Study on death certificates and postmortem examination certificates written by Korean emergency physicians.

INTRODUCTION: Statistics on death are the basis of a country's health, safety, and welfare policies. Emergency physicians issue a postmortem examination certificate (PEC) for death outside the hospital as well as a death certificate (DC) for death in the hospital. This study investigated the actual conditions and criteria for writing a DC and PEC, writing environment, and doctor's experience. MATERIAL AND METHODS: The physicians' DC and PEC writing experience and demographic data were analyzed. The questions focused on CPR, patient's medical certificate, time and place of death, difficulty in

Transcranial direct current stimulation enhances motor learning in Parkinson's disease: a randomized controlled trial.

Writing training has shown clinical benefits in Parkinson's disease (PD), albeit with limited retention and insufficient transfer effects. It is still unknown whether anodal transcranial direct current stimulation (atDCS) can boost consolidation in PD and how this interacts with medication. To investigate the effects of training + atDCS versus training + sham stimulation on consolidation of writing skills when ON and OFF medication. Second, to examine the intervention effects on cortical excitability. In this randomized sham-controlled double-blind study, patients underwent writing training (o

Identity construction in medical student stories about experiences of disgust in early nursing home placements: a dialogical narrative analysis.

OBJECTIVES: To explore medical students' reflective essays about encounters with residents during preclinical nursing home placements. DESIGN: Dialogical narrative analysis aiming at how students characterise residents and construct identities in relation to them. SETTING: Medical students' professional identity construction through storytelling has been demonstrated in contexts including hospitals and nursing homes. Some preclinical students participate in nursing home placements, caring for residents, many living with dementia. Students' interactions with these residents can expose them to

The patient's voice and the writer's role in mental health records.

BACKGROUND AND AIM: Over recent decades, formal requirements for medical records have been strengthened, for example through patients' rights of access. However, clinical documentation in mental health services has been criticised for becoming increasingly juridified and for failing to recognise patients as whole persons. The aim of this study was to conduct an exploratory investigation of current record-writing practices in mental health care. MATERIAL AND METHOD: We reviewed ten medical records from patients discharged from the Adult Inpatient Mental Health Unit at the Clinic for Mental Hea

Case writing as a vehicle for promoting cultural competency: a retrospective, descriptive qualitative analysis.

BACKGROUND: A variety of approaches are available that provide cultural competency education for practicing physicians. There is, however, still a need for additional, innovative approaches that address continuing education and professional improvement regarding cultural competency for physicians after their training. OBJECTIVE: To assess the potential impact on established clinicians of writing an extended case narrative on cultural competency. METHODS: We conducted structured interviews of physician contributors (n = 14) to a book of cases on cultural competency. Authors were invited to co

Writing errors and anosognosia in amyotrophic lateral sclerosis with dementia.

Amyotrophic lateral sclerosis (ALS) with dementia (ALS-D) is known to exhibit characteristics of frontotemporal dementia. However, in clinical situations, it is often difficult to evaluate their cognitive functions because of impaired voluntary speech and physical disabilities. In order to identify characteristic and diagnostic cognitive symptoms of relatively advanced ALS-D patients, we retrospectively reviewed the clinical features of seven cases of clinically definitive ALS who had dementia, impaired voluntary speech, and physical disability. Their medical records showed that six out of sev

Physician-assisted dying: thoughts drawn from Albert Camus' writing.

Physician-assisted dying (assisted suicide and euthanasia) is currently an intensely discussed topic in several countries. Despite differences in legislation and application, countries with end-of-life laws have similar eligibility criteria for assistance in dying: individuals must be in a hopeless situation and experience unbearable suffering. Hopelessness, as a basic aspect of the human condition, is a central topic in Albert Camus' philosophical work The Myth of Sisyphus, which addresses the question of suicide. Suffering in the face of a hopeless situation, and the way doctors approach thi

Primary writing tremor responds to unilateral thalamic deep brain stimulation.

Primary writing tremor is a task associated tremor which occurs during and interferes with handwriting. Considered in most cases to be a nonprogressive disorder, a number of patients are significantly debilitated by the disease. The pathophysiology of the disorder is not fully understood, but felt to represent a variant of either essential tremor or dystonia versus a separate entity. Treatment has been limited to medications, writing devices and botulinum toxin type A for most patients. Recently, deep brain stimulation has been reported in a few patients as an effective option for those patien

Papers presented at the fall 2020 Pediatric Urologic Oncology Work Group of the Societies of Pediatric Urology meetingBladder Duplication in a Setting of VACTER Association.

