Assessment and categorization of medication errors in a veterinary teaching hospital.

BACKGROUND: Medication errors are one of the most common types of errors in health care; however, similar data are limited to veterinary institutions. Pharmacists are not always present in veterinary hospitals. Therefore, all staff members actively engaged in the dispensation of medications must be aware of the high incidence of medication errors. OBJECTIVES: This study aimed to quantify the amount of and assess the types of medication errors that occur frequently within a veterinary teaching hospital (including both large and small animal patients), identify the specific medications that are

Clinical and economic outcomes associated with medication errors in kidney transplantation.

BACKGROUND AND OBJECTIVES: Modern immunosuppressant regimens have significantly decreased acute rejection rates, but may have increased the risk of graft loss driven by adverse drug reactions (ADRs) and medication errors (MEs). The objectives of this study were to determine the incidence and risk factors for MEs and ADRs and determine the association between transplant outcomes and these events. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: This was a post hoc analysis of a prospective, randomized trial that included patients aged>18 years that received a solitary renal transplant at an acad

Monitoring the patient with diabetes mellitus--how to avoid medication errors.

The hospital pharmacist is in a unique position to assist in the management and monitoring of the diabetic patient. He can alert the nurse and physician concerning medication errors involving the use of insulin and oral sulfonylureas. Medication errors may not only jeopardize the metabolic control of a person with diabetes but may also result in the development of significant hypoglycemia. Insulin orders should be written clearly and concisely. The advocation for intensive split-dose regimens and the use of insulin infusion devices require careful attention to technique, administration times a

Understanding Medication Errors Through Patient Complaints in a Danish University Hospital.

Patient complaints are an important source of data, as they may help illuminate medication safety issues from the patient's perspective. We aimed to identify and describe medication errors (MEs) reported in patient complaints and compare them with MEs from safety incident reports. This descriptive hospital-based study (1) compared patient complaints with and without MEs using the Health Complaint Analysis Tool (HCAT) and (2) analysed MEs identified in both patient complaints and incident reports using the standard taxonomy of the reporting system. Of 6301 complaints, 310 (4.9%) were related to

Medication Errors in Overweight and Obese Pediatric Patients: A Narrative Review.

BACKGROUND: The childhood obesity epidemic in the United States has increased utilization of health care and prescribing of medications in overweight and obese children, yet it is unclear whether this has led to more medication errors. The objective of this study was to review all available literature on incidence and types of medication errors in overweight and obese children. METHODS: A search of MEDLINE, Embase, and Scopus databases was conducted for all studies and oral abstracts through December 2020 reporting medication errors in overweight or obese children aged ≤ 18 years. All studies

Reporting of Medical Errors in Autopsied Cases.

This report describes the cases autopsied at B. P. Koirala Institute of Health Sciences- Nepal, during the period of two years in which the treatment provided falls below the accepted standard practice causing the death of the patient. Postmortem examination provides an opportunity to find out the actual cause of death in which the cause of death is suspicious. Sometimes the cause of death found through postmortem examination is different from the clinical cause of death. The autopsy finding on the cause of death will help doctors even to think about unusual conditions that lead to death and h

Ambiguity and workarounds as contributors to medical error.

Why are some organizations error-prone-regularly subject to interruptions and inconveniences, some of which periodically coalesce catastrophically-whereas other organizations, although similar in the products and services they generate and the process technologies they use, are reliable, adaptable, and continuously self-improving, relentlessly learning from experience to get ever better? Analyzing medical error reports and studies of high-performing, non-health care organizations reveals 2 differences. High performers know how to prevent problems from producing further consequences once they o

Use of complete medication history to identify and correct transitions-of-care medication errors at psychiatric hospital admission.

Methods for categorizing the scale and severity of medication errors corrected by pharmacy staff during admission medication reconciliation using complete medication history continue to evolve. We established a rating scale that is effective for generating error reports to health system quality leadership. These reports are needed to quantify the value of investment in transitions-of-care pharmacy staff. All medication errors that were reported by pharmacy staff in the admission medication reconciliation process during a period of 6 months were eligible for inclusion. Complete medication histo

Iatrogenic intravenous medication errors reported to the GIZ-Nord Poisons Center Göttingen.

BACKGROUND: We analyzed the iatrogenic intravenous medication errors (IIME) reported to the GIZ-Nord Poisons Center Göttingen. Medication errors in paediatric outpatient prescriptions of a teaching hospital in Nigeria.

BACKGROUND: Proper and adequate pharmacovigilance is lacking in children's drug therapy, especially in developing countries. Therefore these children are at risk of developing adverse reactions to drugs as a result of medication errors. Having a better understanding of the types of error in a teaching hospital would be an important step toward quality improvement in children drug therapy. OBJECTIVES: This study was aimed at identifying the medication errors most frequently committed in the paediatric outpatient prescriptions at the Lagos State University Teaching Hospital, Ikeja and to sugges

A Review of Medication Errors and the Second Victim in Pediatric Pharmacy.

The concept of the second victim, described as the sense of victimization of health care professionals following the exposure to a traumatic, unanticipated medical error, was first introduced in 2000 by Albert W. Wu. Since then, the concept has gained immense traction and inspired the generation of assistance programs for second victims. With most second victim occurrences resulting from medication errors, pediatric pharmacists are at a high risk of experiencing second victim phenomenon. Second victims may experience both psychological and physical symptoms of distress often akin to post-traum

Risperidone medication errors in children: an analysis of French poison centres data.

OBJECTIVES: To describe clinical consequences of risperidone medication errors in children of less than 13 years and to estimate a clinically relevant toxic dose. METHODS: All cases of risperidone medication errors managed by French Poison Centres from 2001 to 2012 were analyzed. Inclusion criteria were a delay of at least 2 hours between ingestion and request to the FPC in asymptomatic children, an ingested dose above two-fold the maximal daily dose for children above 5 years or any symptomatic patient at the time of first contact. RESULTS: One hundred and sixty cases met our criteria. Medi
