System-Wide, Electronic Health Record-Based Medication Alerts for Appropriate Prescribing of Direct Oral Anticoagulants: Pilot Randomized Controlled Trial.

BACKGROUND: While direct oral anticoagulants (DOACs) have improved oral anticoagulation management, inappropriate prescribing remains prevalent and leads to adverse drug events. Antithrombotic stewardship programs seek to enhance DOAC prescribing but require scalable and sustainable strategies. OBJECTIVE: We present a pilot, prescriber-level randomized controlled trial to assess the effectiveness of electronic health record (EHR)-based medication alerts in a large health system. METHODS: The pilot assessed prescriber responses to alerts for initial DOAC prescription errors (apixaban and riva

An Investigation of Drug-Drug Interaction Alert Overrides at a Pediatric Hospital.

OBJECTIVES: Drug-drug interactions (DDIs) can result in patient harm. DDI alerts are intended to help prevent harm; when the majority of alerts presented to providers are being overridden, their value is diminished. Our objective was to evaluate the overall rates of DDI alert overrides and how rates varied by specialty, clinician type, and patient complexity. METHODS: A retrospective study of DDI alert overrides that occurred during 2012 and 2013 within the inpatient setting described at the medication-, hospital-, provider-, and patient encounter-specific levels was performed at an urban, qu

Development of a drug allergy alert tiering algorithm for penicillins and cephalosporins.

INTRODUCTION: Limited research is available regarding recommendations about which drug allergy alerts (DAAs) in clinical decision support (CDS) systems should interrupt provider workflow. The objective was to evaluate the frequency of penicillin and cephalosporin DAA overrides at two institutions. A secondary objective was to redesign DAAs using a new tiered alerting system based on patient factors. METHODS: A retrospective, observational study evaluated CDS DAA overrides for penicillins and cephalosporins at two large academic medical centers. Included patients were at least 18 years of age

Efficiency of computerized clinical decision support systems involving anticoagulants: A flashmob study in Dutch hospital pharmacies.

AIMS: Computerized decision support systems (CDSSs) aim to prevent adverse drug events. However, these systems generate an overload of alerts that are not always clinically relevant. Anticoagulants are frequently involved in these alerts. The aim of this study was to investigate the efficiency of CDSS alerts on anticoagulants in Dutch hospital pharmacies. METHODS: A multicentre, single-day, cross-sectional study was conducted using a flashmob design in Dutch hospital pharmacies, which have CDSSs that operate on both a national medication surveillance database and on self-developed clinical ru

SOA approach for integration of departmental systems.

In this paper a unified method for integration of departmental systems into the main systems of a healthcare organization is described. The approach is based on combining Business Process Modeling (BPM) and Service Oriented Architecture (SOA) methods and technologies. A top-down approach is used for modeling the care process and supporting care services which in turn are decomposed down to such a level of granularity that they can be described and implemented as web services described with Web Services Description Language (WSDL) documents. Then a bottom-up approach is used for wrapping the ex

Infusion Pump Interoperability and Resulting Alert Reduction: Multisystem Findings.

BACKGROUND: Intravenous (IV) medication errors pose significant patient safety risks. While most hospitals use IV smart pumps, adoption of interoperability with electronic health records remains limited. Interoperability can reduce nursing workload and programming errors, a common source of IV medication errors. OBJECTIVE: This study aimed to explore the impact of interoperability on programming alerts across diverse health systems that used a similar process and IV smart pump vendor for implementation. METHODS: This pre-post retrospective study analyzed data from 22 clinical sites across 5

Retrospective analysis of the impact of electronic medical record alerts on low value care in a pediatric hospital.

OBJECTIVES: Hospital costs continue to rise unsustainably. Up to 20% of care is wasteful including low value care (LVC). This study aimed to understand whether electronic medical record (EMR) alerts are effective at reducing pediatric LVC and measure the impact on hospital costs. MATERIALS AND METHODS: Using EMR data over a 76-month period, we evaluated changes in 4 LVC practices following the implementation of EMR alerts, using time series analysis to control for underlying time-based trends, in a large pediatric hospital in Australia. The main outcome measure was the change in rate of each

Implementation and Evaluation of an Alternative Electronic Health Record Tool for Ordering Blood Products in Pediatric Oncology and Stem Cell Transplantation: Mixed Methods Analysis.

BACKGROUND: Repeated blood product ordering is associated with order entry errors and potential patient harm. Traditional electronic health record order sets require repeated re-entry for recurrent transfusions, creating inefficiencies and opportunities for error, and contributing to physician burnout. Historically, we have used order sets to order blood products, which must be re-entered each time a transfusion is needed. Reusable transfusion therapy plans may address these challenges by standardizing and streamlining transfusion workflows. We conducted a pre-post study at a single pediatric

Interrupting providers with clinical decision support to improve care for heart failure.

BACKGROUND: Evidence-based therapy for heart failure remains underutilized at hospital discharge, particularly for patients with heart failure who are hospitalized for another cause. We developed clinical decision support (CDS) to recommend an angiotensin converting enzyme (ACE) inhibitor during hospitalization to promote its continuation at discharge. The CDS was designed to be implemented in both interruptive and non-interruptive versions. OBJECTIVES: To compare the effectiveness and implementation of interruptive and non-interruptive versions of a CDS to improve care for heart failure. ME

Pharmacogenomic augmented machine learning in electronic health record alerts: A health system-wide usability survey of clinicians.

Pharmacogenomic (PGx) biomarkers integrated using machine learning can be embedded within the electronic health record (EHR) to provide clinicians with individualized predictions of drug treatment outcomes. Currently, however, drug alerts in the EHR are largely generic (not patient-specific) and contribute to increased clinician stress and burnout. Improving the usability of PGx alerts is an urgent need. Therefore, this work aimed to identify principles for optimal PGx alert design through a health-system-wide, mixed-methods study. Clinicians representing multiple practices and care settings (

Comparison of antibiotic dosing recommendations for neonatal sepsis from established reference sources.

Background Incorrect dosing is the most frequent prescribing error in neonatology, with antibiotics being the most frequently prescribed medicines. Computer physician order entry and clinical decision support systems can create consistency contributing to a reduction of medication errors. Although evidence-based dosing recommendations should be included in such systems, the evidence is not always available and subsequently, dosing recommendations mentioned in guidelines and textbooks are often based on expert opinion. Objective To compare dosage recommendations for antibiotics in neonates with

Detecting pregnancy use of non-hormonal category X medications in electronic medical records.

OBJECTIVES: To determine whether a rule-based algorithm applied to an outpatient electronic medical record (EMR) can identify patients who are pregnant and prescribed medications proved to cause birth defects. DESIGN: A descriptive study using the University of Pennsylvania Health System outpatient EMR to simulate a prospective algorithm to identify exposures during pregnancy to category X medications, soon enough to intervene and potentially prevent the exposure. A subsequent post-hoc algorithm was also tested, working backwards from pregnancy endpoints, to search for possible exposures that

Evaluation of alerts for potassium-increasing drug-drug-interactions.

Electronic alerts for preventing hyperkalaemia during potassium-increasing drug-drug-interactions (DDIs) are often overridden due to their low specificity. Treatments of 76,467 inpatients were retrospectively analysed to establish more specific alerts. Alerting concepts for identifying DDIs that induced hyperkalaemia (serum potassium ≥5.5 mEq/l were compared. The positive predictive value (PPV) of alerts was 2.9% if they were triggered at onset of each potassium-increasing DDI. The PPV increased to 5.1% if alerts at onset were suppressed for serum potassium levels of <4.0 mEq/l. The PPV ros
