: The 2015 American Geriatrics Society (AGS) Beers Criteria were used in the

Introduction of early medical abortion in New Zealand: an audit of the first 67 cases.

BACKGROUND: In New Zealand, mifepristone became available in 2001, but because of uncertainty about the law, the first 67 cases were carried out under a very strict protocol. Once the prostaglandin had been administered it was necessary that the woman remain in the unit until the products of conception (POC) had been passed and, if this had not occurred within 8 h, she underwent suction curettage. AIMS: To demonstrate that an early medical termination of pregnancy (EMTOP) service could be offered as a safe option for women, despite the constraints of the law. METHODS: An audit of patient not

Pentoxifylline and Tocopherol for the Management of Medication-Related Osteonecrosis of the Jaw (MRONJ): A Retrospective Clinical Audit.

Background/Objectives: Medication-related osteonecrosis of the jaw (MRONJ) is a challenging complication in patients receiving antiresorptive therapy. Management strategies range from conservative pharmacological approaches to extensive surgical resection of necrotic bone. This clinical audit retrospectively evaluated the clinical outcomes of patients undergoing sequestrectomy for MRONJ, comparing those treated with antibiotics alone with those receiving antibiotics in combination with the pentoxifylline-tocopherol (PENTO) protocol. The PENTO protocol was introduced at our institution in 2021

Auditing description-logic-based medical terminological systems by detecting equivalent concept definitions.

OBJECTIVE: To specify and evaluate a method for auditing medical terminological systems (TSs) based on detecting concepts with equivalent definitions. This method addresses two important problems: redundancy, where the same concept is represented more than once (described by different terms), and underspecification, where different concepts have the same representation and hence appear indistinguishable from each other. DESIGN: The auditing method is applicable for TSs that are or can be represented in a description logic (DL). The method relies on the assumption that concept definitions are

Improving End-of-Life Care for Patients in Australian Cardiology Wards: A Retrospective Medical Record Audit.

BACKGROUND & AIM: Dynamic hospital settings such as cardiology wards face challenges in delivering high-quality end-of-life care due to high patient volumes, urgent clinical demands, variable disease trajectories, and time pressures, often resulting in delayed goals-of-care discussions and suboptimal care. Despite the availability of national End-of-Life Care Standards and associated quality audit tools, evidence describing their use and end-of-life care practices in Australian cardiology wards remains limited. We aimed to examine end-of-life care in cardiology wards within Australian hospital

Pharmacological thromboprophylaxis in elderly medical inpatients: Lessons from a local audit in the era of NEJM Evidence.

The recent NEJM Evidence trial questioned the benefit of enoxaparin for venous thromboembolism (VTE) prevention among older hospitalised medical patients. We conducted a retrospective, observational audit of 142 elderly inpatients at a large Australian tertiary institution; most received enoxaparin unless contraindicated, with excellent adherence to guidelines. Clinical events were rare with two VTE (1.4%) and five minor bleeds (3.5%) recorded and no major bleeding observed. Approximately 10% of patients were ambulant and prescribed thromboprophylaxis outside guideline criteria, yet bleeding r

Was that hysterectomy really necessary? Audit of operative justification at the Aga Khan University Medical Centre, Karachi.

Hysterectomy in Pakistan, like in other parts of the world is considered to be overused in a number of cases. As a part of a quality assurance process at the Aga Khan University Medical Centre, Karachi, 376 hysterectomies performed between January, 1987 and December, 1989 were retrospectively analysed and the results are presented. In 250 (66.5%) cases, where pathology was expected to be found, the hysterectomy was considered justified if the preoperative diagnosis was verified by the pathology report of if significant alternate pathology was present. In 126 (33.5%) cases, where no pathology w

Root Cause Analysis, Action, and Audit (RCA3): A Novel Approach to Sustainably Reduce Medication Errors.

BACKGROUND: Medical errors threaten patient safety. Although root cause analysis (RCA) is commonly used to investigate medication errors, such errors often recur, posing ongoing risks to patient safety. The RCA2 framework, introduced by the National Patient Safety Foundation, aimed to strengthen traditional RCA by emphasizing action-oriented outcomes. To further enhance sustainability, we implemented RCA3-an iterative process adding follow-up audits to evaluate long-term effectiveness of corrective actions. We aim to highlight RCA3's ability to maintain the effectiveness of interventions in pr