Bladder duplication is a rare anomaly classified as sagittal or coronal, depending on the axis of the septum. VACTER association involves congenital anomalies affecting the vertebrae, anus, heart, trachea, esophagus, kidney and genitourinary system. This is the first description of a case of coronal bladder duplication in a patient that also presented with anomalies on the foregut and other organs that correspond to VACTER association. Pertinent literature is systematically reviewed and compared with our case.

Imaginaries of Writing and Pain: The Backstage of a Ghostwritten Paper.

Ghostwritten medical papers are a cornerstone of pharmaceutical marketing. We trace how one manuscript written under the instructions and supervision of opioid producer Mallinckrodt was produced and eventually published in a medical journal to present a marketing message. We systematically analyse an archive of internal communications made public after litigation through the lens of Ricœur's work on imaginaries. Along his first analytic axis, we show how the ghostwriting process shapes and reshapes identities, ascribing agency and portraying social order. Along the second, legitimacy was const

Writing tremor after discrete cortical infarction.

BACKGROUND: Hand tremor is a rare manifestation of stroke, and writing tremor has not been reported to be produced by stroke. We describe a patient who developed a unilateral hand tremor mimicking primary writing tremor after discrete cerebral cortical infarction. CASE DESCRIPTION: A 67-year-old man developed mild right hemiparesis. Brain magnetic resonance imaging showed a discrete cortical infarct in the left frontal area. After recovery of motor power, the patient showed significant right hand tremor exclusively during writing or similar motor activities including tooth brushing or shaving

Grid to glide: A simple visual cue that alleviates upper-limb freezing.

Freezing of the upper limb (FoUL) is a poorly explored manifestation of Parkinson's disease. This report describes a case of a 54-year-old woman with FoUL predominantly affecting handwriting. Writing on grid paper with small squares (0.5 × 0.5 cm) markedly reduced FoUL frequency. A subsequent daily home-based writing and calligraphic training program led to sustained improvements in handwriting legibility, drawing ability, and transfer to writing on blank paper without the grid, without any change in medication. The immediate response to grid paper suggests that simple visual cueing-possibly b

Case-control study of writer's cramp.

Task-specific focal dystonias are thought to be due to a combination of individual vulnerability and environmental factors. There are no case-control studies of risk factors for writer's cramp. We undertook a case-control study of 104 consecutive patients and matched controls to identify risk factors for the condition. We collected detailed data on medical history and writing history as part of hobbies or occupation. Cases had a college or university degree more frequently than controls [OR = 4.6 (1.3-20.5), P = 0.01]. The risk of writer's cramp increased with the time spent writing each day (

The relevance of queries and coding procedures to the writing of cause-of-death statements.

A cause-of-death statement may result in a query by the vital records registrar to clarify cause of death information or to educate the certifier of death in recommended death certification procedures. The level of querying depends on local need and resources, and is usually conducted at one of six query priority levels. When applying International Classification of Disease (ICD) coding to cause-of-death information, nosologists use a General Rule and 12 additional rules, as needed, to identify an underlying cause of death for statistical purposes. Incorrectly or poorly written cause-of-death

Aspects of activities and participation of 7-8 year-old children with an obstetric brachial plexus injury.

BACKGROUND: Children with an obstetric brachial plexus injury (OBPI) can experience problems in the performance of meaningful activities such as writing, bimanual activities, and participation in sports and leisure activities. AIMS: To quantify the everyday functioning and participation of 7-8 year-old children with an OBPI, with special emphasis on writing, and to investigate associated characteristics. METHODS: Parents of children with an OBPI were sent a self-report questionnaire regarding the school performance, writing abilities, bimanual hand use, and participation in sports and leisur

Occupational Protection in Interventional Radiology. A Joint Guideline of the Cardiovascular and Interventional Radiological Society of Europe and the Society of Interventional Radiology.

PURPOSE: Since the initial edition of this Guideline was published in 2010, it has become increasingly clear that occupational protection involves more than just radiation protection. Additionally, radiation exposure to operators and staff occurs from sources other than fluoroscopy. This Guideline has been updated to incorporate these topics and to reflect new information on occupational protection in interventional radiology. METHODS: The CIRSE Standards of Practice Committee, in conjunction with SIR, established a writing panel of twelve clinicians, medical physics experts, and others with

The shorthand publications of Sir William Richard Gowers.

OBJECTIVE: To examine the shorthand publications of Sir William Richard Gowers. BACKGROUND: Gowers developed an almost obsessional interest in Pitman shorthand. During the later part of his active career (1894-1910), the bulk of his professional writing, comprising nearly 100 articles, was printed entirely in Pitman shorthand in the Phonographic Record of Clinical Teaching and Medical Science. The obscurity and rarity of this periodical, and the increasingly arcane nature of Pitman shorthand, has left the bulk of these articles "buried in obscurity and locked up in code" (M. Critchley, 1949).
