Twenty-one-day treatment cycles of ethynylestradiol or mestranol at dosages of 50 to 100 mug per day were administered to 191 normal volunteer women from six cycles, followed by six cycles of this estrogen treatment combined with 2.5 mg. of norethindrone acetate, 2 mg. of megestrol acetate, or 0.5 mg. of norgestrel. Cimicifuga racemosa (CR) is widely used in the treatment of menopausal symptoms. Mechanistic studies suggest that unlike hormone-replacement therapy, CR does not stimulate estrogen-receptor positive breast cancer cells. To evaluate CR safety, we performed an in vivo investigation of a clinically tested isopropanolic CR extract. One hundred and eighty-five women were followed up for periods up to 15 months (a total of 964 menstrual cycles) to determine the contraceptive efficacy and side effects of 2 mg. norethindrone with 0.1 mg. mestranol, as well as other therapeutic benefits. No pregnancies occurred in those using this method alone for contraception. Side effects were minimal. This low dosage was also useful in the management of dysmenorrhea, menorrhagia and irregular menses.

The nipples of male tan guinea pigs were used to test the effects on the skin of oral contraceptive pills and drugs. The daily topically applied doses were 1/2,000 of a pill. After 3 weeks, the nipples treated with solutions of ethinyloestradiol-containing pills had grown (200-300%), the epidermis showed acanthosis (150-250%) and strong hyperpigmentation. Mestranol-containing pills as well as pure progestogen substances did not produce these effects.

The chronological course of serum triglycerides was determined in 12 girls aged 9--10 years, who have been cyclically treated with mestranol (80 microgram/d) and chlormadinonacetate during 3 1/2 years on average, because of constitutional tall stature. A significant increase of the triglycerides level from 80 to 130 mg/100 ml (0.91 to 1.49 mmol/l) could be noted, followed by normalisation within 6 months. Mechanism and metabolic risk of the reversible increase of serum triglycerides are discussed.

Nearly 50 years ago, the introduction of Enovid (norethynodrel 10 microg and mestranol 150 microg), which provided convenient and reliable contraception, revolutionized birth control. Reports of interactions between oral contraceptives (OCs) and other drugs began to trickle into the literature. At first, these drug interactions appeared to be random and unrelated. Increased understanding of P450 enzymes and phase II reactions of sulfation and glucuronidation has permitted preliminary categorization and assessment of the clinical relevance of these drug interactions.

Since 1973 a number of investigators have reported an association between liver neoplasia and steroid usage. Through referral material we have examined the histology of over 250 cases of hepatic neoplasia, most in patients receiving steroid medications. The majority have been benign, predominantly focal nodular hyperplasia (55%) and hepatocellular adenoma (39%). In 9 patients with Turner's syndrome (karyotype 45/X0) aged from 23 to 50 years (average age 34 years) a curettage was performed. A study of the endometrial picture in 75 volunteer multiparous women, fitted with spring-coil devices loaded with silastic capsules containing 60 mg crystalline mestranol at the birth control clinic of Ain Shams University Hospitals was carried out. Thirty-eight post-insertion endometrial biopsies were taken, both in the proliferative and secretory phases of the cycle, up to a 12-week period. 100 women who had taken part in a prospective controlled trial of oestrogen therapy for prevention of post-oophorectomy bone loss were reviewed after a median follow-up period of nine years.
