Transitive Sequencing Medical Records for Mining Predictive and Interpretable Temporal Representations.

Electronic health records (EHRs) contain important temporal information about the progression of disease and treatment outcomes. This paper proposes a transitive sequencing approach for constructing temporal representations from EHR observations for downstream machine learning. Using clinical data from a cohort of patients with congestive heart failure, we mined temporal representations by transitive sequencing of EHR medication and diagnosis records for classification and prediction tasks. We compared the classification and prediction performances of the transitive sequential representations

Computational techniques for ECG analysis and interpretation in light of their contribution to medical advances.

Widely developed for clinical screening, electrocardiogram (ECG) recordings capture the cardiac electrical activity from the body surface. ECG analysis can therefore be a crucial first step to help diagnose, understand and predict cardiovascular disorders responsible for 30% of deaths worldwide. Computational techniques, and more specifically machine learning techniques and computational modelling are powerful tools for classification, clustering and simulation, and they have recently been applied to address the analysis of medical data, especially ECG data. This review describes the computati

Interpretation of Medical Findings in Suspected Child Sexual Abuse: An Update for 2018.

Most sexually abused children will not have signs of genital or anal injury, especially when examined nonacutely. A recent study reported that only 2.2% (26 of 1160) of sexually abused girls examined nonacutely had diagnostic physical findings, whereas among those examined acutely, the prevalence of injuries was 21.4% (73 of 340). It is important for health care professionals who examine children who might have been sexually abused to be able to recognize and interpret any physical signs or laboratory results that might be found. In this review we summarize new data and recommendations concern

Interpretation of uniocular and binocular trials of glaucoma medications: an observational case series.

BACKGROUND: To predict the effectiveness of topical glaucoma medications based on initial uniocular and binocular treatment. To test a traditional hypothesis that effectiveness following a uniocular trial is associated with the change in IOP in the initially treated eye minus the change in the initially untreated eye. To determine whether uniocular or binocular treatment trials are superior. METHODS: Based on a review of medical records, we identified 168 instances in 154 patients with bilateral primary open angle glaucoma of initial uniocular use of a topical glaucoma medication with well-do

[Height-induced vertigo and its medical interpretation: Goethe and the Strassburger Münster].

An analysis combining medicine and literature challenges the methodology of both disciplines. This essay on the vertigo Goethe suffered on the tower of the Strasbourg Minster attempts to trace the vicissitudes of interpreting an emblem, like vertigo, burdened by cultural meaning and implications. Thus, Goethe's own report of this event 40 years after the fact, in his "Dichtung und Wahrheit", has to be related to another, hidden chronology of vertigo and fear in his account which, at first glance, conveys quite different implications. The first part of this paper refers to a medical interpretat

A comparative analysis of the interpretations of lumbar spinal radiographs by chiropractors and medical doctors.

Ninety-nine anteroposterior and lateral lumbar radiographs taken of men, 18 to 55 years of age, were randomly selected from participants in a population survey of low-back pain. Thirty-one (31%) had never had low-back pain; 44 (44%) had or were having mild low-back pain; and 24 (24%) had or were having severe low-back pain. Three chiropractors assessed 56 radiographic variables, which included determinations of disc space height, vertebral malalignments and subluxations, spondylosis, postural disturbances, relationships among pelvis and spine and other congenital or acquired abnormalities. Interpreting the Spine Patient Outcomes Research Trial. Medical vs surgical treatment of lumbar disk herniation: implications for future trials.

The Spine Patient Outcomes Research Trial (SPORT) consisted of two parallel studies: an observational study and a randomized comparison of medical and surgical treatment of disk herniation. In the long-term, patients did well with either treatment, and an intention-to-treat analysis found no difference in outcomes. However, at 2 years 40% of patients in the surgical group of the randomized study still hadn't had surgery, and 40% of the medical patients did have surgery, muddying the results. Surgery was superior according to an analysis by the treatment patients actually received, but the stud

Medical students are accurate in interpreting the presence of pathologic interstitial edema on focused lung ultrasound compared to expert reviewers.

BACKGROUND: Over the past two decades, studies have demonstrated that lung ultrasound is useful in diagnosing alveolar interstitial syndrome, which is seen in patients with decompensated congestive heart failure (CHF). METHODS: We studied medical students performing lung ultrasound on patients admitted to the hospital with a presumed diagnosis of decompensated CHF in a prospective convenience observation study. Two ultrasound fellowship-trained emergency medicine attendings independently reviewed the lung ultrasounds at a later date, blinded to the students' interpretation and other clinical

Token-based fidelity scoring for trustworthy vision transformer interpretations in medical imaging.

PURPOSE: This work addresses the problem that interpretability methods for medical imaging models are still mostly evaluated by agreement with human annotations, even though such agreement does not necessarily indicate that an explanation is faithful to the model's actual decision-making. The goal is to quantify fidelity, defined here as the extent to which an explanation reflects the evidence that truly drives model predictions, independent of human intuition, bias, or incomplete annotation. METHODS: We propose a prediction confidence-based approach to estimate token importance in vision tra

ECGSight: A Digital Caliper Application for Teaching Electrocardiogram Interpretation in Undergraduate Medical Education.

