Reasoning-driven large language models in medicine: opportunities, challenges, and the road ahead.

Developments in large language models (LLMs) in the past 2 years have shifted the focus from text, image, and audio generation to LLMs capable of multistep reasoning (thinking). The development of LLMs is particularly important for medicine and health care, but the translation of these models has been limited by the black-box nature of previous LLMs. New reasoning-driven LLMs incorporate chain-of-thought prompting and reveal intermediate reasoning steps, offering transparency and traceability, potentially improving the clinical adoption and utility of LLMs. In this Viewpoint, we examine four e

TriFuse: A multi-signal three-stage data distillation framework for efficient medical model training.

OBJECTIVE: Large-scale medical instruction datasets often contain substantial noise and redundancy, leading to high training costs without significant performance gains. Existing data selection and distillation methods rely on single static criteria, failing to capture the diversity and dynamic nature of medical samples during learning. This study proposes TriFuse, a three-stage data distillation framework that integrates multiple complementary learning signals for efficient training of medical question-answering models. METHODS: The framework comprises three stages: (1) Proxy-student filteri

A model curriculum of health care informatics for Dutch higher professional education.

This paper describes the results of a two year project to design a model curriculum of health care informatics for Dutch higher professional education. The core of the curriculum are sixteen modules which cover the broad range of medical informatics and which are closely related to the profiles of the professions involved (nursing, physiotherapy, speech therapy, occupational therapy and dietetics). The curriculum emphasizes the need of using structured data and information to perform tasks in health care delivery and management, for which modern information technology is indispensable. The mod

Empowering Health Care Actors to Contribute to the Implementation of Health Data Integration Platforms: Retrospective of the medEmotion Project.

Health data integration platforms are vital to drive collaborative, interdisciplinary medical research projects. Developing such a platform requires input from different stakeholders. Managing these stakeholders and steering platform development is challenging, and misaligning the platform to the partners' strategies might lead to a low acceptance of the final platform. We present the medEmotion project, a collaborative effort among 7 partners from health care, academia, and industry to develop a health data integration platform for the region of Limburg in Belgium. We focus on the development

Big Data, Information Technology and Information Professionals: Some Considerations for Digital Ethics.

It is generally accepted that the global evolution of health information technology, both in design and usage, raises ethical issues that should be addressed. However, the terms in which this concern is expressed are shrouded in ambiguity. Even what constitutes an ethical issue itself is never clearly defined. The present discussion attempts to clarify the landscape and suggests how the concern should be addressed.

A comprehensive data level analysis for cancer diagnosis on imbalanced data.

The early diagnosis of cancer, as one of the major causes of death, is vital for cancerous patients. Diagnosing diseases in general and cancer in particular is a considerable application of data analysis for medical science. However, imbalanced data distribution and imbalanced quality of the majority and minority classes, which lead to misclassification, is a great challenge in this field. Though the samples of the majority class and their proper classification are more important to classifier, cancer is diagnosed by relying on the minority class samples (cancer data class). While the conseque

Big data, big knowledge: big data for personalized healthcare.

The idea that the purely phenomenological knowledge that we can extract by analyzing large amounts of data can be useful in healthcare seems to contradict the desire of VPH researchers to build detailed mechanistic models for individual patients. But in practice no model is ever entirely phenomenological or entirely mechanistic. We propose in this position paper that big data analytics can be successfully combined with VPH technologies to produce robust and effective in silico medicine solutions. In order to do this, big data technologies must be further developed to cope with some specific re

Multi-channel causal variational autoencoder for multimodal biomedical causal disentanglement.

The multimodal nature of clinical assessment and decision-making, and the high rate of healthcare data generation, motivate the need to develop approaches specifically tailored to the analysis of these complex and potentially high-dimensional multimodal datasets. This poses both technical and conceptual challenges: how can such heterogeneous data be analyzed jointly? How can modality-specific information be identified from shared information? Variational autoencoders (VAEs) offer a robust framework for learning latent representations of complex data distributions, while being flexible enough t

DOME: Directional medical embedding vectors from Electronic Health Records.

MOTIVATION: The increasing availability of Electronic Health Record (EHR) systems has created enormous potential for translational research. Recent developments in representation learning techniques have led to effective large-scale representations of EHR concepts along with knowledge graphs that empower downstream EHR studies. However, most existing methods require training with patient-level data, limiting their abilities to expand the training with multi-institutional EHR data. On the other hand, scalable approaches that only require summary-level data do not incorporate temporal dependenci

Multimodal representation learning for medical analytics - a systematic literature review.

Objectives: Machine learning-based analytics over uni-modal medical data has shown considerable promise and is now routinely deployed in diagnostic procedures. However, patient data consists of diverse types of data. By exploiting such data, multimodal approaches promise to revolutionize our ability to provide personalized care. Attempts to combine two modalities in a single diagnostic task have utilized the evolving field of multimodal representation learning (MRL), which learns a shared latent space between related modality samples. This new space can be used to improve the performance of ma

Managing multiple perspectives in the collaborative design process of a team health information technology.

We need to design technologies that support the work of health care teams; designing such solutions should integrate different clinical roles. However, we know little about the actual collaboration that occurs in the design process for a team-based care solution. This study examines how multiple perspectives were managed in the design of a team health IT solution aimed at supporting clinician information needs during pediatric trauma care transitions. We focused our analysis on four co-design sessions that involved multiple clinicians caring for pediatric trauma patients. We analyzed design se

Ensuring IT service continuity in the face of increasing threats.

How is IT service continuity related to business continuity management? Is it just a glorified disaster recovery procedure? Will IT service continuity help increase the assurance of IT services from the business owner to the customer? This paper is an attempt at answering these and many such questions. It is presented as a case study of IT service continuity management implementation at Emirates Group IT, Dubai. It takes the reader through the need for the process as felt by the business, through the learning acquired during implementation, to the practices deployed for managing the process on

E-health: steps on the road to interoperability.

Interoperable electronic medical records (EMRs) have the potential to produce better health outcomes while improving the efficiency of care delivery and reducing its costs. Implementation will require massive changes at all levels. In many instances, the costs of implementation could fall on one group, while savings will accrue to some other group. A successful transition strategy identifies a series of steps, where each step pays its own way, at the level of the local groups directly affected, and lays the foundation for the next step. Such a strategy implies an era in which large groups will