The interpretation of the electrocardiogram (ECG) is a core competency for medical students and is traditionally taught using paper ECG tracings and physical calipers. As clinical practice increasingly shifts toward digital ECG records, the use of physical calipers on computer screens has become impractical, often leading students to rely passively on automated machine-generated interpretations. To address this gap, we developed ECGSight, an open-source, interactive application built using R and Shiny. ECGSight functions as a digital caliper, enabling users to upload any ECG image, calibrate t

Determinants of authentic responses to medical student wellbeing surveys: An interpretive description study.

INTRODUCTION: Medical students develop higher rates of depression and burnout relative to similarly aged university graduates, suggesting that the medical school journey contributes to adverse mental health. Medical students prefer self-report surveys to characterize their wellbeing; however, enlisting authentic responses can be challenging. Our study sought to identify factors that impact medical student engagement and honesty when completing wellbeing surveys. METHODS: From June 2023 to September 2023, we conducted semi-structured interviews with 20 medical students at the University of Alb

[Interpretation of the connotation of the colored drawing of Neijing Tu in the Museum of Chinese Medical History].

Neijing Tu(, Chart of Inner Landscape), collected by the Museum of Chinese Medical History, is a colored drawing which is used to guide Taoist internal alchemy training pattern. It belongs to the inheritance of the immortals in the Tao Yin(physical and breathing exercise) of traditional Chinese medicine in folk. It is the essence for nurturing vitality of the traditional Chinese medicine. Its core tenet is that one practices with both Shen(spirit) and Qi(pneuma) and makes both to fuse in perfect harmony way. The specific practice process includes four phases: refining Jing and converting it in

Medical termination of pregnancy in general practice in Australia: a descriptive-interpretive qualitative study.

BACKGROUND: Australian Government approval in 2012 for the use of mifepristone and misoprostol for medical termination of pregnancy (MTOP) allows general practitioners (GPs) to provide early gestation abortion in primary care settings. However, uptake of the MTOP provision by GPs appears to be low and the reasons for this have been unclear. This study investigated the provision of and referral for MTOP by GPs. METHODS: We undertook descriptive-interpretive qualitative research and selected participants for diversity using a matrix. Twenty-eight semi-structured interviews and one focus group (

Direct and indirect immunofluorescence as a diagnostic adjunct in the interpretation of nonneoplastic medical lung disease.

Fresh open lung biopsy material from 57 patients was incubated with fluoresceinated complement and immunoglobulin antisera. An indirect immunofluorescent assay using neonatal lung as substrate was conducted as well. Direct immunofluorescent patterns could be categorized into interalveolar septal capillary deposition, large vessel wall localization, alveolar basement membrane localization, or a pauci-immune immunofluorescence pattern. With respect to the septal capillary pattern, endothelial cell decoration was seen with scleroderma, mixed connective tissue disease, anti-Ro-associated lupus ery

Medical Therapy, Radiofrequency Ablation, or Cryoballoon Ablation as First-Line Treatment for Paroxysmal Atrial Fibrillation: Interpreting Efficacy Through the Shiny Method.

In patients with paroxysmal atrial fibrillation, cryoballoon ablation (CBA) and radiofrequency ablation (RFA) represent two therapeutic approaches supported by increasing literature. While both these ablation techniques play a role during different stages of the patient's therapeutic pathway, their use as first-line is being increasingly recognized. This scoping review comparatively examined the evidence of effectiveness for these two ablation techniques. Our analysis was limited to the evaluation of the end-point of time to recurrence of atrial fibrillation (or other forms of atrial arrhythmi

A new quantitative method for determining patient risk for reusable medical device categorization based on using and interpreting Kremer's cleaning classification system.

BACKGROUND: The cleaning of reusable medical devices involves inherent challenges that can impact on the effectiveness of the cleaning process; consequently, the subsequent safety of patients. Fluid dynamics play a critical role in determining the flow and distribution of cleaning agents where the design of the device can either facilitate or hinder this important process. Complex geometries, narrow channels, or irregular surfaces can impede effective flushing of contaminants leading to incomplete cleaning that creates a greater likelihood for patient contamination risks. METHODS: Device feat

What do we know about volumetric medical image interpretation?: a review of the basic science and medical image perception literatures.

Interpretation of volumetric medical images represents a rapidly growing proportion of the workload in radiology. However, relatively little is known about the strategies that best guide search behavior when looking for abnormalities in volumetric images. Although there is extensive literature on two-dimensional medical image perception, it is an open question whether the conclusions drawn from these images can be generalized to volumetric images. Importantly, volumetric images have distinct characteristics (e.g., scrolling through depth, smooth-pursuit eye-movements, motion onset cues, etc.)

Prior knowledge of normal ECGs enhances accuracy in ECG interpretation among medical students.

Electrocardiograms (ECGs) are frequently misdiagnosed, with potentially serious consequences, yet evidence for effective teaching strategies remains limited. Eye-tracking offers insight into the perceptual processes behind accurate interpretation. We conducted an eye-tracking study among medical students to assess whether priming with normal ECGs improves diagnostic accuracy. We hypothesized that participants viewing normal ECGs before interpreting unknown ECGs would perform more accurately. This between-subjects priming study presented students with one of three image types before an ECG task
